BACKGROUND: Postoperative seroma is the most frequent complication of modified radical mastectomy (MRM) with axillary lymph node dissection (ALND). Quilting sutures mechanically obliterate the dead space between the mastectomy flaps and chest wall; however, prior syntheses included heterogeneous breast procedures. We evaluated quilting versus conventional closure after MRM with ALND using only randomized evidence. METHODS: The protocol was registered with PROSPERO (CRD420251237379). We searched MEDLINE (PubMed), Embase, Scopus, Web of Science, CENTRAL, ClinicalTrials.gov, WHO ICTRP, and Google

BACKGROUND: A single injection of local anaesthetic (LA) in the erector spinae plane block (ESPB) can reduce pain after modified radical mastectomy (MRM) surgery, but the duration of analgesia is affected by the duration of the LA. The aim of this study is to investigate the effect of continuous ESPB on acute and chronic pain and inflammatory response after MRM surgery.
METHODS: In this prospective, randomised, controlled trial, we will recruit 160 patients, aged 18-80 years, scheduled for elective MRM surgery under general anaesthesia. They will be randomly assigned to two groups: a continuo

INTRODUCTION: For patients with breast cancer receiving preoperative neoadjuvant chemotherapy with anthracyclines, there is an increased risk of postoperative myocardial injury due to the cardiotoxicity of the chemotherapeutic agents. The optimal intraoperative blood pressure regulation regimen for these patients is unclear. This study is being conducted to determine whether targeting mean arterial pressure (MAP) to 100%-120% of the patient's baseline blood pressure reduces the incidence of myocardial injury after non-cardiac surgery (MINS) compared with targeting MAP to 80%-100%. MINS is defi

INTRODUCTION: Pain after breast cancer surgery is one of the main reasons for postoperative morbidity and pulmonary complications leading to increased hospital stay. Therefore, effective management of postoperative pain becomes necessary to alleviate patients' suffering and allow early mobilization and hospital discharge. Traditionally, opioids have been used to manage perioperative pain but they are associated with side effects. So, an opioid-sparing multimodal analgesia approach is used nowadays. Ultrasound-guided pectoral type-II (PEC II) block is increasingly being used to address acute po

A 52-year-old female underwent radical mastectomy at the age of 41 for left breast cancer (n0, positive for ER). After a few years of adjuvant TAM therapy, follow-up was stopped at the age of 50 with no recurrence. She had suffered from symptoms of cold since January 2001 and came to our hospital complaining dyspnea on February 11. CXP showed pleural effusion of the entire thoracic cavity and she was admitted to the hospital immediately. Pleural exudate cytodiagnosis showed carcinomatous pleurisy; however, dyspnea and thoracic effusion were improved by continuous thoracic drainage and instilla

We evaluated wound complications and potential risk factors after mastectomy with immediate breast reconstruction and compared them with similar data after modified radical mastectomy. The incidences of infection, seroma, hematoma, and epidermolysis were compared among 395 patients (305 with modified radical mastectomies and 90 with mastectomy with immediate breast reconstruction) from Virginia Mason Medical Center, Seattle, Wash, between 1983 and 1989. Obesity, age (60 years or older), smoking, antibiotics, and wound drainage were examined as possible risk factors. There were more wound compl

BACKGROUND: The quality of life of Mexican patients with breast cancer has been studied, but female sexual function has only been explored superficially. The Female Sexual Function Index (FSFI) questionnaire has been validated as a tool to measure sexual function among women with cancer. However, no study in Mexico has been published.
PURPOSE: To evaluate and compare female sexuality of breast cancer survivors treated with three surgical procedures.
MATERIALS AND METHODS: This is a cross-sectional questionnaire survey applied in patients without active disease and free of any oncologic treat

WHAT IS KNOWN AND OBJECTIVE: A previous randomized clinical trial concluded that an optimal concentration of 0.3% ropivacaine could provide satisfactory analgesia for breast cancer patients undergoing modified radical mastectomy. We wondered if a smaller volume (30 ml vs. 40 ml) of 0.3% ropivacaine could still provide adequate analgesia in an ultrasound-guided PECS II block in modified radical mastectomy.
METHODS: We performed a prospective parallel randomized double-blind controlled clinical trial. Eligible patients were assigned to either the P30 or P40 group (30 or 40 ml of 0.3% ropivacain

For about three quarters of a century, radical mastectomy was considered standard initial treatment for invasive breast cancer. Within the past 15 years, modified radical mastectomy (total mastectomy with axillary dissection) has replaced radical mastectomy as standard treatment, even though superiority of the former has not been scientifically proven, with the exception of a single study. During the same period, total mastectomy with postoperative irradiation of axillary lymph nodes came into vogue, and debate centered on whether irradiation of nodes was as efficacious as surgical extirpation

The use of a contact Nd:YAG laser scalpel during radical and modified radical mastectomy was evaluated in 18 patients with carcinoma of the breast. The laser scalpel performed well as a haemostatic tool, the associated mean blood loss for modified radical mastectomy being 132 ml. Operating time, operative blood loss, laser energy required and postoperative wound drainage were all related to patient obesity, correlating significantly with body weight and/or breast weight. The incidence of axillary seroma was not reduced by laser surgery and occurred in 53% of patients undergoing modified radica

There is no ideal single operation for breast cancer. In planning the choice of surgery for breast cancer, one must be aware of its multicentric origin, and of the regional spread from the breast to the axillary and internal mammary lymph nodes. The scope of the surgical attack should be correlated with the clinical pathologic extent of disease in the individual patient with the aim of removing all disease present, while preserving appearance and function to the utmost. The main goal remains removal of all disease from the breast and its regional nodes. Three distinct operative procedures have

The treatment of potentially curable carcinoma of the breast has changed from one operation, radical mastectomy, to a flexible approach. At the Cleveland Clinic, we use four types of treatment for primary potentially curable carcinoma of the breast (Stages 0, I and II)--modified radical mastectomy, simple mastectomy, partial mastectomy with postoperative adjuvant radiation therapy and partial mastectomy without radiation therapy. The latter treatment (partial mastectomy without adjuvant radiation) is controversial. We recommend this procedure for patients with T(is) and T1 carcinomas that appe

Two groups of patients with central or medical primary breast cancer (T1-2N0-1M0) were studied. One group of 478 patients had an extended radical mastectomy (Urban-Kholdin) that included removal of the parasternal lymph nodes and adjoining costal cartilages. A second group of 519 concurrently treated patients had a conventional radical mastectomy (Halsted-Meyer). Among the patients who had an extended radical mastectomy, metastases were found in parasternal lymph nodes in 17.7% of those who had no metastasis in axillary or subclavicular lymph nodes, 40.9% of those who had a single metastatic f

A retrospective study is presented on 225 selected patients with local primary carcinoma of the breast treated from 1963 to 1973 at the Surgical Clinic of the Cantonal Hospital, St. Gallen, Switzerland. All patients were under 70 years old. The results are compared of our two therapies, namely radical mastectomy of Halsted and a modified radical mastectomy consisting of mastectomy with dissection of the axilla but without removing the pectoral muscle. Both operations were followed by postoperative irradiation. In view of the short postoperative interval after modified radical mastectomy (from

For public and professional acceptance, clinical studies in breast cancer must be scientifically necessary and ethically justifiable. To resolve controversy based upon retrospective data, they must be prospective and randomized. These guidelines have been used by the National Surgical Adjuvant Breast Project in its investigations carried out during the past 15 years in numerous cooperating institutions. Neither thio-tepa adjuvant to radical mastectomy, nor prophylactic oophorectomy, nor post-operative radiation therapy were found to prolong life. Therefore, prophylactic oophorectomy is contrai

Nipple-sparing mastectomy (NSM) is a surgical protocol designed to reduce the disabling psychological effects of radical or skin-sparing mastectomy. The preservation of the nipple-areola complex produces a more-natural result of the breast reconstruction, but this preservation is suspected of increasing tumor local recurrence. To reduce this risk, different approaches have been proposed: restrict the inclusion criteria and/or add localized radiation therapy. The local recurrence rate in recent series of patients receiving NSM is comparable with the local recurrence rate in modified radical or

OBJECTIVE: To summarize the experience of diagnosis and treatment of occult breast cancer in 44 cases.
METHODS: Clinicopathological data of 44 cases of occult breast cancer initially presenting axillary mass alone treated in our department during Jan 1997 to Dec 2008 were retrospectively analyzed.
RESULTS: The 44 patients with occult breast cancer accounted for 0.42% of all breast cancer patients admitted to our hospital and institute in the same period. The surgery included radical mastectomy in 16 cases, modified radical mastectomy in 19 cases, axillary clearance in 1 case, and simple axil

Strontium nitrate Sr 87m bone scans were made preoperatively in a group of women with suspected breast cancer, 35 of whom subsequently underwent radical mastectomy. In 3 of the 35 (9%), the scans were abnormal despite the absence of clinical or roentgenographic evidence of metastatic disease. All three patients has extensive axillary lymph node involvement by tumor, and went on to have additional bone metastases, from which one died. Roentgenograms failed to detect the metastases in all three. Occult bone metastases account in part for the failure of radical mastectomy to cure some patients wi
