Management of Primary Necrotizing Fasciitis of the Breast: A Systematic Review.

PURPOSE: Necrotizing fasciitis (NF) is a life-threatening infection that involves spreading necrosis of the subcutaneous tissue and fascia that affects the extremities, abdominal wall, and perineum. Primary infection of the breast is a rare occurrence. Shah et al described the first case of primary breast NF and recommended radical "pseudotumor" excision and delayed skin closure months after resolution. Numerous other cases reported were successfully managed with different strategies. We aimed to summarize management options for primary breast NF through a systematic review of the literature.

Preoperative breast MRI: reproducibility and significance of findings relevant to nipple-areolar complex involvement.

BACKGROUND: Eligibility of nipple-sparing mastectomy has been expanded. The purpose of this study was to evaluate interobserver agreement regarding magnetic resonance imaging (MRI) descriptors important in determining eligibility for mastectomy, and to investigate the significance of enhancement extending to the areola concerning nipple-areolar complex (NAC) involvement. METHODS: Fifty-one cases with histologically confirmed NAC involvement and 54 cases with negative NAC were enrolled. Two radiologists assessed the following factors: lesion morphology (mass or non-mass enhancement); intra-nip

Breast reconstruction: current state of the art.

The current state of the art makes breast reconstruction possible in any patient who has undergone mastectomy, whether simple, modified radical, or radical. Indications for reconstruction are a strong desire on the part of the patient and a reasonable life expectancy. Methods of reconstruction include the simple placement of a breast prosthesis alone (submuscularly when possible) or in combination with advancement of upper abdominal skin or rotation flaps. Expander implants are useful in patients with limited skin and musculature. For the patient with serious muscle and skin deficits, more com

Breast rescue after necrotizing fasciitis.

Necrotizing fasciitis is a dangerous and rapidly spreading infection of soft tissue involving skin, subcutaneous tissue and fascia; muscles can be concerned but often omitted. It's considered as emergency due to its fulminant nature. The necrotizing fasciitis of the breast is exceptional. Management is based on surgical debridement and, in the case of breast, mastectomy in most cases is inevitable. We describe a case-report of breast necrotizing fasciitis with prompt management and with satisfactory cosmetic result owing to dermal autograft.

An atypical case of necrotizing fasciitis of the breast.

Necrotizing fasciitis is a rare and aggressive soft tissue infection involving the fascia and subcutaneous tissues. It carries a high mortality and morbidity rate. In literature, the few case reports on necrotizing fasciitis of the breast, describe the need for a mastectomy in 90% of the cases. We report on a case of a 72-year old Caucasian women with an atypical presentation of necrotizing fasciitis of the breast in combination with an acute abdomen, successfully treated with breast-conserving debridement and secondary wound closure.

Nonpalpable breast cancer.

Thirty-seven patients with nonpalpable breast cancer treated between 1981 and 1992 were analysed. Twenty-five (68%) of the patients were detected by microcalcification of mammogram, 8 by nipple discharge and 4 by other means. Biopsy was carried out in all patients to get a final diagnosis and histopathological examinations were performed using continuous sections (especially the section with microcalcification). The incidence of noninvasive carcinoma was 59% (22 cases) and the remaining invasive carcinomas were 3 minimal lesions less than 0.5 cm, 3 lesions from 0.5 to 1.0 cm, 3 diseases more t

Immunoglobulin localization in benign and malignant lesions of the human mammary gland.

Using direct imunofluorescence, lesions from 266 human breast specimens were studied for the presence of IgA, IgM, or IgG localization. The lesions included benign elements from 66 subcutaneous mastectomy specimens in which the absence of simultaneous breast malignancy was documented, primary breast carcinomas from 153 mastectomy specimens, and 47 biopsies containing metastatic breast cancer. A statistically significant association of IgA and IgM with benign lesions was contrasted to the association of IgG with malignant lesions. In both primary and metastatic lesions, IgG localization was ass

Prepubertal gynecomastia with lobular differentiation.

We report the case of a 19-month-old African American boy with a unilateral breast mass that developed shortly after birth. A sonogram confirmed the presence of a "fatty" breast mass. Microscopy of the subcutaneous mastectomy specimen revealed florid gynecomastia with marked ductal hyperplasia and focal lobular differentiation. This exceptional example of prepubertal gynecomastia with lobular differentiation has been previously reported only four times in the literature. We report a fifth patient, as well as the youngest, with this unusual histologic finding.

[Breast areola and nipple sensitivity after reduction mammoplasty].

Considering the female breast to be a secondary sexual organ the sensibility of nipple and areola gains essentially in functional significance. This sensory function is impaired as well by reduction mammaplasty. Thus all different operation methods propagated in recent years have to be rated as to functional and esthetic results with regard to resection weight. The optimal operation method presupposes: 1. Satisfactory esthetic result, 2. optional reductional possibility up to subcutaneous mastectomy, 3. minor sensory impairment of nipple and areola.

Bilateral Prophylactic Nipple-Sparing Mastectomy: Analysis of the Risk-Reducing Effect in BRCA1/2 Mutation Carriers.

BACKGROUND: Mutations in the BRCA1 or BRCA2 genes increase the lifetime risk of developing breast cancer to 68-72% by the age of 80. One of the modalities to manage the risk is a prophylactic mastectomy. Bilateral nipple-sparing mastectomy specifically offers the most favorable esthetic outcomes but the evidence for its oncological safety remains limited. Thus, we aimed to compare the occurrence of breast cancer between nipple-sparing mastectomy and surveillance groups of BRCA1 or BRCA 2 mutations carriers. MATERIALS AND METHODS: BRCA1 or BRCA2-positive patients undergoing bilateral prophylac

Efficacy of bilateral prophylactic mastectomy in women with a family history of breast cancer.

BACKGROUND: Options for women at high risk for breast cancer include surveillance, chemoprevention, and prophylactic mastectomy. The data on the outcomes for surveillance and prophylactic mastectomy are incomplete. METHODS: We conducted a retrospective study of all women with a family history of breast cancer who underwent bilateral prophylactic mastectomy at the Mayo Clinic between 1960 and 1993. The women were divided into two groups - high risk and moderate risk - on the basis of family history. A control study of the sisters of the high-risk probands and the Gail model were used to predic

Computer aided detection of deep inferior epigastric perforators in computed tomography angiography scans.

The deep inferior epigastric artery perforator (DIEAP) flap is the most common free flap used for breast reconstruction after a mastectomy. It makes use of the skin and fat of the lower abdomen to build a new breast mound either at the same time of the mastectomy or in a second surgery. This operation requires preoperative imaging studies to evaluate the branches - the perforators - that irrigate the tissue that will be used to reconstruct the breast mound. These branches will support tissue viability after the microsurgical ligation of the inferior epigastric vessels to the receptor vessels i

Multimodality Imaging of Postmastectomy Breast Reconstruction Techniques, Complications, and Tumor Recurrence.

For women undergoing mastectomy, breast reconstruction can be performed by using implants or autologous tissue flaps. Mastectomy options include skin- and nipple-sparing techniques. Implant-based reconstruction can be performed with saline or silicone implants. Various autologous pedicled or free tissue flap reconstruction methods based on different tissue donor sites are available. The aesthetic outcomes of implant- and flap-based reconstructions can be improved with oncoplastic surgery, including autologous fat graft placement and nipple-areolar complex reconstruction. The authors provide an

Factors affecting post-operative complications following skin sparing mastectomy with immediate breast reconstruction.

UNLABELLED: Skin sparing mastectomy (SSM) followed by immediate breast reconstruction (IBR) is not only oncologically safe but provides also significant benefits both cosmetically and functionally. The superiority of this technique can only be fully established, however, by developing a framework for minimising complications. The present study seeks to elucidate the key factors affecting outcome. METHODS: Data for all skin sparing mastectomies with immediate autologous and implant based reconstructions, performed in a three year period (2006-2008) was retrospectively collated. Complications w

The surgical management of early breast cancer.

Definitive surgical treatment should never be undertaken without a histological confirmation of the diagnosis. For operable disease primary treatment is normally breast-conserving surgery or mastectomy, in association with axillary surgery. Patients with larger technically operable tumours may be treated with primary chemotherapy to try to shrink the tumour so that less radical surgery can be undertaken. The objective of surgery is clearance of local and regional disease with a margin of clear tissue, and to achieve an acceptable cosmetic outcome. Clinical assessment of the axilla is unreliabl

Eccentric skin resection and purse-string closure for skin reduction with mastectomy for gynecomastia.

Skin redundancy following mastectomy for gynecomastia does not shrink and resolve in a predictable manner. Excess skin can be addressed best with resection at the time of the original operation if the scar can be minimized. Previously described techniques for resection of skin have shortcomings that include unacceptable scars, poor nipple positioning, and bulkiness of redundant, folded dermal tissue. The current technique for grade 2 and some grade 3 gynecomastias is performed with the creation of a 20-mm-diameter nipple-areolar complex based as a cephalad flap and designed eccentrically aroun

Body contouring.

The prospect of surgical body contouring is a rather recent innovation for the surgeon. For underdeveloped or atrophic breasts, augmentation mammaplasty is reasonably successful. For ptosis and/or hypertrophy of the breast, reduction and elevation on the chest wall by making the breasts more conical and more projecting is a common procedure for the plastic surgeon. Breast reconstruction is now done after either simple or radical mastectomy, and subcutaneous mastectomy procedures, preserving the nipple and areola, are also commonly done for disease of the breast which is not yet malignant. The

Subcutaneous Implant-based Breast Reconstruction with Acellular Dermal Matrix/Mesh: A Systematic Review.

BACKGROUND: The availability of acellular dermal matrix (ADM) and synthetic mesh products has prompted plastic surgeons to revisit subcutaneous implant-based breast reconstruction. The literature is limited, however, with regards to evidence on patient selection, techniques, and outcomes. METHODS: A systematic review of the Medline and Cochrane databases was performed for original studies reporting breast reconstruction with ADM or mesh, and subcutaneous implant placement. Studies were analyzed for level of evidence, inclusion/exclusion criteria for subcutaneous reconstruction, reconstruction

Bilateral nipple-sparing mastectomy and breast reconstruction in BRCA1 mutation-positive simultaneous bilateral breast cancer: A case study.

BACKGROUND: Mutation-positive patients who develop unilateral breast cancer require different treatments, such as prophylactic mastectomy of the contralateral breast, from those used for other breast cancer patients. If a mutation is found before surgery, it is necessary to consider a surgical procedure that includes reconstruction. For BRCA mutation-positive patients, a suitable treatment must be selected. In Japan, a test for BRCA mutation has been covered by health insurance since 2020, making it possible to preoperatively test patients who are suspected of being positive. We report a case
