Aspergillus mastoiditis in acquired immunodeficiency syndrome.

OBJECTIVE: This study aimed to analyze the clinical presentation, diagnosis, management, and results of treatment in a series of three patients with acquired immunodeficiency syndrome (AIDS) in whom Aspergillus mastoiditis developed. This study also aimed to compare these aspects of Aspergillus mastoiditis in patients with AIDS with three additional cases present in the current literature. A classification system for fungal infections of the ear and temporal bone is proposed. STUDY DESIGN: The study design was a retrospective case review. SETTING: The study was conducted at multiple tertiary…

[Problems of diagnosis and treatment of atypical mastoiditis].

The specific features of the diagnosis and therapy of the dry form of atypical mastoiditis are discussed. Of great diagnostic importance were local changes in the mastoid area, otoscopic signs in the posterior-superior tympanic membrane and bone compartment of the auditory meatus, hearing impairment, thermographic signs of purulent-destructive lesions in the antrum and other cells of the mastoid process, and X-ray symptoms of bone destruction of the mastoid process. It is obvious that the clinical picture and development of atypical mastoiditis are closely related to the age and reactivity of …

[Acute mastoiditis: 16 clinical cases].

Sixteen cases of acute mastoiditis were treated between 1980 and 1984. In infants the course of the mastoiditis was brief. An external retroauricular swelling soon occurred that was generally due to edema without abscess formation, and medical treatment resulted in rapid healing. For the older children, the acute mastoiditis occurred on an acute otitis media that had already been present for some time. Abscess formation occurred in half of the cases, and a mastoidectomy was generally required. For the adult patients, there was always an underlying chronic ear pathology, either a cholesteatoma …

Acute mastoiditis: a review of 34 patients.

A review of 34 patients presenting with acute mastoiditis was undertaken. Prior treatment was noteworthy for lack of medical or surgical drainage at the time of initial otitis media. Typical findings, in hospital, included postauricular edema and tenderness, displaced auricle, and distorted tympanic membrane. X-rays were not diagnostic, nor were they helpful in determining who was a surgical candidate. Treatment was individualized. Patients with obvious subperiosteal abscesses, and those who did not respond rapidly to enteric antibiotics, underwent early mastoid surgery. The remaining patients…

A Child With Retroauricular Tenderness: Is It Really Mastoiditis?

The presence of retroauricular tenderness and erythema has always been closely linked to a diagnosis of acute mastoiditis (AM), a condition that has become less common over the years given the advent of antibiotics and pneumococcal vaccination. However, other etiologies such as retroauricular cellulitis can also present similarly. We present the case of a 10-year-old patient who presented with outward and downward protrusion of the left ear with retroauricular tenderness and erythema and was initially presumed to have a diagnosis of AM and admitted for intravenous antibiotic management. Imagin…

Outpatient management of pediatric acute mastoiditis.

OBJECTIVE: Evaluate the Montreal Children's Hospital experience with outpatient management of uncomplicated acute mastoiditis with parenteral antibiotic therapy alone and determine if it is a safe alternative to inpatient management. SUBJECTS AND METHOD: A retrospective review of pediatric patients diagnosed with acute mastoiditis at a tertiary care pediatric hospital between 2013 and 2015 was performed. Patients with syndromes, immunodeficiency, cholesteatoma, chronic otitis media, cochlear implant in the affected ear, or incidental mastoid opacity were excluded. RESULTS: 56 children age 6 …

Bilateral inflammatory myofibroblastic tumor mastoiditis.

BACKGROUND: Inflammatory myoblastic tumors (IMTs) are rare, mesenchymal neoplasms most commonly located in areas of previous inflammation. In the temporal bone, these tumors typically present as solitary, space-occupying lesions that clinically resemble chronic otitis media. METHODS: We present a case of bilateral IMTs with facial nerve involvement, sigmoid sinus thrombosis, leptomeningeal enhancement, and seizures. RESULTS/CONCLUSION: This case demonstrates the need for a high index of suspicion of IMT when imaging reveals bilateral mastoid opacification. These tumors are more aggressive th…

Challenges in the management of acute mastoiditis in children.

Acute mastoiditis (AM) is a rare but serious complication of otitis media. Recent studies consolidated the role of Streptococcus pyogenes as the second most important etiologic agent of AM and suggested an increased involvement of a relatively new pathogen (Fusobacterium necrophorum). The recently accumulated evidence on AM epidemiology in children is conflicting and not convincing in demonstrating clear trends during the last years. While a significant decrease was recorded in the incidence of pneumococcal invasive disease and complicated AOM following the introduction of the pneumococcal con…

Dural enhancement and thickening in acute mastoiditis.

Dural enhancement and thickening in imaging studies observed in acute mastoiditis patients is an uncommon phenomenon. It is infrequently seen in dural sinus thrombosis, and may be caused by infiltration of inflammatory cells and an increased number of thin-walled blood vessels. We present a three-year-old boy who presented with acute mastoiditis, complicated by subperiosteal abscess. Computerized tomography (CT) demonstrated subperiosteal abscess, and the child underwent mastoidectomy. Despite adequate treatment, symptoms worsened and neurological sequelae were suspected. CT and magnetic reson…
