Neuroendoscopic Resection of Trigeminal Schwannoma in the Pterygopalatine/Infratemporal Fossa via the Transnasal Perpendicular Plate Palatine Bone or Transnasal Maxillary Sinus Approach.

BACKGROUND: Both the pterygopalatine fossa (PPF) and the infratemporal fossa (ITF) lie outside the midline of the skull base. Lesions in the PPF or ITF include trigeminal schwannoma (trigeminal schwannoma, TS), which originates from the second or third branch of the trigeminal nerve (maxillary nerve or mandibular nerve). Due to their typically deep anatomic location, lesions in the PPF or ITF can be difficult to treat using traditional surgical approaches. In recent years, because of their advantages, which include the fact that they allow the problem to be observed close up, neuroendoscopic t…

[Solitary neurofibroma of the maxillary sinus].

Neurofibromas are benign tumours of peripheral nerve tissue, frequently associated with neurofibromatosis type I. Their isolated occurrence in the maxillary sinus is rare, with only 6 cases described in the literature. We present the case of a 70-year-old woman who consulted for left hemifacial paresthesias of 10 years' evolution. CT showed a heterogeneous lesion in the left maxillary sinus, eroding its lateral wall without signs of infiltration. Endoscopic surgery with nasalisation of the maxillary sinus was performed. The pathology was neurofibroma (positive for S-100 protein and vimentin).

Extradural extranasal combined transmaxillary transsphenoidal approach to the cavernous sinus: a minimally invasive microsurgical model.

UNLABELLED: The authors have previously described an extradural transmaxillary approach to the anterior compartment of the cavernous sinus. In an effort to expand the surgical access to that area without necessitating a craniotomy or wide transfacial dissection, they present a modification of the transmaxillary approach to the sellar region and cavernous sinus. METHODS: The approach was developed on 12 fresh and 12 embalmed cadaveric specimen, and 2 dry skulls. The initial sublabial incision is followed by a maxillotomy to expose the course of the infraorbital nerve (terminal branch of maxill

Patterns of innervation of the anterior maxilla: a cadaver study with relevance to canine fossa puncture of the maxillary sinus.

OBJECTIVES/HYPOTHESIS: Complications from canine fossa puncture of the maxillary sinus are caused by damage to the anterior superior alveolar nerve (ASAN) and the middle superior alveolar nerve (MSAN). The aim of this study was to elucidate the pattern of ASAN and MSAN within the anterior maxilla and to secondly determine suitable surgical landmarks to aid in accurately localizing the area of the canine fossa least likely to produce complications when a trocar is passed into the maxillary sinus. METHODS: Anatomic dissection of the anterior face of the maxilla from 20 cadaver heads was perform

Anaesthetic efficacy of a labial infiltration method on the nasopalatine nerve.

BACKGROUND AND AIM: The conventional nasopalatine nerve block is commonly used to obtain anaesthesia in the anterior portion of the palate. The painful nature of this approach, however, has led investigators to seek alternative methods to obtain anaesthesia. Labial infiltration of the maxillary central incisors can be considered an effective anaesthetic approach for the anterior palate. Our study aimed to assess the anaesthetic effect of a modified labial infiltration method on the nasopalatine nerve. MATERIALS AND METHOD: A case-control clinical trial was done on 60 patients referring for ex

Sixth nerve palsy of para-infectious origin.

Acute abducens palsy of para-infectious origin is uncommon in adulthood. We present the case history of a 28-year-old woman suffering from acute onset diplopia due to abducens palsy. Magnetic resonance imaging revealed signs suggestive of frank ipsilateral maxillary sinusitis. The lack of anatomical relationship between the maxillary sinus and the abducens nerve led us to conclude that the abducens palsy was of para-infectious origin.

Canalis sinuosus: a rare anatomical variation.

The infraorbital canal issues a small branch on its lateral face close to its midpoint to allow passage of the anterior superior alveolar nerve. This small canal, sometimes called the canalis sinuosus, runs forward and downward to the inferior wall of the orbit, lateral to the infraorbital canal and medially bent to the anterior wall of the maxillary sinus, passing below the infraorbital foramen. Anatomical variations in the maxilla are rarely described in the literature and, in most cases, are related to the nasopalatine canal. This article describes a rare anatomical variation of the presenc

A supportive donor nerve for long-term facial paralysis: Anatomical analysis of the posterior auricular nerve and micro-anatomical comparison with zygomatic nerve.

BACKGROUND: The posterior auricular nerve (PAN) is an inspiring candidate for the additional axonal source in long-term facial paralysis to improve the functional results of the cross-facial nerve (FN) graft technique. However, no studies have analyzed the PAN's axonal load and its microscopic anatomy to assess its utilization in facial reanimation. The present study aims to examine the anatomical and microscopic features of the PAN to analyze its feasibility as a donor nerve. METHODS: The bilateral facial side of 14 fresh frozen adult human cadavers was examined for the study. The PAN's anat

Measurement of lingual and palatine somatosensory evoked potentials.

OBJECTIVE: A technique is presented for generating and recording lingual and palatine nerve somatosensory evoked potentials (SEPs). METHODS: Pairs of thin, stainless steel disk electrodes were mounted onto mandibular or maxillary acrylic splints, similar to orthodontic retainers. Mandibular splint electrodes were oriented to contact the under surface of the tongue along the course of the right and left lingual nerves and maxillary splint electrodes were oriented to contact the hard palate bilaterally along the course of the palatine nerves. SEP recording electrodes were placed on the scalp 1

Peripheral nerve tumors involving paranasal sinuses: a case report and review of the literature.

A 45-year-old man, afflicted with von Recklinghausen's disease, with symptomatic schwannomas and plexiform neurofibromas involving the right maxillary antrum, a rare site for peripheral nerve tumors, is presented. After a literature review, 15 cases were considered adequate for a clinicopathologic study. Twelve schwannomas, 2 plexiform neurofibromas, 2 neurofibromas, and 1 probable malignant schwannoma were included. Epistaxis was common in tumors of the ethmoid sinus and nasal fossae, while pain was related to lesions of the maxillary sinus. Two benign schwannomas recurred and were cured by s

Late diagnosis of an occult tumour - what lessons can we learn?

OBJECTIVE: Malignant tumours of the nasal cavity and paranasal sinuses are rare and late presentation of a maxillary sinus tumour is common due to the vague nature of the symptoms which can delay diagnosis. METHODS: We report a female with a maxillary sinus tumour who was initially diagnosed with chronic idiopathic facial pain (CIFP) and sinusitis, which subsequently led to a delay in diagnosis and treatment of her tumour. RESULTS: There was no clinical extra- or intra-oral pathology, however, she had varying clinical presentations of facial pain, anosmia, loss of gustatory function, and inf

Cranial nerves of the coelacanth, Latimeria chalumnae [Osteichthyes: Sarcopterygii: Actinistia], and comparisons with other craniata.

We reconstructed the cranial nerves of a serially sectioned prenatal coelacanth, Latimeria chalumnae. This allowed us to correct several mistakes in the literature and to make broad phylogenetic comparisons with other craniates. The genera surveyed in our phylogenetic analysis were Eptatretus, Myxine, Petromyzon, Lampetra, Chimaera, Hydrolagus, Squalus, Mustelus, Polypterus, Acipenser, Lepisosteus, Amia, Neoceratodus, Protopterus, Lepidosiren, Latimeria and Ambystoma. Cladistic analysis of our data shows that Latimeria shares with Ambystoma two characters of the cranial nerves. Our chief findi

Distribution of CGRP and TRPV2 in Human Paranasal Sinuses.

Immunohistochemistry for protein gene product 9.5 (PGP 9.5), calcitonin gene-related peptide (CGRP) and the transient receptor potential cation channel subfamily V member 2 (TRPV2) was performed on human paranasal sinuses. It was found that in the paranasal sinuses, mucous membranes contain PGP 9.5-immunoreactive (PGP 9.5-IR) nerve fibers. Such nerve fibers terminated around large blood vessels as fine varicosities. Isolated PGP 9.5-IR nerve fibers were scattered beneath the epithelium. Glandular tissues were also innervated by PGP 9.5-IR nerve fibers. These fibers were numerous in the maxilla

[Persistent facial paralysis: contribution of imaging to identification of perineural infiltrating tumor].

To describe imaging features suggestive of retrograde perineural spread in progressive facial palsy without known tumoral origin. Two reports of perineural infiltration by salivary malignant tumors were diagnosed on abnormal enhancement of the facial nerve associated with an abnormal nerve enlargement (1/2). These unsuspected tumors were located in parotid gland (1/2) and the sub-maxillary gland (1/2). ENT tumors (especially adenoid cystic carcinomas) should be carefully searched in case of perineural spread, as these tumors may be misdiagnosed due to their size, location and signal. Therefore
