Mediastinal cysts are mostly congenital, but rarely, infections and malignancies can cause cystic degeneration of enlarged mediastinal lymph nodes. Diagnosis is challenging as the presenting symptoms are nonspecific. Surgical resection is the reference modality both for diagnosis and management. Secondary infection of mediastinal bronchogenic cyst with Mycobacterium tuberculosis is rare. Herein, we describe a young male who was managed as bronchial asthma with inhalational bronchodilators and glucocorticoids. Computed tomography revealed a cystic lesion in the subcarinal region. Endobronchial …

We experienced a rare case of anterior mediastinal bronchogenic cyst. A 55-year-old female was admitted to our hospital because of paroxysmal supraventricular tachycardia (PSVT) and an abnormal shadow on the chest computed tomography. She had a 5.5 cm tumor at anterior mediastinum. The tumor was surgically removed completely by video assisted thoracoscopic surgery, and the diagnosis of bronchogenic cyst was established pathologically. After surgery, PSVT has disappeared.

Mediastinal gastroenteric cyst is an uncommon congenital malformation and a distinct histopathological entity within the family of foregut duplication cysts. This lesion is mainly encountered in neonates and infants. Histologically, it is characterized by double-layered smooth muscle wall and gastric lining mucosa. We report on a case of a 2-day-old girl, with a posterior mediastinal cystic mass associated with T3-T4 hemivertebrae, presenting with severe respiratory distress. The cyst was multilocular, surgically removed, and histopathologic analysis revealed that it was of gastroenteric type.…

A notion has prevailed that carinal bronchogenic cyst and other congenital mediastinal cystic lesions, particularly those occurring in older children and adults, are usually asymptomatic, innocuous, and frequently only an incidental finding on routine chest roentgenogram or postmortem examination. Some physicians, therefore, have adopted a policy of observation for these patients. Our experience, however, as demonstrated in three cases reported herein, and the experience of others, clearly shows that carinal bronchogenic cyst is far from being usually asymptomatic and innocuous, but in fact of…

We present a 52-year-old gentleman with an unusual cause of progressive dysphagia, namely due to extrinsic lower oesophageal compression from a cystic mass of the posterior mediastinum. Cystic masses in adults are uncommon, and there is a wide differential diagnosis. This includes neoplastic, such as germ cell tumour (cystic teratoma), and non-neoplastic aetiologies. The later include foregut duplication cysts, lymphatic malformations, infective (hydatid), simple mediastinal cysts or pseudocysts. Management is principally surgical with complete excision, or alternatively, in cases of benign cy…

A 47-year-old man presented with hematuria. Computed tomography( CT) showed a posterior mediastinal cyst. Chest magnetic resonance imaging showed a well defined mass with high intensity on T2-weighted images. Echocardiogram revealed severe aortic regurgitation, moderate mitral regurgitation and no continuity between the cyst and the pericardium. We performed aortic valve replacement, mitral annuloplasty and cyst resection after confirming it was not malignant by intraoperative rapid pathological examination. We performed sufficient and safe cyst resection through full sternotomy under cardiac …

INTRODUCTION: We report the first case of complex supracardiac middle mediastinal foregut duplication lesion compressing and adherent to the heart, managed successfully thoracoscopically in an infant, in an innovative way.
MATERIALS AND SURGICAL TECHNIQUE: A 1-year-old girl was diagnosed as having supracardiac middle mediastinal foregut duplication lesion. It was completely overlying and adherent to her heart giving transmitted pulsations to the lesion, causing indentation over the left atrium and surrounded by all vital structures such as aorta, pulmonary artery, bronchi and phrenic nerve. A

INTRODUCTION: Bronchogenic cyst (BC) is an uncommon, benign congenital mediastinal tumor. Its progressive course is often asymptomatic, making the diagnosis difficult or fortuitous, unless non-specific respiratory signs appear, such as persistent cough or respiratory discomfort. We herein report the first documented case in the Democratic Republic of Congo (DRC).
PRESENTATION OF CASE: A 63-year-old man, with no prior medical or surgical history, consulted for a persistent dry cough, unresponsive to usual medical treatment. A chest CT scan revealed a cystic mediastinal mass close to both esoph

Mediastinal Mullerian cysts should be included in the differential diagnosis of posterior mediastinal cysts. Cystic lesions in the posterior mediastinum should be removed surgically and undergo immunohistochemical examination.

Thoracic duct cysts (TDC) within the retropharyngeal space and mediastinum are exceedingly rare lesions, with the majority related to trauma or neoplasm. We describe a case of an otherwise healthy 8-month-old boy who presented with severe respiratory distress, which was found to be caused by a large, spontaneous TDC occupying most the retropharyngeal and mediastinal space. To our knowledge, this is the youngest patient to date presenting with TDC. Ultimately, his TDC was completely resolved with sclerotherapy, however the patient's age and size presented unique challenges to his medical manage

Acquired pulmonary stenosis is rare. When it occurs, it usually results from compression of the pulmonary artery or the heart by a mediastinal tumour. As these masses rarely cause compression that results in murmurs, it is hard to make a diagnosis. We describe a 23-year-old female patient who presented with dull, aching chest pain lasting for 6 months. Echocardiography showed a large anterior mediastinal cystic mass compressing the main pulmonary artery. She was operated through a left anterolateral thoracotomy and had an uneventful recovery.

Bronchogenic cysts are one of the most common bronchopulmonary congenital malformations found in adults. Most authors agree that symptomatic or infected cysts should be surgically removed. We report our experience in which we treated an infected mediastinal bronchogenic cyst with a combination of antibiotics and drainage by endobronchial ultrasound-guided fine-needle aspiration, with resolution of symptoms and no evidence of recurrence after an 18-month follow-up.

A 51-year-old woman was admitted because of general fatigue assumed to be due to climacteric disturbance. Laboratory examinations suggested a diagnosis of hyperparathyroidism based on high serum levels of Ca, intact PTH, and high-sensitivity PTH. No abnormalities were found in the cervix. Chest X-ray films and CT scans showed a mass measuring 4 cm in the right postero-upper mediastinum. The tumor was positive on a parathyroid scintigram, and was diagnosed as a mediastinal parathyroid adenoma with hyperparathyroidism. Operative findings showed a tumor surrounded by the superior vena cava, vagus

The computed tomography (CT) appearance of a large cystic caseating granuloma in the middle mediastinum is reported. CT is helpful in evaluating the extent and density of the lesion and its relationship to neighboring structures.
