Medication errors in home care settings: a 13-year retrospective cohort study in the Ramathibodi Poison Center.

INTRODUCTION: Medication errors are a leading cause of preventable harm worldwide, yet evidence has predominantly focused on healthcare settings, with limited data from home care, particularly in Southeast Asia. METHODS: We conducted a retrospective cohort study using 2011-2023 data from the Ramathibodi Poison Center, including patients with medication errors occurring outside healthcare facilities. Clinical effects, management of medication errors, and outcomes were considered as primary outcomes, while the secondary outcome was factors associated with harm, defined as National Coordinating

Fatal acute methotrexate intoxication resulting from medication error in a rheumatoid arthritis patient: a case report.

Methotrexate (MTX) has long been a first-line anti-rheumatic agent due to its efficacy and cost-effectiveness. While generally safe at the recommended weekly dosage of 5-25 mg for rheumatoid arthritis, medication errors can lead to acute toxicity, severe adverse effects, and potentially fatal outcomes. We report a case of a 51-year-old male who experienced fatal pancytopenia following an MTX medication error during treatment for rheumatoid arthritis. His blood MTX concentration measured 0.04 μmol/L 11 days after discontinuation. Postmortem examination revealed MTX-related toxicity in multiple

Exploring multidimensional determinants of medication error reporting in China: a qualitative study using the theoretical domains framework.

OBJECTIVE: Medication errors, defined as preventable events leading to inappropriate medication use or patient harm, remain a critical global healthcare challenge. Despite regulatory mandates in China, underreporting remains systemic. Existing research on healthcare professionals' reporting behaviors has been constrained by reliance on isolated theoretical frameworks, which inadequately address the interplay of individual, organizational, and cultural determinants. The aim of this study is to systematically explore barriers and facilitators affecting medication error reporting among Chinese he

Prevalence of medication administration errors and its determinants among nurses in Ethiopia: a systematic review and meta-analysis.

BACKGROUND: Despite nurses being the backbone of patient care, medication administration errors (MAEs) remain a serious risk to patient safety in low-income countries, including Ethiopia. However, the previous review was outdated, included fewer than 10 studies, focused solely on tertiary hospitals, and did not pool determinants. As a result, this meta-analysis aimed to determine the pooled prevalence of MAEs and their determinants among nurses in Ethiopia, addressing gaps in the study setting, time, and outcomes. METHODS: The study protocol was registered in PROSPERO. Observational studies c

Identifying high-risk medications and error types in Danish patient safety database using disproportionality analysis.

BACKGROUND: Medication error (ME) surveillance in Danish healthcare relies on the mandatory national incident reporting system, the Danish Patient Safety Database (DPSD). Individual case reviews and descriptive statistics with frequency counts are the most often used approaches when analyzing MEs in incident reporting systems, including the DPSD. However, incident reporting systems often generate a large number of reports and may suffer from underreporting; consequently, additional approaches are needed to overcome these challenges. Disproportionality analysis (DPA) is a statistical tool used

Uncertainty as risk: A plug-and-play evidence-guided framework with error-aligned calibration for medical image segmentation.

Medical image segmentation is crucial for clinical diagnosis and treatment planning. However, in complex scenarios, factors such as boundary ambiguity, annotation noise, and domain shift often lead models to be overconfident, limiting the interpretability and trustworthy deployability. To address this, we propose a "plug-and-play" evidence-guided framework for reliable medical image segmentation. By incorporating an evidential constraint to suppress "high-confidence but erroneous" predictions, and without altering the existing segmentation backbone architecture, we append a lightweight evident

Medication errors involving intravenous paracetamol in children: experience from enquiries to the National Poisons Information Service.

INTRODUCTION: Children are at higher risk of medication errors due to the complexity of drug prescribing and administration in this patient group. Intravenous (IV) paracetamol overdose differs from overdose by ingestion as there is no enteral absorptive buffering. We provide the first national UK data focusing on paediatric IV paracetamol poisoning. METHODS: All telephone enquiries to the National Poisons Information Service between 2008 and 2021 regarding children less than 18 years old in the UK concerning IV paracetamol overdose were extracted from the UK Poisons Information Database (UKPI
