Health Care Affordability Problems by Income Level and Subsidy Eligibility in Medicare.

IMPORTANCE: Standard Medicare benefits include substantial cost sharing, often resulting in high out-of-pocket expenses for beneficiaries. Financial assistance programs help mitigate these costs by covering Medicare Parts A and B premiums and cost sharing for beneficiaries with limited assets and incomes below 100% of the federal poverty level (FPL) and premiums for beneficiaries with incomes between 100% and 150% of the FPL. OBJECTIVE: To examine health care affordability problems among Medicare beneficiaries by income levels aligned with subsidy eligibility. DESIGN, SETTING, AND PARTICIPAN

The costs of treating and not treating patients with chronic myeloid leukemia with tyrosine kinase inhibitors among Medicare patients in the United States.

BACKGROUND: Patients with high cost-sharing of tyrosine kinase inhibitors (TKIs) experience delays in treatment for chronic myeloid leukemia (CML). To the authors' knowledge, the clinical outcomes among and costs for patients not receiving TKIs are not well defined. METHODS: Using the Surveillance, Epidemiology, and End Results (SEER)-Medicare database, the authors evaluated differences in TKI initiation, health care use, cost, and survival among patients with CML with continuous Medicare Parts A and B and Part D coverage who were diagnosed between 2007 and 2015. RESULTS: A total of 941 pati

Long-Term Opioid Therapy in Older Cancer Survivors: A Retrospective Cohort Study.

OBJECTIVES: To examine the rates and predictors of long-term opioid therapy in older cancer survivors. DESIGN: Retrospective cohort study. SETTING: Texas, United States. PARTICIPANTS: Cancer survivors (5 years or more postcancer diagnosis) diagnosed from 1995 to 2008 and who were also Medicare Parts A, B, and D beneficiaries. MEASUREMENTS: We used Medicare Part D event data to calculate the proportion of cancer survivors with a prolonged opioid prescription (90-day or more supply of opioids/year). Adjusted odds ratios were calculated to identify predictors of prolonged opioid prescribing.

Recent trends in utilization of vascular ultrasound among radiologists, surgeons, cardiologists, and other physicians.

PURPOSE: To determine how much vascular ultrasound is performed by the various medical specialties and to assess the utilization trends among them in recent years. METHODS AND MATERIALS: The nationwide Medicare Part B databases for 1998 through 2004 were used. The authors studied the 15 Current Procedural Terminology, Version 4, codes in the 93000 series pertaining to vascular ultrasound and determined the volume of examinations performed under global and professional component claims. Using Medicare's physician specialty codes, utilization rates per 1,000 Medicare beneficiaries and trends in…

Variation in hip fracture treatment: are black and white patients treated equally?

To examine disparity in race for hip fracture treatment, we identified femoral neck fractures and pertrochanteric fractures from a 20% sample of 1999-2003 Medicare part B claims data and stratified patients by treatment: total hip arthroplasty (THA), hemiarthroplasty (HA), open reduction and internal fixation (ORIF), and nonoperative management (NM). Covariables included patient race, age, sex, and Charlson Comorbidity Index score. The geographic variable was the hospital referral region (306 such US regions are defined in The Dartmouth Atlas of Health Care). Logistic regression was performed …

High incidence of type 2 endoleak and low associated adverse events in the Vascular Quality Initiative linked to Medicare claims.

INTRODUCTION: Type 2 endoleak (T2EL) is the most common adverse finding on postoperative surveillance after endovascular aortic aneurysm repair (EVAR). A low rate of aneurysm-related mortality with T2EL has been established. However, the optimal management strategy and the efficacy of reintervention remain controversial. This study used data from the Vascular Quality Initiative linked to Medicare claims (VQI-Medicare) to evaluate T2LE in a real-world cohort. METHODS: This retrospective review of EVAR procedures in VQI-Medicare included patients undergoing their first EVAR procedure between 20

Use of Claims Data to Screen for Functional Limitations Among Medicare Beneficiaries.

IMPORTANCE: The only functional impairment data available for all Medicare beneficiaries identify those who qualified for Medicare before age 65 years due to work-related disability. This administrative measure omits impairments arising after age 64 years or affecting nonwork-related activities. Surveys may offer more accurate assessments, but survey data on functional limitations are not routinely available. Claims data may provide a practical, scalable alternative. OBJECTIVE: To describe the development of a survey-based index of functional limitations and a new claims-based model for predi

Readmissions and postdischarge mortality by race and ethnicity among Medicare beneficiaries with multimorbidity.

BACKGROUND: Disparities in readmission risk and reasons they might exist among diverse complex patients with multimorbidity, disability, and unmet social needs have not been clearly established. These characteristics may be underestimated in claims-based studies where individual-level data are limited. We sought to examine the risk of readmissions and postdischarge mortality by race and ethnicity after rigorous adjustment for multimorbidity, physical functioning, and sociodemographic and lifestyle characteristics. METHODS: We used Health and Retirement Study (HRS) data linked to Medicare clai

Aortic valve replacement among patients with Alzheimer's disease and related dementias.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has made palliation from aortic stenosis more broadly available to populations previously thought to be too high risk for surgery, such as those with Alzheimer's disease and related dementias (ADRD); however, its safety and effectiveness in this context are uncertain. METHODS: We performed a retrospective cohort study of national Medicare beneficiaries, aged 66 and older with Parts A and B, between 2010 and 2016. Patients undergoing AVR were identified, and follow-up was available through 2017. Multivariable regression was used to meas
