Issues and controversies surrounding the diagnosis and treatment of social anxiety disorder.

Although much has been learned about social anxiety disorder (SAD) in recent decades, many questions and controversies surrounding its diagnosis and treatment have remained. Similar to the state of affairs with other psychiatric disorders, no clear pathophysiology has been identified for SAD, and the question of where to draw the line between shyness, SAD and even avoidant personality disorder continues to be debated. Much of the evidence to date suggests that among persons with SAD, it is under-recognized and undertreated; however, other researchers contend that it may be overdiagnosed in som

Client perspectives of internet-based treatment for depression in Arabic-speaking countries.

BACKGROUND: Mental health care faces significant challenges, particularly in Arabic-speaking countries, where only a fraction of individuals with depression receive treatment. While internet-based interventions (IBIs) have shown promising results, how clients experience IBIs is not well known, highlighting the need for a deeper understanding of client experiences. OBJECTIVE: To better understand the impact of IBIs, qualitative interviews were conducted with participants from Arabic-speaking countries regarding perceived changes, causes of changes, and hindering and helpful aspects of IBIs. MET

Efficacy and mechanisms of internet-delivered mindfulness-based cognitive therapy for Adjustment Disorder: A randomized controlled trial.

BACKGROUND: Adjustment Disorder (AjD) is a common stress-related condition, yet evidence-based interventions remain scarce. Mindfulness-Based Cognitive Therapy (MBCT) targets transdiagnostic mechanisms such as cognitive fusion and low self-compassion, which may be particularly relevant for AjD. OBJECTIVE: This randomized controlled trial tested the efficacy of a brief, guided, asynchronous internet-delivered MBCT (iMBCT) for AjD during the COVID-19 pandemic, compared with internet-delivered progressive muscle relaxation (iPMR) and a waitlist control (WLC). METHODS: Adults meeting ICD-11 crit

The empirical status of psychodynamic psychotherapy - an update: Bambi's alive and kicking.

BACKGROUND: The Task Force on Promotion and Dissemination of Psychological Procedures proposed rigorous criteria to define empirically supported psychotherapies. According to these criteria, 2 randomized controlled trials (RCTs) showing efficacy are required for a treatment to be designated as 'efficacious' and 1 RCT for a designation as 'possibly efficacious'. Applying these criteria modified by Chambless and Hollon, this article presents an update on the evidence for psychodynamic therapy (PDT) in specific mental disorders. METHODS: A systematic search was performed using the criteria by Ch

The role of psychotherapy in treating psychiatric disorders.

Psychotherapy has shifted from long-term to short-term approaches, which have been found to be effective for the treatment of specific psychiatric disorders. These psychotherapy interventions (primarily behavior therapy, CBT, and IPT) have been found useful in presenting an educational framework for disorders and the treatment rationale for intervention programs. Short-term and maintenance empirical data support the effectiveness of using behavior therapy and CBT as adjunctive interventions with medications for bipolar I disorder and schizophrenia. In major randomized clinical trials, psychoth

[Evaluated treatment approaches in child and adolescent psychiatry II].

OBJECTIVES: The principle of evidence-based medicine is to integrate data concerning the efficacy of interventions into clinical practice. This article assesses the level of evaluation of psychosocial, psychopharmacological and combined interventions for mental disorders in childhood and adolescence (schizophrenic disorders, affective disorders, phobias and anxiety disorders, obsessive-compulsive disorder, posttraumatic stress disorder, anorexia nervosa, and bulimia nervosa). METHODS: Three different levels of evaluation were defined for both psychosocial and psychopharmacological interventio

The roles of psychiatrists in organized outpatient mental health settings.

OBJECTIVE: This paper describes the clinical roles of psychiatrists in U.S. organized outpatient mental health settings. METHOD: Data were analyzed from the 1986 National Institute of Mental Health Client/Patient Sample Survey. The authors determined the range, volume, and content of services provided to established outpatients treated by psychiatrists at six types of mental health organizations: state and county mental hospital clinics, general hospital mental health clinics, private psychiatric hospital clinics, U.S. Department of Veterans Affairs medical center mental health clinics, free-

Assessing the approach to perinatal mental health screening and treatment in maternal-child health clinics in Western Kenya.

Integrating mental health care into well-attended health services, such as maternal-child health (MCH), offers a promising approach to increasing access to mental health care. To inform integration, we assessed the current approach to screening and treatment of perinatal mood and anxiety disorders (PMAD) in MCH facilities in Western Kenya. We conducted a cross-sectional survey among the facility managers ("in-charges") at 20 MCH facilities in Western Kenya. Trained data collectors administered questionnaires to assess facility infrastructure and human resources, perinatal mental health screeni

Risks for opioid abuse and dependence among recipients of chronic opioid therapy: results from the TROUP study.

OBJECTIVE: To estimate the prevalence of and risk factors for opioid abuse/dependence in long-term users of opioids for chronic pain, including risk factors for opioid abuse/dependence that can potentially be modified to decrease the likelihood of opioid abuse/dependence, and non-modifiable risk factors for opioid abuse/dependence that may be useful for risk stratification when considering prescribing opioids. METHODS: We used claims data from two disparate populations, one national, commercially insured population (HealthCore) and one state-based, publicly insured (Arkansas Medicaid). Among

Testing child STEPS in school-based mental health care: Cluster-randomized controlled effectiveness trial in five school districts.

OBJECTIVE: More youth mental health care in the United States is provided in schools than any other setting. So, there is a need for evidence-based psychotherapies that can work well with students in those sometimes complex environments. Transdiagnostic modular therapies may help, given their flexibility and capacity to address diverse youth disorders and problems. METHOD: We tested this possibility by conducting a school-based cluster-randomized controlled trial of Child STEPs, an intervention that combines the Modular Approach to Therapy for Children with consultation and measurement-based

Unseen and underserved: A scoping review of perinatal mental health interventions in Sub-Saharan Africa.

Maternal mental health (MMH) remains a critical public health issue across Sub-Saharan Africa (SSA). Despite the high burden, access to culturally appropriate and evidence-based mental health interventions remains limited. This scoping review examines interventions targeting mental health outcomes for pregnant and postpartum women across SSA countries. Guided by PRISMA-ScR and Arksey and O'Malley's framework, a comprehensive search was conducted across 14 electronic databases for peer-reviewed intervention studies published in English between 2000 and March 2026. A total of 57 studies across 1

The power of imagery: examining the prevalence of imagination-based hotspots after a traumatic event in the Grenfell Health and Wellbeing Service.

BACKGROUND: Intrusive re-experiencing of traumatic events is a cornerstone of post-traumatic stress disorder (PTSD). Clinicians notice that clients also experience intrusive mental images of what they think might happen during a traumatic event. As mental imagery has a powerful impact on emotion, imagination-based imagery may be implicated in the peaks of distress ('hotspots') during a trauma. AIMS: A data-only study was undertaken of cognitive therapy for PTSD 'hotspot' charts used by Grenfell Health and Wellbeing Service clinicians after the Grenfell fire disaster. The aim was to establish

Expanding Access to Specialized Mental Health Services for Youths in Crisis Through a State-Provider-University Partnership.

Self-injurious thoughts and behaviors (SITBs) pose a clear risk to adolescents' health and well-being. Although established treatments (e.g., dialectical behavior therapy [DBT]) for youths experiencing SITB exist, numerous challenges reduce the feasibility of delivering such treatments in community-based settings. To address the need for increased access to specialized mental health treatments among youths, a state, provider, and university partnership was developed to codesign and implement a DBT-informed clinical training model within the existing community-based mental health provider teams

Psychological therapies for people with intellectual disabilities: a systematic review and meta-analysis.

The aim of this study was to evaluate the efficacy of psychological therapies for people with intellectual disabilities (IDs) through a systematic review and meta-analysis of the current literature. A comprehensive literature search identified 143 intervention studies. Twenty-two trials were eligible for review, and 14 of these were subsequently included in the meta-analysis. Many studies did not include adequate information about their participants, especially the nature of their IDs; information about masked assessment, and therapy fidelity was also lacking. The meta-analysis yielded an over

Comparison of mental health resources used by patients with bipolar disorder treated with risperidone, olanzapine, or quetiapine.

OBJECTIVE: The atypical antipsychotics, risperidone, olanzapine, and quetiapine, have been approved by the U.S. Food and Drug Administration for treatment of mania associated with bipolar disorder. Information on the relative mental health resource use of these therapies is helpful to pharmacy managers since differences in efficacy and safety may translate into differences in mental health care utilization. We compared charges for other mental health services associated with risperidone, olanzapine, and quetiapine treatment of patients with bipolar disorder to assess whether there were signifi

The RISE study protocol: resilience impacted by positive stressful events for people with cystic fibrosis.

INTRODUCTION: For people with cystic fibrosis (CF), gaining access to elexacaftor/tezacaftor/ivacaftor (ETI) therapy, a new modulator drug combination, is perceived as a positive life event. ETI leads to a strong improvement of disease symptoms. However, some people with CF experience a deterioration in mental wellbeing after starting ETI therapy. The primary objective of this study is to investigate if and in which direction mental wellbeing of people with CF changes after starting ETI therapy. Our secondary objectives include, among others, investigation of underlying biological and psychoso

Stigma, Mental Health, and HIV Adherence: A Cross-sectional Structural Equation Modeling Study Exploring Direct and Indirect Effects Among School-Engaged Ugandan Adolescents Living With HIV.

Our study examined how multiple dimensions of HIV-related stigma affect mental health and antiretroviral therapy (ART) adherence among school-engaged adolescents living with HIV in Uganda. Using structural equation modeling with 833 participants recruited from 42 HIV clinics, we examined multiple dimensions of internalized and externalized HIV stigma, and their associations with adherence outcomes. We found that internalized shame and family-based stigma were significantly associated with poor mental health which, in turn, strongly predicted nonadherence to ART. Mediation analyses confirmed in

The most vulnerable are prone to use AI therapists: The role of attachment, epistemic trust, and mental health symptoms in acceptance of digital mental health interventions.

OBJECTIVE: Digital mental health interventions (DMHIs) offer scalable solutions to traditional barriers in mental healthcare. Acceptability of DMHIs might differ among patients with differing level of symptoms and relational difficulties. This study investigated how a person's psychological profile, including their level of mental health symptoms, epistemic trust, and attachment security, predicts their acceptance of AI-based interventions (avatars, chatbots) versus human-delivered teletherapy. METHOD: Using cluster analysis on survey data from 1,612 (potential) patients and clinicians recrui

Speech, language and communication needs and mental health: the experiences of speech and language therapists and mental health professionals.

BACKGROUND: While the relationship between speech, language and communication needs (SLCN) and mental health difficulties has been recognized, speech and language therapists (SLTs), and mental health professionals face challenges in assessing and treating children with these co-occurring needs. There exists a gap in the evidence base for best practice for professionals working with children and young people (CYP) who experience difficulties in both areas. AIMS: To explore the views of SLTs and mental health clinicians about their experiences of working with CYP exhibiting co-occurring SLCN an
