The development of the DuromTM metal-on-metal hip resurfacing.

The poor results of conventional total hip replacement in young and active patients have led to a renewed interest in alternative interventions and bearing material combinations. Con-temporary metal-on-metal resurfacing of the hip has emerged as an attractive option. This procedure utilises a low wear producing bearing which minimises the risk of periprosthetic osteolysis, preserves femoral bone stock should subsequent procedures become necessary, and, due to the increased stability and range of motion, allows the rapid resumption of an active lifestyle. We describe the development of the Duro

The tribology of metal-on-metal total hip replacements.

Total hip surgery is an effective way of alleviating the pain and discomfort caused by diseased or damaged joints. However, in the majority of cases, these joints have a finite life. The main reason for failure is osteolysis (bone resorption). It is well documented that an important cause of osteolysis, and therefore the subsequent loosening and failure of conventional metal- or ceramic-on-ultra-high molecular weight polyethylene joints, is the body's immunological response to the polyethylene wear particles. To avoid this, interest has been renewed in metal-on-metal joints. The intention of t

The rationale for metal-on-metal total hip arthroplasty.

Metal-on-metal total hip arthroplasty has the longest clinical history of any of the currently used articular couples. Long-term followup of what are now considered suboptimal designs (eg, McKee Farrar, Ring, Sivash) has produced a wealth of knowledge regarding the safety and efficacy of this articular combination. Retrieval analysis of metal-on-metal total hip arthroplasty articular couples has shown wear between 1 and 5 microns per year after initial wear-in, in comparison with 100 to 200 microns per year associated with metal-on-polyethylene wear. The constituent metal ions released through

Belgium experience with metal-on-metal surface arthroplasty.

Hybrid metal-on-metal surface arthroplasty of the hip has recently been introduced, with a vast number of implants used in European countries including Belgium. This article presents results in 252 hips with a mean follow-up of 2.8 years. Using a tight press-fit with minimal cement mantle as the technique of femoral fixation, there have been only three failures. The main complications have been avascular necrosis of the femoral head and femoral neck fracture. In most cases, patients returned to a high functional level with no restrictions in their physical activity and were highly satisfied. F

Speciation in Metal Toxicity and Metal-Based Therapeutics.

Metallic elements, ions and compounds produce varying degrees of toxicity in organisms with which they come into contact. Metal speciation is critical to understanding these adverse effects; the adjectives "heavy" and "toxic" are not helpful in describing the biological properties of individual elements, but detailed chemical structures are. As a broad generalization, the metallic form of an element is inert, and the ionic salts are the species that show more significant bioavailability. Yet the salts and other chelates of a metal ion can give rise to quite different toxicities, as exemplified

Pseudotumours associated with metal-on-metal hip resurfacings.

We report 17 patients (20 hips) in whom metal-on-metal resurfacing had been performed and who presented with various symptoms and a soft-tissue mass which we termed a pseudotumour. Each patient underwent plain radiography and in some, CT, MRI and ultrasonography were also performed. In addition, histological examination of available samples was undertaken. All the patients were women and their presentation was variable. The most common symptom was discomfort in the region of the hip. Other symptoms included spontaneous dislocation, nerve palsy, a noticeable mass or a rash. The common histologi

Rationale for metal-on-metal total hip replacement.

Traditional metal-on-polyethylene total hip replacements are prone to wear with secondary osteolysis and aseptic loosening, especially in younger, more active patients. Metal-on-metal represents an alternative bearing surface that offers much lower wear rates with the aim of improving longevity and the use of larger femoral heads that confer increased inherent stability, resulting in fewer patient restrictions and improved patient satisfaction. This article discusses the science and clinical evidence in support of choosing metal-on-metal as a bearing as well as its limitations.

Proteomics of metal transport and metal-associated diseases.

Proteomics technology has the potential to identify groups of proteins that have similar biological function. However, few attempts have been made to identify and characterize metal-binding proteins by using proteomics strategies. Many transition metals are essential to sustain life. Copper, iron, and zinc are the most abundant transition metals relevant to biological systems. In addition to their important biological functions, metals can also catalyze the formation of damaging free radical species. Hence, their intracellular transport is tightly regulated. Despite recent insights into the in…

Development and problems of metal-on-metal hip arthroplasty.

For over 40 years, the metal-on-polyethylene bearing has dominated the field of total hip replacement. Problems of wear, osteolysis (dissolution of bone), and ultimately failure of prostheses have led to the development of alternative bearing surfaces. Metal-on-metal hip resurfacing has taken current orthopaedic surgery almost by storm. However, metal ion release following metal-on-metal hip resurfacing remains a major cause for concern. This article looks into the development and examines problems and issues surrounding metal-on-metal resurfacing arthroplasty.

Fixation and osteolysis with Metasul metal-on-metal articulation.

The purpose of this study was to report the prevalence of osteolysis and loss of fixation using Metasul metal-on-metal articulation couples. Ninety-two patients with 96 hips with Metasul articulation couples were followed for 5 to 11 years. By patient self-assessment, 89 (93%) patients scored themselves as good or excellent. The occurrence of osteolysis and the grading of fixation were obtained from plain radiographs. The clinical evaluation for these patients was performed by patient self-assessment. With the use of plain radiographs, there was no osteolysis observed in these 96 hips other th

Anterior Subluxation of a Metal-on-Metal Total Hip Arthroplasty Resulting in Erosion and Metal Debris.

Introduction: Total hip arthroplasty (THA) is a commonly performed and highly successful surgical procedure. Metal-on-metal (MoM) THA implants were introduced two decades ago and subsequently recalled due to high early revision rates. Acetabular cup erosion and fragmentation secondary to chronic edge loading causing delayed instability are rare but devastating complications of MoM THA warranting expeditious revision surgery. Case Presentation: We report a 70-year-old male with a history of bilateral MoM THA who presented with left hip instability. In addition to the radiographic and clinical f

Self-reported neurological clinical manifestations of metal toxicity in metal-on-metal hip arthroplasty.

Adverse reactions to metal particle debris have been increasingly reported as a complication following large head metal-on-metal (MoM) hip arthroplasty. Elevated metal ion levels are a cause for concern. The aim of this study is to evaluate whether exposure to cobalt is associated with patient characteristics and symptoms of neuropathy, representing nervous system dysfunction. A cross-sectional study was conducted comparing patients with a MoM total hip arthroplasty and patients with a conventional hip arthroplasty. They received three questionnaires, one to assess neurotoxic complaints and tw

Catastrophic failure of a metal-on-metal total hip arthroplasty secondary to metal inlay dissociation.

Metal-on-metal bearing surfaces in total hip arthroplasty have been recently shown to have acceptable survivorship properties (J Bone Joint Surg Am. 2006;88:1183; J Bone Joint Surg Am. 2006;88:1173), and they have certain advantages and disadvantages when compared to conventional metal-on-polyethylene bearing surfaces. Like traditional metal-on-polyethylene bearings, these metal-on-metal implants may also suffer from catastrophic failure. This case report represents an unusual situation in a 57-year-old man in which dissociation of a metal inlay in a metal-on-metal total hip arthroplasty resul

Understanding the differences between the wear of metal-on-metal and ceramic-on-metal total hip replacements.

Hip simulator studies have been carried out extensively to understand and test artificial hip implants in vitro as an efficient alternative to obtaining long-term results in vivo. Recent studies have shown that a ceramic-on-metal material combination lowers the wear by up to 100 times in comparison with a typical metal-on-metal design. The reason for this reduction remains unclear and for this reason this study has undertaken simple tribometer tests to understand the fundamental material loss mechanisms in two material combinations: metal-on-metal and ceramic-on-ceramic. A simple-configuration

Stabilization of electrocatalytic metal nanoparticles at metal-metal oxide-graphene triple junction points.

Carbon-supported precious metal catalysts are widely used in heterogeneous catalysis and electrocatalysis, and enhancement of catalyst dispersion and stability by controlling the interfacial structure is highly desired. Here we report a new method to deposit metal oxides and metal nanoparticles on graphene and form stable metal-metal oxide-graphene triple junctions for electrocatalysis applications. We first synthesize indium tin oxide (ITO) nanocrystals directly on functionalized graphene sheets, forming an ITO-graphene hybrid. Platinum nanoparticles are then deposited, forming a unique tripl

Dual Metalation Site Zr-Metal-Organic Frameworks: Controlled Synthesis of Metal-Organic Framework-Bound Proximal Metal Complexes.

Isolating multiple metal ion species within a confined microenvironment can facilitate cooperative interactions that underpin multielectron processes and synergistic or tandem catalysis. Herein, we report the design and synthesis of three flexible Zr-MOFs (UAM-1001-Py, UAM-1002-Py, and UAM-1003-Py) constructed from a trifunctional linker featuring two distinct coordination sites, based on bispyrazole and pyridyl donors, that can be postsynthetically metalated. Using UAM-1002-Py and UAM-1003-Py, we successfully isolated both cis and trans-PdCl2 isomers within each framework; however, enhanced r

Decoupling Stable Cyclic Deformation From Defect Accumulation in Metals.

Fatigue resistance in metals is fundamentally constrained by a long-standing dilemma: the strain hardening required to suppress deformation localization is typically achieved through the accumulation of dislocations and other defects that ultimately promote fatigue damage. Whether cyclic plasticity can be sustained without progressive defect storage remains an unresolved question. Here we demonstrate that this limitation can be overcome through the introduction of amorphous nanoparticles into a crystalline metal matrix. We uncover a cyclic deformation regime characterized by sustained resistan…

Nanoporosity-driven deformation of additively manufactured nano-architected metals.

3D printing methods for small-scale metals enable a unique 10-100 nm dimensional niche where functional feature sizes, critical microstructural detail and atomic-level defects converge, challenging conventional hierarchical relationships and carrying significant nanomechanical implications. We introduce a metal nano-printing system combining two-photon lithography, hydrogel infusion-based additive manufacturing and in situ mechanical experiments on 3D nano-architected Ni, achieving ~100 nm critical dimensions, ~10 nm surface roughness, and a broad range of geometries (periodic vs. non-periodic…
