Neonatal jaundice and human milk.

Breastfeeding is linked both to a greater jaundice frequency and intensity in the first postnatal days ("breastfeeding jaundice") and to visible jaundice persisting beyond the first two weeks of life ("breast milk jaundice"), but the appearance of skin jaundice is not a reason for interrupting breastfeeding which can and should continue without any interruption in most cases. There have been numerous contributions to the literature, which have rescaled the direct role of breast milk, both in early jaundice and in the more severe cases of late jaundice. In fact, the reviewed guidelines for dete…

Breast milk: friend or foe?

Anaphylaxis can occur secondary to breastfeeding. The authors present a case of a 30-year-old woman who complained of chest tightness, shortness of breath and a rash on the third day postpartum. She was treated for anaphylaxis and her symptoms resolved. Because she had taken tramadol prior to this event, a drug reaction was initially suspected. However, she experienced further episodes related to breastfeeding, despite stopping tramadol. Effective control of her symptoms was achieved with regular antihistamines, enabling her to continue breastfeeding. Antihistamines were interrupted after 8 we

Neonatal brucellosis and breast milk.

In this case report the authors present an extremely low birth weight premature infant with neonatal brucellosis whose mother had been treated for brucellosis during pregnancy. Infant developed mild respiratory distress syndrome soon after birth. At 2nd wk of postnatal age findings of bronchopulmonary dysplasia were evident and she and her mother were diagnosed to have brucellosis at the same time. After commencement of antibrucellosis therapy and nonspesific treatment for bronchopulmonary dysplasia, infant was completely cured of the symptoms related to both brucellosis and bronchopulmonary d

Trefoil factors in human milk.

We measured concentrations of the gastrointestinal protective peptides Trefoil factors in human milk. By the use of in-house ELISA we detected high amounts of TFF3, less TFF1 and virtually no TFF2 in human breast milk obtained from 46 mothers with infants born extremely preterm (24-27 wk gestation), preterm (28-37 wk gestation), and full term (38-42 wk gestation). Samples were collected during the first, second, third to fourth weeks and more than 4 wks postpartum. Median (range) TFF1 [TFF3] concentrations in human milk were 320 (30-34000) [1500 (150-27,000)] pmol/L in wk 1, 120 (30-720) [310 …

Hypotensive peptides from milk proteins.

Hypertension is the major controllable risk factor associated with cardiovascular disease (CVD) events such as myocardial infarction, stroke … potassium levels, reduced renal function, cough, angioedema, skin rashes, and fetal abnormalities. Milk proteins, both caseins and whey proteins, are a rich source of ACE inhibitory peptides. Several studies in spontaneously hypertensive rats show that these casokinins and lactokinins can significantly reduce blood pressure. Furthermore, a limited num …

Milk-alkali syndrome in pregnancy.

BACKGROUND: Severe hypercalcemia, a potentially life-threatening medical emergency, is rare in pregnancy. CASE: We report a 32-year-old woman presenting early in the second trimester with severe hypercalcemia (total calcium 22 mg/dL), alkalosis, and acute renal insufficiency resulting from excessive ingestion of calcium carbonate-containing antacid for gastroesophageal reflux. The patient was treated with aggressive hydration and furosemide, and received 1 dose of intravenous etidronate, leading to short-term symptomatic hypocalcemia. To our knowledge, this is the third reported case of milk-…

Milk consumption in older Americans.

OBJECTIVES: The purpose of this study was to identify individual characteristics associated with types and frequency of milk consumption in older American adults. METHODS: A national probability-based sample (response rate = 91%) completed a telephone survey. Generalized logit and cumulative logit analyses were used to identify predictors of and barriers to fluid milk consumption in 494 elderly people. RESULTS: The likelihood of drinking skim or 1% milk rather than whole milk increased with nutrition knowledge, income, trying to reduce cholesterol intake, and being female (P < .05). Frequenc…

Lidocaine excretion in breast milk.

Nursing mothers are occasionally treated with intravenous lidocaine for ventricular dysrhythmias. There have been no reports on the excretion of lidocaine into breast milk. This case documents the excretion of lidocaine into breast milk in small amounts and shows the validity of the TDx methodology used in the whole-milk lidocaine assay. We observed breast-milk concentrations of lidocaine at 40 percent of the serum levels. Clinical practitioners should be aware the lidocaine is excreted into breast milk in small amounts and the mother could probably continue to safely breast-feed her child whi…

[Listeria monocytogenes in pasteurized milk].

An haemolytic Listeria monocytogenes strain pathogenic to mice was isolated from 6 out of 28 (21.4%) pasteurized milk samples (3.2% fat milk treated at 78 degrees C for 15 s) marketed by a Madrid processing plant. Listeria grayi was recovered from 25 of the samples (89.2%) and L. innocua from 3 samples (10.7%). One milk sample was contaminated with L. welshimeri. No strains of L. ivanovii, L. seeligeri, L. murrayi, or L. denitrificans were isolated. These results show that pathogenic Listeria strains can be isolated from pasteurized milk and reinforce the hypothesis that this food product may …

Amphetamine secretion in breast milk.

The excretion of amphetamine in human breast milk was studied in a nursing mother with narcolepsy, who was treated with 20 mg daily of a racemic preparation of amphetamine. The concentration of amphetamine was 3 and 7 times higher in breast milk than in maternal plasma on the 10th and 42nd days after delivery. Small amounts of amphetamine were found in urine samples from the infant.

Carbamazepine levels in breast milk.

The carbamazepine (CBZ) concentration of breast milk was determined in 19 epileptic patients of whom 13 were receiving CBZ monotherapy. Two lactation periods under CBZ monotherapy were followed in 2 of these 13 patients. The CBZ concentration of the breast milk was 36.4% of the maternal CBZ serum level (mean value). The correlation coefficient was 0.44; the regression line follows the equation y = 0.12X + 1.63. The CBZ concentration was measured in a total of 50 milk samples during the lactation period (cases of monotherapy plus combination therapy). The CBZ concentration ranged from 1 to 4.8 …

Epidemiology of Milk-Borne Diseases.

Secular trends in milk-borne diseases in the U.S.A. show numerous outbreaks associated with ingestion of raw milk in the early 1900s until the end of World War II. Diseases common in this period, but no longer milk-borne, were typhoid fever, scarlet fever, septic sore throat, diphtheria, tuberculosis, shigellosis, and milk sickness. Milk-borne and milk-product-borne diseases rarely reported somewhere in the world were botulism, Escherichia coli enteritis, Pseudomonas aeruginosa enteritis, listeriosis, Clostridium perfringens enteritis, Bacillus cereus gastroenteritis, Haverhill fever, Q fever,…

Pregnanediols and breast milk jaundice.

Samples of breast milk collected from mothers of infants with breast milk jaundice were analysed for 5 beta-pregnane-3 alpha, 20 beta-diol, and other pregnanediols using gas chromatography-high resolution mass spectrometry. None was detected in any of the specimens and therefore it is unlikely to be the inhibitory factor in bilirubin conjugation. The plasma osmolalities of the infants, which were determined at the onset of jaundice, were within normal limits.
