The unsatisfactory results of radical operations in advanced anal cancer could be bettered by earlier diagnosis of the malignant lesion. The possibility of cancer should always be kept in mind when treating any of the common chronic anal diseases. Preliminary biopsy studies should be done more frequently if suspicious lesions are present and all tissues removed during minor anorectal operations should always be examined by a pathologist. BACKGROUND: A dramatic case of infected emphysema involving multiple deep cervical and mediastinal fascial planes following surgical extraction of a lower third molar in an otherwise healthy man is presented.
METHODS: The differential diagnosis and management of this condition are discussed, specifically contrasting it to necrotizing fasciitis.
RESULTS: The direct cause of this complication was the use of an air turbine handpiece and air syringe for the tooth extraction.

Necrotizing fasciitis is a severe infection with a mixed bacterial population involving a single tissue plane. It is best diagnosed by the history of minor trauma, extreme toxicity, anesthesia of the skin and roentgenologic evidence of soft tissue gas. During the 25 years 1952--77 31 surgical procedures were performed on children aged up to 16 years with sickle cell anaemia (SCA). Six emergency operations were carried out, all for complications of SCA indistinguishable from the acute surgical conditions they mimicked.
