Analysis of prognostic factors associated with longitudinally extensive transverse myelitis.

OBJECTIVE: The aim of this study is to report the clinical profile and outcome of longitudinally extensive transverse myelitis (LETM). METHODS: We prospectively studied adult patients who presented with LETM from January 2008 to December 2011. Information on demographic, clinical course, magnetic resonance imaging (MRI) and outcome was collected. HLA-DRB1 genotype was compared with those of 225 normal controls and patients with MS (228) and neuromyelitis optica (NMO) (22). RESULTS: In total, 23 patients (16 female) with a median age of 44.5 years (range: 20-77 years) were included. Most (74%

Diffusion tensor imaging in pediatric transverse myelitis: a case study.

Transverse myelitis is diagnosed based on the presence of spinal cord inflammation and the absence of infection to the central nervous system. In support of these criteria, patients undergo lumbar puncture to determine Cerebrospinal Fluid (CSF) pleocytosis and un-enhanced or Gadolinium-enhanced spinal Magnetic Resonance Imaging (MRI). We present the case of an 11~year-old previously healthy male who underwent a series of lab tests and MRI scans before a definite diagnosis of transverse myelitis four years prior to this study. The patient still shows deficits at the C4 cord level according to I

A case of longitudinally extensive transverse myelitis (LETM): neuromyelitis optica.

Neuromyelitis optica (NMO), characterised by longitudinally extensive transverse myelitis (LETM), was previously thought to be a variant of multiple sclerosis. Transverse myelitis may be a manifestation of autoimmune connective tissue diseases and NMO is now recognised to be a humorally mediated autoimmune disease. We present a case of NMO associated with non-organ-specific autoantibodies and the absence of the characteristic NMO-IgG antibody. Our case provides an opportunity to review the diagnostic criteria of NMO and its distinction from other autoimmune and demyelinating conditions. We rep

Neuromyelitis optica IgG predicts relapse after longitudinally extensive transverse myelitis.

OBJECTIVE: We investigated whether neuromyelitis optica (NMO) IgG seropositivity at the initial presentation of longitudinally extensive transverse myelitis (LETM) predicts relapse of myelitis or development of optic neuritis. METHODS: Prospective study of patients with initial LETM who were tested for the presence of NMO-IgG. RESULTS: Eleven of 29 patients (37.9%) were seropositive after a first attack of LETM spanning three or more vertebral segments on magnetic resonance imaging. Of 23 patients followed up for 1 year, none of 14 who were seronegative experienced a relapse or developed opt

Acute transverse myelitis: a retrospective study using magnetic resonance imaging.

The prognostic value of magnetic resonance imaging (MRI) in the syndrome of acute transverse myelitis had not been evaluated. After retrospective study of 14 cases, we found that MRI is valuable for both diagnosis and prognosis in this illness. The criteria for the diagnosis of acute transverse myelitis consisted of acute onset (over less than 3 weeks) of symmetrical motor and sensory dysfunction referable to a distinct spinal cord level, with sphincter dysfunction. Patients with abnormal MRIs of the spinal cord had significantly worse outcomes than patients with normal MRIs.

Post-Infectious Acute Transverse Myelitis in a COVID-19 Patient.

A 41-year-old man with no comorbidities presented with features consistent with a subacute partial upper thoracic myelopathy, two days after treatment in the emergency department for acute urinary retention. On examination, he was ambulatory but had an unsteady gait and lower limb weakness, with brisk reflexes, upgoing plantars, and subtle sensory changes at the T4-T5 level. The patient developed neurological symptoms eight days after the initial onset of COVID-19. An urgent magnetic resonance imaging (MRI) of the spine demonstrated a T2 hyperintense signal extending from C3 to the conus, whil

A case report of acute idiopathic longitudinal extensive transverse myelitis.

Transverse myelitis is a rare disease characterized by inflammation of the spinal cord. These findings may present with findings related to primary motor neuron damage or with nonspecific findings, such as loss of muscle strength and sensation in the extremities. Herein, we presented a 21-year-old female patient with transverse myelitis with complaints of muscle weakness and urinary incontinence who was diagnosed with lumbar disc herniation after lumbar imaging and whose complaints did not improve after surgery.

Tuberculous longitudinal extensive transverse myelitis: 2 cases and literature review.

CONTEXT: This study aimed to investigate the characteristics, diagnosis, and management of tuberculous longitudinally extensive transverse myelitis (TB-LETM), a rare manifestation of tuberculosis. FINDINGS: We analyzed two rare cases of TB-LETM and discussed their clinical manifestations and imaging findings in the context of the relevant literature. Patient 1, a 23-year-old female, presented with quadriplegia and dysuria, and spinal magnetic resonance imaging (MRI) revealed lesions extending from C1 to T3. Patient 2, an 18-year-old female, reported acute-onset numbness and weakness in both l

Childhood Transverse Myelitis with Atypical Fever Presentation: A Case Report.

Transverse myelitis (TM) is a rare inflammatory spinal cord disorder, particularly uncommon in children. It is characterized by symptoms such as motor weakness, sensory disturbances, and autonomic dysfunction. This report describes a 10-year-old male presenting with bilateral lower limb weakness, urinary and fecal incontinence, and high-grade fever. Initial treatment at a local hospital with corticosteroids and antibiotics did not yield significant improvements, prompting advanced care at a tertiary facility. A magnetic resonance imaging (MRI) confirmed a longitudinally extensive TM lesion. Su

Can Failed Back Surgery Syndrome Be Healed by Transverse Myelitis?

Failed back surgery syndrome (FBSS) is a condition characterized by persistent or recurring back pain following spinal surgery. Etiological factors for FBSS are being studied by investigators and clinicians in an attempt to organize them based on their temporal relation to the surgery event. However, many questions regarding the pathophysiology of FBSS remain, which has resulted in a lack of efficacy among its treatment options. In this report, we present a remarkable case of longitudinally extensive transverse myelitis (LETM) in a patient with a medical history of FBSS who was taking multiple

Rare case of acute transverse myelitis associated with Lyme neuroborreliosis.

Acute transverse myelitis is a neurological disorder that leads to acute spinal cord injury due to inflammation caused by autoimmune disorders or by parainfectious etiologies. Among the many different infectious causes of transverse myelitis, one of the rare ones is Lyme disease. Here we describe a case of a 62 year old male who presents with bilateral paresthesia and weakness. MRI imaging of the cervical and thoracic spine led to the initial diagnosis of cervical cord edema leading to the symptoms associated transverse myelitis. However further workup of different infectious causes lead to po

Concurrent tuberculous transverse myelitis and asymptomatic neurosyphilis: A case report.

BACKGROUND: Tuberculous myelitis is a rare manifestation of tuberculosis (TB) that is usually caused by hematogenous spread of Mycobacterium tuberculosis (MTB). Neurosyphilis is a neurological disease that occurs when Treponema pallidum invades the brain or the spinal cord. Individually, these two diseases involving the spinal cord are rare and cases of concurrent tuberculous transverse myelitis and asymptomatic neurosyphilis have seldom been reported. CASE SUMMARY: A 56-year-old man presented with numbness and pain of both lower limbs for 2 wk and dysuria for 1 wk. Syphilis serology and cere

Tubercular longitudinally extensive transverse myelitis with lower motor neuron paralysis.

Transverse myelitis typically extends two or less spinal segments, whereas longitudinal extensive transverse myelitis (LETM) extends three or more spinal segments in length and may occasionally span all the segments of the spinal cord. We present a case of spinal tuberculosis presenting with LETM with true lower motor neuron-type flaccid paraplegia.

Longitudinally Extensive Transverse Myelitis Due to Toxoplasma: An Autopsy Study.

Toxoplasma is an obligate intracellular parasite that remains asymptomatic in humans but, at times, can cause devastating disease. Here, we describe an autopsy study of a young immunocompetent gentleman with no comorbidities whose presentation was acute transverse myelitis. Magnetic resonance imaging spine showed longitudinally extensive spinal cord lesion (LESCL) that mimicked neuromyelitis optica with normal brain imaging at presentation. Investigations showed albuminocytological dissociation which prompted a course of parenteral steroid. However, the lesion relentlessly progressed to involv

Utility of extension views in spondylotic myelopathy mimicking transverse myelitis.

Cervical spondylotic myelopathy is a common cause of myelopathy and may mimic transverse myelitis. We report a 55 year-old lady with subacute myelopathy initially treated with glatiramer acetate for suspected clinically isolated syndrome. MRI head and spine revealed a single short cervical cord T2-hyperintense lesion with enhancement just below a region of moderate stenosis. Cerebrospinal fluid revealed elevated oligoclonal bands. Repeat MRI 7 months later showed persistent enhancement. Dynamic MRI revealed cord compression during extension. Surgical decompression resulted in marked clinical a

Longitudinally extensive transverse myelitis with and without aquaporin 4 antibodies.

IMPORTANCE: Aquaporin 4 antibody (AQP4-Ab)-negative patients with longitudinally extensive transverse myelitis (LETM) behave differently from those with AQP4-Ab. Aquaporin 4 antibody-negative neuromyelitis optica (NMO) is rare when good assays are used. OBJECTIVE: To assess if AQP4-Ab-negative patients with LETM share similar disease characteristics with AQP4-Ab-positive patients or whether they have distinct features and alternative diagnoses. DESIGN We collated clinical and paraclinical data on patients with LETM identified through the Oxford NMO clinical database. Aquaporin 4 antibodies we
