Single- and multiple-dose pharmacokinetics of long-acting injectable naltrexone.

BACKGROUND: Oral naltrexone is effective in the treatment of alcohol dependence; however, a major limitation of its clinical utility is poor patient adherence to the daily dosing schedule. A biodegradable, long-acting naltrexone microsphere formulation was developed to achieve continuous naltrexone exposure for 1 month in the treatment of alcohol dependence. METHODS: The single- and multiple-dose safety and pharmacokinetics of a long-acting naltrexone microsphere preparation were evaluated in healthy subjects. One group of subjects (n = 28) received a single dose of oral naltrexone 50 mg foll…

Ethanol familiarity and naltrexone treatment affect ethanol responses in rats.

In the present study, the effects of ethanol familiarity on the ability of naltrexone to alter ethanol palatability and consumption were examined. One group of rats was allowed continuous access to 10% vol/vol ethanol and water for 3 weeks. A second group received only water. At the end of this time, the groups were further subdivided and injected with either 3mg/kg naltrexone or saline (total of four groups; n=11-13 per group) before ethanol taste reactivity tests with 10% vol/vol ethanol and ethanol consumption tests. Results showed that naltrexone effectively decreased ingestive responding …

Combined therapy: what does acamprosate and naltrexone combination tell us?

AIMS: Relapse prevention treatment with both acamprosate and naltrexone has been shown to be efficacious in the treatment of alcoholism. Whereas both compounds act pharmacologically differently, there is up to now only limited evidence as to whether combined treatment is efficacious and pharmacologically safe. It remains to be answered whether data justify the combination of both drugs in clinical practice. METHODS: Review of the three pre-clinical and four clinical studies that have been published to date on either combined tolerability or efficacy. RESULTS: Data available up to now show no…

Naltrexone attenuates plasma nitric oxide release following acute heat stress.

Previous studies have shown that naltrexone attenuates morbidity and mortality in heat stress by inhibiting endogenous opioids. In this study, we hypothesized that naltrexone can decrease heat stress by attenuating nitric oxide release. Male Sprague-Dawley rats were pretreated with naltrexone or normal saline, and exposed to 45 degrees C for 25 min; controls were exposed to 25 degrees C. Colonic temperatures were recorded and plasma samples from an in-dwelling i.v. cannula were analyzed for nitrate/nitrite levels. Following heat stress, peak colonic temperature was significantly diminished (P …

Placebo-controlled acute dosage naltrexone study in young autistic children.

In a double-blind, placebo-controlled crossover trial 23 autistic children were treated with a single 40-mg dose of the opiate antagonist naltrexone. Drug effects were monitored by detailed playroom observations, actometers, and parents' checklist ratings (Aberrant Behavior Checklist, social items and target behaviors). Naltrexone treatment failed to produce significant changes in social behavior, but it did reduce irritability and target scores on behavior checklists. The playroom data indicated that naltrexone significantly affected indices of activity and attention.

Naltrexone attenuates the antiparkinsonian effects of picoTesla range magnetic fields.

Extracranial treatment with magnetic fields (MF) in the picoTesla range has been shown an efficacious treatment modality in the management o … f the opioid systems and following informed consent, we administered the opiate receptor antagonist naltrexone (50 mg, P.O.) to a Parkinsonian patient after he showed improvement of symptoms with application of MF. Results of the trial showed that naltrexone partially reversed the antiparkinsonian effects of MF thus suggesting that opioid peptides are involv …

Quaternary naltrexone reveals the central mediation of conditional opioid analgesia.

Earlier research has demonstrated that when rats are placed in a context associated with mild electric shock (1 mA/0.75 sec), environmental cues alone can produce conditional analgesia that suppresses pain sensitivity on the formalin test. This analgesia appears to be mediated by endogenous opioids since it is reversed by the centrally active opioid antagonists naloxone and naltrexone. Two experiments attempted to determine if peripheral or central opioids mediate this analgesia by employing quaternary naltrexone (QNTX), an antagonist which does not readily penetrate the blood-brain barrier at…

Short-term effects of naltrexone in 155 heroin ex-addicts.

The narcotic antagonist naltrexone was administered for periods of up to 8 months to a total of 155 patients at a dose of 40-200 mg per day. The antagonistic effect of naltrexone was tested by injections of heroin. Eighty milligrams of natrexone was effective for 48hr. The antagonistic effect decreased at 72 hr after the administration of 120-200 mg of naltrexone. Laboratory tests indicated no signs of toxicity. Naltrexone may elicit an increase in blood pressure and opigastric pain. Neither of these side effects appear clinically important. No signs of dependence on naltrexone were detected. …

Therapeutic Uses and Efficacy of Low-Dose Naltrexone: A Scoping Review.

Low-dose naltrexone (LDN) has been suggested as a novel treatment option for several conditions and is of increasing interest due to its potential ability to address certain medical conditions that lack effective treatments or frequently rely on the use of opioids as treatment. This article will synthesize evidence and assess the scope of literature that examines low-dose naltrexone's efficacy against painful and other relevant medical conditions. A scoping review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-Sc…

Neuroprotective effects of naltrexone in a mouse model of post-traumatic seizures.

Traumatic Brain Injury (TBI) induces neuroinflammatory response that can initiate epileptogenesis, which develops into epilepsy. Recently, we identified anti-convulsive effects of naltrexone, a mu-opioid receptor (MOR) antagonist, used to treat drug addiction. While blocking opioid receptors can reduce inflammation, it is unclear if post-TBI seizures can be prevented by blocking MORs. Here, we tested if naltrexone prevents neuroinflammation and/or seizures post-TBI. TBI was induced by a modified Marmarou Weight-Drop (WD) method on 4-week-old C57BL/6J male mice. Mice were placed in two groups: …

Naltrexone at low doses (LDN) and its relevance to cancer therapy.

INTRODUCTION: Naltrexone was designed to inhibit opioid receptors without activating them and hence used to block the stimulatory effects of morphine and heroin. It was noted that in certain patients being treated with naltrexone for an opioid addiction many reported significant secondary benefit when being weaned off naltrexone. This group of patients had chronic inflammatory and autoimmune conditions and reported improvements whilst using the lower dosages of naltrexone. There have also been recent anecdotal reports of cancer resolution following the use of low doses of naltrexone (LDN). How…

Simultaneous Transdermal Delivery of Buprenorphine Hydrochloride and Naltrexone Hydrochloride by Iontophoresis.

The opioids buprenorphine hydrochloride (BUP) and naltrexone hydrochloride (NTX) show promise as a combination treatment for addiction, but no means of delivering the two compounds in one medicine currently exist. In this paper, we report sufficient input rates of both these drugs from one iontophoretic transdermal drug delivery system. Experiments were performed using dermatomed pig skin mounted in glass side-bi-side cells. BUP and NTX were iontophoretically delivered together from the anode using direct constant current from Ag/AgCl electrodes. The transdermal drug fluxes and the masses of d…

Effect of combined naltrexone and bupropion therapy on the brain's functional connectivity.

BACKGROUND: The control of food intake in environments with easy access to highly rewarding foods is challenging to most modern societies. The combination of sustained release (SR) naltrexone and SR bupropion (NB32) has been used in weight-loss and obesity management. However, the effects of NB32 on the brain circuits implicated in the regulation of food intake are unknown. Here we used functional connectivity density (FCD) mapping to evaluate the effects of NB32 on resting brain FC. METHODS: Thirty-six healthy women underwent magnetic resonance imaging (MRI) before and after 4-week treatment…

Difference of the naltrexone's effects in social drinkers by spicy food preference.

The purpose of this study was to investigate the differences in subjective acute effects of alcohol and naltrexone among those who prefer spicy food to varying degrees. Acute biphasic alcohol effects scale (BAES), visual analogue scale for craving (VAS-C), blood alcohol concentration (BAC) and food preference scale were measured in 26 men. Repeated measures ANOVA (2 preference groups×4 time blocks) on the stimulative subscale of BAES revealed a significant group by block interaction in naltrexone condition (N+) (P<0.001), but not in non-naltrexone condition (N-). Furthermore, repeated measures…
