Serous Cystadenoma of the Pancreas With Complex Florid Papillary Architecture: A Case Report and Review of the Literature.

Serous cystadenoma (SCA) is a relatively rare benign pancreatic neoplasm. It has a very distinctive gross and microscopic appearance including pure and mixed microcystic and macrocystic patterns as well as rare solid architectural pattern. In this article, we present a rare case of SCA with a complex florid papillary architecture. A 40-year-old man was diagnosed with a 3.5 cm SCA of the uncinate process of the pancreas based on abdominal computed tomography scan. The tumor was monitored radiographically until recent magnetic resonance imaging showed a new 1.0-cm eccentric mural nodule within t

Multimodal Treatment Approach for Mixed Neuroendocrine-Non-Neuroendocrine Neoplasms of the Gallbladder: A Patient with a Favorable Survival Outcome.

Mixed neuroendocrine-non-neuroendocrine neoplasm of the gallbladder (GB-MiNEN) is an exceptionally rare and aggressive tumor with no established treatment guidelines. We herein present the case of a 53-year-old woman with GB-MiNEN who was treated with irinotecan and cisplatin therapy, which led to a significant tumor reduction and subsequent conversion surgery. Despite a later recurrence, multimodal therapy extended survival for over two and a half years. This case underscores the potential benefit of conversion surgery combined with systemic chemotherapy, thus suggesting that multimodal treat

Transition between urothelial carcinoma in situ and non-invasive micropapillary carcinoma as a pivot connection between diverse morphologies of bladder carcinoma: a case report of urothelial carcinoma with villoglandular differentiation.

Urothelial carcinoma has numerous histological variants, and these variants may coexist in a single case. Here, we present a case of a 70-year-old man with urothelial carcinoma of the bladder with a maximal diameter of 5 mm that involved micropapillary and plasmacytoid variants, with villoglandular differentiation. The presence of these variants was confirmed by pathological examination of a transurethral resection specimen, and high-grade urothelial carcinoma was found as a minor component. Although this bladder carcinoma was classified as pT1, cystoprostatectomy, urethrectomy, and lymphadene

Large cell neuroendocrine carcinoma of the ampulla of Vater with glandular differentiation.

Large cell neuroendocrine carcinoma of the ampulla of Vater is extremely rare. A 55 year old woman presented with an ampullary tumour causing pancreaticobiliary obstruction and a pancreaticoduodenectomy was performed. Microscopically, the tumour was diagnosed as a CD117 positive large cell neuroendocrine carcinoma with glandular differentiation. Four months later the patient developed a general recurrence. The metastatic tumours showed CD117 negativity and pure neuroendocrine features. The patient died of disease six months after diagnosis. It is postulated that the two components originated f

Long-term survival after chemotherapy combined immunotherapy for recurrent mixed neuroendocrine-non-neuroendocrine neoplasms of the common bile duct.

Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) of the common bile duct (CBD) are extremely rare, with few literature reported. A 68-year-old male was admitted to our hospital with choledocholithiasis with acute cholangitis. Abdominal computed tomography showed that the CBD was occupied and clearly dilated. Consequently, the patient underwent pancreaticoduodenectomy with regional lymph node dissection. The resected tumor at the distal bile duct was 25 × 25 × 13 mm, consisting of 30% adenocarcinoma and 70% neuroendocrine carcinoma microscopically. The patient then received six cycles

Tubulopapillary Cystic Adenoma With Apocrine Differentiation: A Unifying Concept for Syringocystadenoma Papilliferum, Apocrine Gland Cyst, and Tubular Papillary Adenoma.

Syringocystadenoma papilliferum (SCAP), apocrine gland cyst (AGC, also called apocrine hidrocystoma or apocrine cystadenoma), and tubular papillary adenoma (TPA) with apocrine differentiation are defined as proliferations of apocrine epithelium with myoepithelial cells. At Sapporo Dermatopathology Institute, we retrieved 308 benign neoplastic lesions diagnosed as SCAP, AGC, or TPA and combinations of these entities. Among the 308 lesions, 202 (66%) exhibited features of only one type, of which 144 (47%) were AGC, 39 (13%) were TPA, and 19 (6%) were SCAP. The other 106 lesions (34%) had feature

Fine-needle aspiration biopsy of pleural metastases from a carcinosarcoma or true malignant mixed tumor of the parotid gland mimicking a mesothelioma.

Malignant mixed tumor of the parotid is known to have odd sites for metastases. We describe the fine-needle aspiration cytology (FNAC) findings of pleural metastasis from a malignant mixed tumor misdiagnosed as a mesothelioma on cytology at the onset.A 47-year-old man presented to us with breathlessness and a massive pleural effusion with pleural-based nodules. He had been operated 2 years before for a pleomorphic adenoma of the parotid and had a healthy scar at that site. FNAC from the pleural lesions showed myxoid background substance with entrapped cuboidal epithelial cells with atypical nu

Parotid Salivary Duct Carcinoma With a Prominent Squamous Component: Immunohistochemical Profile, Diagnostic Pitfalls, and Therapeutic Implications.

Salivary duct carcinoma of the parotid gland is a highly aggressive epithelial malignancy morphologically resembling high-grade, invasive, and in situ breast carcinoma. It can occasionally present with variable morphology making it diagnostically challenging in cases with unusual morphological components. Ancillary testing, particularly androgen receptor (AR) positivity on immunohistochemistry, can be very helpful in cases that demonstrate extensive squamous morphology, since AR positivity is uncommon in both the primary salivary gland and metastatic squamous cell carcinomas to the parotid. Molecular identification of a new splicing variant of the MLL - MLLT11 fusion transcript in an adult with acute myeloid leukemia and t(1;11)(q21;q23).

More than 70 different mixed lineage leukemia (MLL) rearrangements involving 11q23 have been molecularly characterized in acute leukemia. Among these, the MLLT11 gene is highly unique as MLL fusion partner because the entire open reading frame is usually fused in-frame to the N-terminal portion of the MLL gene. By using molecular genetic methods, we identified the chromosomal fusion site within MLL exon 10 sequences which were fused to the MLLT11 intron 1 sequences. This unusual break site results in the creation of two in-frame MLL-MLLT11 fusion transcripts in this acute myeloid leukemia pati

Neonatal Intra-Abdominal Mixed Germ Cell Tumour with Retroperitoneal Extension Mimicking Hepatoblastoma: A Diagnostic Challenge - A Case Report.

INTRODUCTION: Neonatal germ cell tumours (GCTs) are rare, and intra-abdominal mixed GCTs are even less common. Although most solid tumours in neonates are benign, malignant cases occur at an estimated rate of 1 in 27,500 live births, accounting for about 2% of all childhood cancers. Differentiating benign from malignant neonatal neoplasms is a significant diagnostic challenge due to overlapping clinical and radiological features. We report one of the few documented cases of an intra-abdominal mixed GCT in an Indian neonate, highlighting the importance of early recognition and a multidisciplina

Gastric Mixed Neuroendocrine-Non-neuroendocrine Neoplasms Develop from POU2F3+ Multipotent Progenitors.

Gastric mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are aggressive malignancies that exhibit complex cellular heterogeneity driven by the dual lineages. The dismal patient survival rates highlight the need to elucidate the cellular origin of gastric MiNENs and the key regulatory mechanisms to develop improved treatment strategies. Here, we identified a gastric POU2F3+epithelial population as a potential cell of origin for gastric MiNENs. Lineage tracing and single-cell RNA sequencing further identified that POU2F3+ cells include three distinct subpopulations with divergent diffe

Urachal carcinomas of the nonglandular type: salient features and considerations in pathologic diagnosis.

The vast majority of malignant urachal epithelial tumors have a glandular morphology (ie, adenocarcinoma), to which our principal understanding of urachal carcinoma and its prevailing set of diagnostic criteria are largely ascribed. The 2004 World Health Organization classification of genitourinary tumors recognizes other rarer histologic types of urachal carcinomas such as urothelial, squamous cell, and other carcinomas. However, the clinicopathologic data for these nonglandular groups of urachal carcinomas are very limited, being detailed only in sporadic case reports. Some of the criteria r

Intraductal papillary neoplasm of the bile duct accompanying biliary mixed adenoneuroendocrine carcinoma.

We present the first case of an intraductal papillary neoplasm of the bile duct (IPNB) accompanying a mixed adenoneuroendocrine carcinoma (MANEC). A 74-year-old woman presented with fever of unknown cause. Laboratory data revealed jaundice and liver injury. Contrast-enhanced computed tomography revealed a 20 mm polypoid tumor in the dilated distal bile duct, which exhibited early enhancement and papillary growth. Upper gastrointestinal endoscopy revealed mucus production from the papilla of Vater, characterized by its protruding and dilated orifice. Endoscopic ultrasonography visualized the po

BACKGROUND: The human "Elston and Ellis grading method" was utilized in dogs with mammary tumor to examine its relation to prognosis in this species, based on a 2-year follow-up period. Although cytopathology is widely used for early diagnosis of human neoplasms, it is not commonly performed in veterinary medicine. Our objectives in this study were to identify cytopathology criteria of malignancy for canine mammary tumors and the frequency of different types of mammary lesions and their relationship with histologic grade was investigated. Another aim of this study was to differentiate the simp

Appendiceal goblet cell carcinoid: common errors in staging and clinical interpretation with a proposal for an improved terminology.

Goblet cell carcinoid (GCC) is staged and treated as adenocarcinoma (AC) and not as neuroendocrine tumor (NET) or neuroendocrine carcinoma. The term carcinoid may lead to incorrect interpretation as NET. The aim of the study was to explore pitfalls in staging and clinical interpretation of GCC and mixed GCC-AC, and propose strategies to avoid common errors. Diagnostic terminology, staging, and clinical interpretation were evaluated in 58 cases (27 GCCs, 31 mixed GCC-ACs). Opinions were collected from 23 pathologists using a survey. Clinical notes were reviewed to assess the interpretation of p
