Gliomatosis cerebri in a young patient showing various cranial nerve manifestations: a case report.

A case of gliomatosis cerebri in a 27-year-old man showing various cranial nerve manifestations is described. He was diagnosed as having cranial mononeuritis multiplex (bilateral oculomotor nerve paralysis, left facial nerve paralysis, bulbar palsy manifestations, and hypoglossal nerve paralysis) and was hospitalized in the neurology department on August 1, 2000. Although he continued to visit the neurology department after discharge, his manifestations showed no improvement. He was sent to our department for brain biopsy in August 2001. A biopsy performed at the Sylvian fissure from the front

Astroblastoma-like tumor with an EWSR1::BEND2 fusion in the fourth ventricle: a case report and narrative review.

PURPOSE: To report an EWSR1::BEND2 fusion astroblastoma-like tumor arising in the fourth ventricle of a 7-year-old girl and to highlight the diagnostic value of molecular pathology in astroblastoma-like tumors with an atypical immunophenotype. METHODS: Clinical, histopathological, immunohistochemical, and molecular findings of the present case are presented. RESULTS: Despite gross total resection followed by radiotherapy, the patient died 7 months after surgery. Histologically, the tumor showed typical astroblastoma-like morphology but an unusual immunophenotype characterized by complete los

Recent Advances in Renal Medullary Carcinoma.

Renal medullary carcinoma (RMC) is a rare renal malignancy that has been associated with sickle hemoglobinopathies. RMC is aggressive, difficult to treat, and occurs primarily in adolescents and young adults of African ancestry. This cancer is driven by the loss of SMARCB1, a tumor suppressor seen in a number of primarily rare childhood cancers (e.g., rhabdoid tumor of the kidney and atypical teratoid rhabdoid tumor). Treatment options remain limited due in part to the limited knowledge of RMC biology. However, significant advances have been made in unraveling the biology of RMC, from genomics

Dysembryoplastic neuroepithelial tumor and temporal arachnoid cyst on a child with epilepsy. Which one to operate? Case report and literature review.

A 6 year-old male, with seizures characterized by abnormal epigastric sensation, behavioral arrest, upper extremities search automatisms and secondary tonic-clonic generalization. Magnetic resonance imaging showed a hypointense cystic extra-axial image with an increase in the thickness of the convolutions in the first and second gyri of the right frontal lobe. It was decided to resect the frontal lesion with transoperative motor mapping. Morphological and immunohistochemical findings corresponded to dysembryoplastic neuroepithelial tumor with focal cortical dysplasia. Adequate semiology, analy

An Adolescent Presenting With Seizures as a Symptom of Gliomatosis Cerebri.

Gliomatosis cerebri is a diffuse infiltrating glioma of neuroepithelial origin that affects more than 2 cerebral lobes. This is rarely seen in pediatric patients. The clinical presentation and imaging are very unspecific, and a biopsy is typically needed for the diagnosis. Given the widespread nature of the disease, surgical treatment is not possible and has a poor overall prognosis. A pediatric patient presented with elevated intracranial pressure. All initial studies were negative, and the imaging showed a symmetrical affection involving the supratentorial and infratentorial regions. A biops

Primary spinal cord astroblastoma: case report.

Astroblastoma is a rare tumor that is thought to occur exclusively in the cerebrum. To the authors' knowledge, no cases of spinal cord astroblastoma have been reported. A 20-year-old woman presented with numbness in her legs. MRI demonstrated a 2-cm intramedullary enhancing lesion in the spinal cord at the T-1 level. The patient declined to undergo resection of the tumor because she was able to walk unassisted; however, she returned for surgery 1 month later because she had developed paraplegia with bladder and rectal dysfunction, and MRI showed enlargement of the tumor. Intraoperatively, the

Recurrent low-grade astroblastoma with signet ring-like cells and high proliferative index.

Astroblastoma is a rare primary glial tumor of children and young adults. Radiologically astroblastoma presents as a large well-circumscribed supratentorial, solid-cystic heterogeneous mass. Histology shows perivascular pseudorosettes with hyalinization. Only a single case has been reported with signet-ring-like cell morphology. Signet-ring morphology in primary central nervous system tumors is exceedingly rare. Complete surgical resection is the recommended treatment. Prognosis of astroblastoma depends on the extent of resection and histology. The proliferative index may be a useful tool to d

The clinical value of proton magnetic resonance spectroscopy in adult brain tumours.

Proton magnetic resonance spectroscopy (1H MRS) non-invasively provides information on the biochemical profile (typically including up to nine metabolites and mobile lipids) of brain tissue, which varies according to the underlying disease process. A number of studies have assessed its accuracy in the diagnosis of adult brain tumours. This article describes the basic principles of 1H MRS, the metabolic profiles of different brain tumours, and practical points to aid interpretation of spectra. The literature is reviewed regarding the role of 1H MRS in the diagnosis of brain tumours and more spe

Malignant Transformation of a Rosette-Forming Glioneuronal Tumor to Glioblastoma.

BACKGROUND: A rosette-forming glioneuronal tumor (RGNT), a rare brain tumor, presents as a benign feature with a favorable outcome. To date, a few cases with aggressive behaviors, such as recurrence or dissemination, but none with malignant transformation, have been reported. We describe 1 case that recurred as glioblastoma after complete resection of the benign RGNT. CASE DESCRIPTION: A man aged 58 years presented with headache and dizziness without neurologic symptoms. Magnetic resonance imaging showed a 4 × 2.5 cm, well-demarcated mass in the left cerebellar hemisphere. The patient underwe

Malignant nonteratoid medulloepithelioma of the ciliary body in an adult.

BACKGROUND: The authors describe a 62-year-old man with a malignant nonteratoid malignant medulloepithelioma of the ciliary body with extension into the anterior choroid and posterior chamber. He developed gradual loss of vision in his right eye, and a clinical diagnosis of malignant melanoma of the ciliary body and choroid was made. RESULTS: Histopathologic examination of the enucleated eye showed a malignant nonteratoid medulloepithelioma of the ciliary body with extension into the anterior choroid and posterior chamber. CONCLUSION: This report provides clinicopathologic evidence that intr

[Dysembryoplastic neuroepithelial tumors].

Dysembryoplastic neuroepithelial tumors DNTs are highly polymorphic tumors that arise during embryogenesis. They are preferentially, but not exclusively, located in the supratentorial cortex. Histologically they may mimic any categories of low-grade or even of high-grade gliomas, but from a carcinological point of view, they behave as stable lesions. Their differential diagnosis from gliomas is obviously important to spare these young patients with a normal life expectancy the long- term deleterious effect of radiation or chemotherapy. The diagnosis of DNT must be considered when all the follo

Immunohistochemical analysis of ghrelin expression in various types of adrenal tumors.

INTRODUCTION: Ghrelin, originally isolated from the endocrine cells of the gastric mucosa, is also expressed in many peripheral tissues, including normal adrenals and adrenocortical tumors. It was shown that ghrelin stimulates proliferation and inhibits apoptosis of adrenocortical cells. In the current study, we compared ghrelin expression at the protein level in various adrenal tumors. We analyzed whether immunoreactive ghrelin could be considered as a potential marker for different types of adrenal tumors. MATERIAL AND METHODS: Study was carried out on 200 adrenal specimens arranged on micr

Neuroendocrine components of the bronchopulmonary tract: hyperplasias, dysplasias, and neoplasms.

The dispersed neuroendocrine (NE) system is represented in the bronchopulmonary tract by the solitary neuroendocrine cells and the neuroepithelial bodies (NEBs). Immunohistochemically, neuron-specific enolase, serotonin, bombesin, and calcitonin are demonstrable in both components, whereas leu-enkephalin is demonstrable only in solitary NE cells. The precise function of and interplay between these two components under physiologic and pathologic conditions are not entirely clear. Current indications are that NEBs act as intrapulmonary chemoreceptors sensitive to hypoxia and hypercapnia, whereas
