Nephrocalcinosis in children and adolescents: sonographic evaluation during long-term treatment with 1,25-dihydroxycholecalciferol.

In order to study the presence of possible renal damage, 12 patients who had been chronically treated with 1,25-dihydroxycholecalciferol therapy, 1,25(OH)2D3, for hypoparathyroidism, pseudohypoparathyroidism, and hypophosphatemic rickets were examined by renal ultrasound. Two patients had normal values while 10 subjects showed nephrocalcinosis (NC) of mild, medium, or severe degree. The echographic results have been related to some calcium-phosphorus metabolism and therapeutic parameters. In those subjects with medium to severe NC, the age at the beginning of therapy was significantly younger …

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) with ocular involvement due to a novel CLDN19 variant.

Claudin-16 (CLDN16) and claudin-19 (CLDN19) are essential tight junction proteins in the kidney that are critical for magnesium homeostasis. Mutations in CLDN16 and CLDN19 cause Familial Hypomagnesemia with Hypercalciuria and Nephrocalcinosis (FHHNC), a rare autosomal recessive tubular disorder. The disorder can progressively lead to severe complications, including end-stage renal disease. Affected individuals often present with polyuria, polydipsia, nephrolithiasis, hematuria, muscular tetany, seizures, and failure to thrive. Notably, CLDN19 mutations have also been associated with ocular abn…

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis caused by CLDN16/CLDN19 mutations in four Chinese families.

Familial hypomagnesaemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare autosomal recessive tubulopathy caused by mutations in the CLDN16 or CLDN19 genes, patients usually develop hypomagnesemia, hypercalciuria, nephrocalcinosis and renal failure early in life, and those with CLDN19 gene mutations have ocular findings in addition. Five children from four non-consanguineous Chinese Han families presenting with nephrocalcinosis and renal insufficiency were enrolled in our study. Metabolic profiling and radiology examinations were performed, in addition to whole exome sequencing (WES)…

Phenotypes and the Importance of Genetic Analysis in Adult Patients with Nephrolithiasis and/or Nephrocalcinosis: A Single-Center Experience.

BACKGROUND: Molecular analysis in patients with nephrolithiasis (NL) and/or nephrocalcinosis (NC) enables more accurate evaluation of underlying etiologies. The existing clinical evidence regarding genetic testing in adults with NL comprises only a few cohort studies. MATERIALS AND METHODS: We retrospectively analyzed 49 adult patients diagnosed with NL and/or NC from a single center, on whom we performed a genetic test using a nephrolithiasis panel. We reviewed the phenotype of the patients and compared the cases with positive and negative molecular diagnosis. RESULTS: In total, 49 adult pa…

Heterogeneity is a common ground in familial hypomagnesemia with hypercalciuria and nephrocalcinosis caused by CLDN19 gene mutations.

BACKGROUND: Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare tubulopathy caused by mutations in the CLDN16 or CLDN19 genes. Patients usually develop hypomagnesemia, hypercalciuria, nephrocalcinosis and renal failure early in life. Patients with CLDN19 mutations may also have ocular abnormalities. Despite clinical variability, factors associated with kidney function impairment, especially in patients with CLDN19 mutations, have not been addressed. METHODS: Retrospective multicenter study of 30 genetically confirmed FHHNC Spanish patients. We analyzed kidney fu…

Blue Sclera to Brittle Bones: A Rare Case of Osteogenesis Imperfecta With Dentinogenesis Imperfecta and Nephrocalcinosis.

Osteogenesis imperfecta (OI) is a rare genetic disorder characterized by bone fragility. Its association with dentinogenesis imperfecta (DI) is well documented, but the concurrent presentation with nephrocalcinosis is uncommon and poorly understood. We documented the case of an 18-year-old male presenting with a triad of OI, DI, and nephrocalcinosis. The patient exhibited characteristic features including blue sclera, multiple fractures, dental abnormalities, bowing of long bones, a short stature, and biochemical evidence of altered calcium metabolism. Genetic testing revealed mutations in COL…

Assessment of bone status and bone turnover in pediatric patients with familiar hypomagnesemia with hypercalciuria and nephrocalcinosis.

Familiar hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare monogenic tubulopathy. Although some of its features are potentially harmful for skeletal homeostasis, this problem has not been systematically evaluated so far. To evaluate bone mineral density (BMD) in correlation with selected mineral parameters and bone turnover markers (BTMs) to determine the risk of bone mass loss in pediatric patients with FHHNC. The study comprised 28 FHHNC patients aged 4-18 years and 33 healthy, sex - and age matched controls. 6 FHHNC patients showed normal kidney function whereas the …

Distal Renal Tubular Acidosis: A Conundrum of Short Stature, Failure to Thrive, Rickets, and Nephrocalcinosis.

Distal renal tubular acidosis (dRTA) is an uncommon disorder marked by defective hydrogen ion excretion in the distal nephron, leading to hyperchloremic metabolic acidosis, persistently alkaline urine, and hypokalemia. Chronic, untreated dRTA in children often presents with growth failure, refractory rickets, and nephrocalcinosis, yet is frequently misdiagnosed. We report a 13-year-old girl with severe stunting, bone pain, proximal muscle weakness, and polyuria for several years. Examination revealed significant short stature, malnutrition, widened wrists, a protuberant abdomen, and genu valgu…

The Long Pentraxin PTX3 Is an Endogenous Inhibitor of Hyperoxaluria-Related Nephrocalcinosis and Chronic Kidney Disease.

The long pentraxin 3 (PTX3) exerts a variety of regulatory functions in acute and chronic tissue inflammation. In particular, PTX3 acts as an opsonin for a variety of pathogens and endogenous particles. We hypothesized that PTX3 would exhibit opsonin-like functions toward calcium oxalate crystals, too, and inhibit crystal growth. This process is fundamental in kidney stone disease as well as in hyperoxaluria-related nephrocalcinosis, the paradigmatic cause of chronic kidney disease (CKD) in children with primary hyperoxaluria type I due to genetic defects in oxalate metabolism. Direct effects …

A case control analysis investigating risk factors and outcomes for nephrocalcinosis and renal calculi in neonates.

INTRODUCTION: Studies on outcomes and risk factors for neonatal nephrocalcinosis (NC) and renal calculi (RC) are limited, and often do not include controls for comparison. We conducted a case-control analysis to identify risk factors associated with NC and/or RC in neonates and studied the natural course of these anomalies. STUDY DESIGN: Infants diagnosed with NC/RC on ultrasound within the first year of life and corresponding gestational age- and gender-matched controls were identified from the neonatal intensive care unit database at our institution over a 10-year period. Risk factors asses…

Claudin-19 mutations and clinical phenotype in Spanish patients with familial hypomagnesemia with hypercalciuria and nephrocalcinosis.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis is an autosomal recessive tubular disorder characterized by excessive renal magnesium and calcium excretion and chronic kidney failure. This rare disease is caused by mutations in the CLDN16 and CLDN19 genes. These genes encode the tight junction proteins claudin-16 and claudin-19, respectively, which regulate the paracellular ion reabsorption in the kidney. Patients with mutations in the CLDN19 gene also present severe visual impairment. Our goals in this study were to examine the clinical characteristics of a large cohort of Sp…

A novel mutation of the claudin 16 gene in familial hypomagnesemia with hypercalciuria and nephrocalcinosis mimicking rickets.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is caused by a mutation in the gene CLDN16, which encodes paracellin 1 (claudin-16), atight junction protein mediating paracellular transport which is expressed in the thick ascending loop of Henle and in the distal convoluted tubule, where reabsorption of magnesium occurs. We present a 4 years old Turkish female child with a chief complaint of hypocalcemic tetany. A diagnosis of FHHNC was confirmed by genetic testing for a mutation in claudin 16 gene. Claudin 16 gene revealed homozygosity for the p.K183E(AAA>GAA) C. 547A…

The protective effect of sulfur-containing steroids against nephrocalcinosis induced by mercuric chloride in rats.

1. Experiments were conducted on rats and structure-activity correlations were performed using sulfur-containing steroids against nephrocalcinosis induced by mercuric chloride (HgCl2). 2. The protective effect of spironolactone has been described previously and we have now demonstrated the activity of 7-mercapto-17-hydroxy-3-oxo-4-androsten-17-propionic acid-lactone. 3. The activity of the sulfhydryl radical present in this molecule has been shown to be slightly lower than that of the thioacetyl radical present in spironolactone. 4. The importance of the steroid structure to which a specific r…

Nephrocalcinosis Following Roux-en-Y Gastric Bypass Secondary to Suspected Enteric Hyperoxaluria: A Case Report.

Nephrocalcinosis refers to the deposition of calcium oxalate or calcium phosphate in the kidney, most commonly within the renal medulla, and is frequently linked with underlying metabolic abnormalities. Roux-en-Y gastric bypass (RYGB) is a well-established risk factor for enteric hyperoxaluria secondary to fat malabsorption, thereby increasing the risk of nephrolithiasis, oxalate nephropathy, and nephrocalcinosis. Despite this association, nephrocalcinosis following RYGB remains an uncommon and underrecognized clinical entity. A 36-year-old male with a history of complicated peptic ulcer disea…

A Report of Claudin-19 Mutation Causing Nephrocalcinosis and End-Stage Kidney Disease from Iran.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is an autosomal recessive disorder that affect children and young adults. Mutation in gene that coding the tight junction proteins Claudin-16 and Claudin-19(CLDN19) is responsible of this rare disorder. Hypomagnesemia, hypercalciuria, kidney failure and visual impairment (in CLDN 19 gene mutation) are the most common presentations of FHHNC. Here we present a 31-year-old woman with end-stage kidney disease (ESKD) on routine hemodialysis for the past eight years and was referred to Firoozgar nephrology clinic for kidney tra…
