LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Identify common negative outcomes that arise with conventional nasal reconstruction. 2. Understand the technical refinements that help avoid and reduce negative outcomes in nasal reconstruction. 3. Learn about the utility of regional axial island flaps for nasal reconstruction, in particular, the lateral nasal artery flap.
SUMMARY: Nasal reconstruction has been a preoccupation of surgeons dating to before 600 bc. The nose is the central focal point of the face and a key identifying facial feature, and surg

Sinonasalteratocarcinosarcoma (SNTCS) is a highly malignant and aggressive neoplasm which can quickly damage the local soft tissue, bone tissue, and invade orbital and intracranial tissues. It also can result in local lymph node and distant organ metastasis. The data from 2 cases of SNTCS from the First Affiliated Hospital of Bengbu Medical College (China), treated with radiotherapy and chemotherapy followed by cranio-facial resection, were analyzed since 2016. The main symptoms of the two patients were of the nasal cavity with nasal congestion and runny nose. There was no recurrence after sur

BACKGROUND: Endoscopic nasal and sinus surgery has been used greatly for the treatment of inflammatory and neoplasm pathology, as well as for the approach of the extrasinus structures. Although it has proven to be a very successful method, it is not exempt of complications. Our objective was to identify the frequency of complications in patients with sinus and nasal pathology that underwent an endoscopic surgery of the nose and sinuses.
METHODS: A total of 150 patients who underwent endoscopic sinus surgery, 75 women and 75 men, in a specialties hospital of the Instituto Mexicano del Seguro S

Cocaine is a commonly abused illicit substance frequently administered through intranasal inhalation. Its potent vasoconstrictive effects are well recognized and can result in significant ischemic damage to the sinonasal mucosa. A known complication of chronic intranasal cocaine use is the development of cocaine-induced midline destructive lesions, often leading to perforation of the nasal septum and adjacent structures. Although the destructive sequelae of cocaine use are well documented in the literature, nasal hypertrophy has not been widely reported. In this study, we describe 2 patients w

Squamous cell carcinoma (SCC) of the head and neck can be treated with a combination of modalities. There is an esthetic and functional compromise with midfacial defects secondary to ablative surgery and adjuvant therapies for SCC. Osteoradionecrosis, tissue contracture, and trismus, are all negative side effects of treatment and can impact a patient's function and possibly their nutrition. In this report, we describe a procedure for fabrication of a prosthesis that provides cosmetic improvement and labial competence to maintain caloric intake.

PURPOSE: Non-melanoma skin cancers are the most frequent skin tumours; in 25.5% of cases, they are reported to affect the nasal area. For an excellent surgical outcome, first of all the radical excision of the lesion is important, with appropriate margins of healthy skin in order to avoid recurrences. Moreover is important to achieve a good aesthetical result, avoiding distortion of the aesthetic units and preserving their functions.
MATERIAL AND METHODS: We have applied the modified crescentic flap, described by Smadja in 2007, to 24 nasal skin defects left by oncologic surgery. It consists

INTRODUCTION: Hamartomas are benign tumor-like lesions resulting from incorrectly differentiated germplasm and can manifest in different organ systems. In the nasal cavity and the sinuses these lesions are rare. Only few data on etiology, epidemiology and clinical significance of these tumors exist to date.
MATERIALS AND METHODS: In a retrospective study, material from patients treated in the Clinic for Otorhinolaryngology, Head and Neck Surgery of the Ulm Military Hospital was screened on the incidence and clinical courses of respiratory epithelial adenomatoid hamartomas (REAH) of the nose a

BACKGROUND: Forehead flaps are a workhorse for nasal reconstruction, but complications occur in up to 30% of patients and risk factors are not well characterized. This study aimed to identify risk factors for complications, and provide clinicians a method to stratify patient risk to facilitate shared decision-making.
MATERIALS AND METHODS: This retrospective study included patients who underwent forehead flaps between 2007 and 2020. Demographic and treatment characteristics were abstracted, in addition to clinical outcomes data. Multivariable regression was conducted, with step-wise variable

Benign tumors of the nasal cavity originating from a peripheral nerve sheath are rare. The authors present a case of a 3-month-old boy with a mass in the left nasal vestibule. The surgical resection was complicated by full-thickness necrosis of more than 50% of the nasal ala. The tumor was diagnosed histopathologically as a neurofibroma with glial heterotopia. The details of the nasal reconstruction with a paramedian forehead flap in 3 stages and postoperative results are provided with satisfactory cosmetic and functional results.

Pediatric septorhinoplasty has been an area of controversy because early surgical intervention can prevent normal growth. There are certain conditions where early correction of the nose is indicated, such as in cleft lip nasal deformities, severe traumatic deformities, and congenital nasal lesions. Animal and clinical studies have been helpful in elucidating certain areas of the nose that are potential growth zones that should be left undisturbed when performing nasal surgeries on pediatric patients. We discuss the timing, indications, and surgical technique in pediatric septorhinoplasty.

In subtotal nose reconstruction, different tissues have to be reconstructed. We report the case of a 51-year-old man with a severe deformity of the nose (shortening and collapse of the nose, retraction of the right alar area, and a total septal defect after surgery and radiotherapy for a septum nasi carcinoma). Reconstruction with good results was performed with a "gangplank" flap technique (using a U-shaped skin flap for inner lining and for lengthening), rib cartilage for the nasal skeleton, and skin coverage with a paramedian forehead flap.

After four nasal aesthetic functional surgeries in a period of 18 months, a 46-year-old woman was evaluated who presented with moderate functional alteration, saddle-nose deformity, and total loss of the septal cartilage. Four months before presentation the patient sustained severe nasal trauma, resulting in depression of the nasal bridge without loss of function. Her problem was diagnosed initially as a consequence of an infected septal hematoma and loss of the septal cartilage. Based on this diagnosis, the patient was subjected, in an 18-month period, to four reconstructive surgeries by diff

A rare case of extramedullary plasmacytoma arising from the nasal septum with localized amyloid deposition is reported. A 75-year-old woman presented with a history of post-nasal discharge and nasal obstruction for several months. Endonasal endoscopic observation revealed the presence of a mass arising from her nasal septum with an extension to the nasopharynx. No cervical lymph nodes were palpable. The biopsy specimen was diagnosed as a plasmacytoma (IgG, lambda-light chain type). Both serum myeloma-protein and urine Bence-Jones protein were negative. Bone marrow biopsy, a chest radiograph, t

Nasal dermoid cysts are congenital malformations which result from anomalous embryological development. Two cases occurring in siblings are presented. There have been several previous reports that the condition may be familial but in this report the initial suspicion of the milder anomaly in the younger child was raised primarily because of his older brother's history. This further report suggests that the incidence of familial occurrence of this anomaly may be greater than current literature suggests, and the possibility that other family members may be affected should be remembered by clinic
