Right bundle branch block without overt heart disease predicts higher risk of pacemaker implantation: the study of atomic-bomb survivors.

BACKGROUND: We investigated the clinical course of complete right bundle branch block (RBBB) or RBBB with axis deviation (AD) in terms of subsequent pacemaker implantation for high-degree atrioventricular (AV) block or sick sinus syndrome (SSS). METHODS AND RESULTS: Among the 16,170 atomic-bomb survivors in our biennial health examination between July 1967 and December 2010, we detected 520 newly-acquired RBBB subjects with no organic heart disease, and selected 1038 age- (at RBBB diagnosis) and sex-matched subjects without RBBB to serve as comparison subjects. Multivariate Cox regression ana

Use of BMI guidelines and individual dose tracking to minimize radiation exposure from low-dose helical chest CT scanning in a lung cancer screening program.

RATIONALE AND OBJECTIVES: The increasing use of computed tomography (CT) has been accompanied by rising concerns over potential radiation-related health risks, especially cancer, and a need to minimize such risks. MATERIALS AND METHODS: We conducted 2186 low-dose helical chest CT scans among 1235 nuclear weapons workers at elevated risk of lung cancer, setting the CT scanner tube current at 30 mAs for all participants with BMI <35 kg/m(2) and permitting technologists to raise mAs levels for participants with BMI ≥35 kg/m(2). Dose-length product (DLP) was recorded from the CT scanner, permitti…

A Combat-Relevant Model of Traumatic Hemorrhage in Rats for the Study of Combined Radiation Injury: A Pilot Study.

The threat and consequences of nuclear or radiological events remain a military concern today. It is estimated that 65-70% of weapon-related injuries after a nuclear event will be radiation combined injuries, i. e., acute radiation injury along with hemorrhage and traumatic injuries such as blast or other burns, bone fractures, soft tissue injuries, blood loss, and/or hypoxia. However, little is known about most types of traumatic injuries associated with blood loss, as might occur during combat operations. The primary objective of this pilot study was to develop a new animal model that incorp

Methods and Findings on Diet and Lifestyle Used to Support Estimation of Radiation Doses from Radioactive Fallout from the Trinity Nuclear Test.

The Trinity nuclear test was detonated in south-central New Mexico on 16 July 1945; in the early 2000s, the National Cancer Institute undertook a dose and cancer risk projection study of the possible health impacts of the test. In order to conduct a comprehensive dose assessment for the Trinity test, we collected diet and lifestyle data relevant to the populations living in New Mexico around the time of the test. This report describes the methodology developed to capture the data used to calculate radiation exposures and presents dietary and lifestyle data results for the main exposure pathway

Development of occupational exposure limits for the Hanford tank farms.

Production of plutonium for the United States' nuclear weapons program from the 1940s to the 1980s generated 53 million gallons of radioactive chemical waste, which is stored in 177 underground tanks at the Hanford site in southeastern Washington State. Recent attempts to begin the retrieval and treatment of these wastes require moving the waste to more modern tanks and result in potential exposure of the workers to unfamiliar odors emanating from headspace in the tanks. Given the unknown risks involved, workers were placed on supplied air respiratory protection. CH2MHILL, the managers of the …

Responsibilities and activities of the Advisory Board on Radiation and Worker Health.

As noted in the introductory editorial by Moeller and Toohey, the Energy Employees Occupational Illness Compensation Program Act (EEOICPA) was passed by the U.S. Congress in October 2000 and became effective on 31 July 2001. The Act provides a lump-sum compensation of $150,000 to workers who contracted certain diseases due to exposures to beryllium, silica, or radiation while working for the U.S. Department of Energy (DOE), its contractors, or subcontractors, in the nuclear weapons industry. The Secretaries of DOE, the U.S. Department of Health and Human Services (HHS), and the U.S. Department…

Binding, distribution, and plant uptake of mercury in a soil from Oak Ridge, Tennessee, USA.

A large amount of mercury has been discharged on the U.S. Department of Energy's Oak Ridge Site (Tennessee) as a part of the U.S. nuclear weapon program during the 1950s through the early 1960s. Increases in mercury concentration in fish and in lower East Fork Poplar Creek of Oak Ridge have been recently reported. This is an experimental study mimicking the initial stage of transformation and redistribution of mercury in soils, which are comparable to those of the Oak Ridge site. The objectives of this study were to investigate potential transformation, distribution, and plant uptake of mercur

Effects of radiation on the incidence of prostate cancer among Nagasaki atomic bomb survivors.

Atomic bomb survivors have been reported to have an increased risk of some cancers, especially leukemia. However, the risk of prostate cancer in atomic bomb survivors is not known to have been examined previously. This study examined the association between atomic bomb radiation and the incidence of prostate cancer among male Nagasaki atomic bomb survivors. The subjects were classified by distance from the hypocenter into a proximal group (<2 km), a distal group (≥2 km), and an early entrance group (those who entered the region <2 km from the hypocenter within 2 weeks after the explosion). Bet

Elevated chromosome translocation frequencies in New Zealand nuclear test veterans.

In 1957/58 the British Government conducted a series of nuclear tests in the mid-Pacific codenamed Operation Grapple, which involved several naval vessels from Britain and New Zealand. Two New Zealand frigates with 551 personnel onboard were stationed at various distances between 20 and 150 nautical miles from ground zero. In the present study we applied the cytomolecular technique mFISH (multicolour fluorescent in situ hybridisation) to investigate a potential link between chromosome abnormalities and possible past radiation exposure in New Zealand nuclear test veterans who participated in Op

Mortality among workers at the Los Alamos National Laboratory, 1943-2017.

BACKGROUND: During World War II (WWII), the Manhattan Engineering District established a secret laboratory in the mountains of northern New Mexico. The mission was to design, construct and test the first atomic weapon, nicknamed 'The Gadget' that was detonated at the TRINITY site in Alamogordo, NM. After WWII, nuclear weapons research continued, and the laboratory became the Los Alamos National Laboratory (LANL). MATERIALS AND METHODS: The mortality experience of 26,328 workers first employed between 1943 and 1980 at LANL was determined through 2017. Included were 6157 contract workers employ…

Review of the anatomical basis for predicting plutonium alpha particle radiation induced osteogenic cancers.

Plutonium was discovered and first synthesized in the early 1940's. Several isotopes of plutonium are used in nuclear technologies, 238Pu for heat generation and 239Pu for energy production and weapons. Both isotopes emit alpha particles, which pose a significant radiation hazard when incorporated into the body. Alpha particles emitted during 239Pu decay deposit energy along a very short path in biological tissues (≈45 μm in soft tissues). Thus, defining the anatomical locations of these deposits is essential to identify the cells at risk of radiation damage and potential malignant transformat

Pulmonary injury risk assessment for long-duration blasts: a meta-analysis.

BACKGROUND: Long-duration blasts are an increasing threat with the expanded use of thermobaric and other novel explosives. Other potential long-duration threats include large explosions from improvised explosive devices, weapons caches, and other explosives including nuclear explosives. However, there are very few long-duration pulmonary blast injury assessments, and use of short-duration exposure injury metrics is inappropriate as the injury mechanism for long-duration exposures is likely different from that of short-duration exposures. METHODS: This study develops an injury model for long-d

Requirements of radioprotectors for military and emergency needs.

Uncontrolled exposure to radiation from nuclear weapons, space, or accidents presents challenges unlike those met in radiotherapy. For example, the time and quality of exposure are uncertain. Thus it is necessary to develop radioprotectors that can be given chronically (e.g. as vitamin combinations), or shortly before exposure (e.g. as WR-2721 and related compounds), or after exposure (e.g. as immunomodulators) to cover all possible situations of use. Also, for military purposes, performance capability, as well as survival from acute effects, must be maintained. In contrast to clinical and exp…

Impact of Secondary Prevention on Mortality in the Building Trades National Medical Screening Program: Effectiveness of Occupational High-Risk Management.

BACKGROUND: Since 1997 the Building Trades National Medical Screening Program (BTMed) has offered medical exams to construction workers employed in US nuclear weapons facilities. The process consists of two steps: (1) a detailed work history interview; and (2) a medical exam. Some participants only completed the work history interview, and we compared their mortality experience to those who also completed medical exams. METHODS: We compared the mortality of 3470 work-history-only participants to 23,452 participants who completed both the work history interview and medical exams and, of these,…

High Adherence Over a Decade of Annual Lung Cancer Screening in an Occupational Cohort.

INTRODUCTION: Adherence to an annual low-dose chest CT (LDCT) scan regimen is essential to lung cancer screening success but has been scarcely studied beyond 2 annual CT scans. METHODS: From 2014 to 2024, we conducted annual LDCT screening for 1,498 United States nuclear weapons workers based on age, smoking history, and occupational exposure to lung carcinogens. Labor unions co-sponsored the program in Department of Energy communities. Participants were eligible for up to 8 annual CT scans over a decade. Group adherence at each annual CT scan was calculated as the percentage who completed th…

Chemical alteration of UO2 micro-particles in model lung systems.

Uranium dioxide (UO2) particles can be released from mines, nuclear fuel manufacturing, reactor accidents, and weapons use. They pose inhalation risks, yet their behavior in the human lung remains poorly understood. This study investigates the long-term chemical alteration and dissolution of µm-sized UO2 particles in two model lung fluids: Simulated Lung Fluid (SLF) and Artificial Lysosomal Fluid (ALF), representing extracellular and intracellular lung environments, respectively. Particles were exposed to each fluid at 37°C for up to 180 days (SLF) and 900 days (ALF). In SLF, UO2 showed low ap

Trichostatin A mitigates acute and late effects of radiation in intestine by regulation of DNA damage repair and Wnt/TGFβ/Smad signaling.

PURPOSE: Radiation accidents and misuse of nuclear weapons elevate the risk of development of acute life-threatening injuries as well as their late effects are noted in survivors. Currently, no countermeasure agents are available for the management of radiation-induced GI injury (RIGI) in humans. In the present study, the radiomitigative potential of Trichostatin A (TSA) was evaluated against acute and late RIGI. METHODS: 15 Gy gamma radiation was delivered to the whole abdomen of C57BL/6 mice, followed by intravenous TSA (150 ng/kg) administration after 1 h and 24 h. Acute changes were check…

Transcriptome profile changes in the jejunum of nonhuman primates exposed to supralethal dose of total- or partial-body radiation.

The risk of exposure of the general public or military personnel to high levels of ionizing radiation from nuclear weapons or radiological accidents is a dire national security matter. The development of advanced molecular biodosimetry methods, those that measure biological response, such as transcriptomics, to screen large populations of radiation-exposed victims is key to improving survival outcomes during radiological mass casualty scenarios. In this study, nonhuman primates were exposed to either 12.0 Gy cobalt-60 gamma (total-body irradiation, TBI) or X-ray (partial-body irradiation, PBI)…
