Nutritional and metabolic support in patients with amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is the most common form of progressive motor neuron disease and the most devastating neurodegenerative d … tional status. A multidisciplinary approach is crucial to set an appropriate plan for metabolic and nutritional support in ALS. Nutritional management incorporates a continuous assessment and implementation of dietary modifications throughout the duration of the disease. The nutritional and metabolic approaches to ALS should start when the diagnosis of ALS is made an …

[Pre- and postoperative nutritional support in upper disgestive tract surgery].

Patients undergoing upper digestive tract surgery are at risk for malnutrition and must be supported with careful nutritional management after a thorough assessment is performed. Parenteral nutrition is suitable for patients who cannot be fed enterally. However, total parenteral nutrition (TPN) has several metabolic disadvantages and affects the function of the gastrointestional tract. Both enteral nutrition and TPN can be combined for patients undergoing gastrointestinal surgical procedures. Immune-enhancing diets (IEDs) are useful for patients who have moderate to severe malnutrition due to

Adequacy of nutritional support in pediatric blood and marrow transplantation.

The use of nutrition support has become standard practice in blood and marrow transplantation, but what remains unclear is whether patients receive adequate nutrition supplementation during this time. This study is a retrospective audit of the nutritional supplementation of 34 pediatric patients admitted to a pediatric oncology unit for a blood and marrow transplantation. All patients received parenteral nutrition (PN) as their standard nutritional therapy with an average of 79 +/- 10.6% of their estimated requirements given over the transplant period. The glucose/amino acid infusion was inade

Issues of nutritional support for the patient with acute pancreatitis.

Management strategies in the nutritional support of the patient with acute pancreatitis have changed dramatically over the past 10 years. Prospective randomized trials show that maintaining gut integrity is equally as important as placing the pancreas at rest while inflammation within the gland resolves. In comparison to total parenteral nutrition and gut disuse, enteral feeding attenuates disease severity, reduces oxidative stress, and improves patient outcome. Nasojejunal feeds infused at or below the Ligament of Treitz should be provided to those patients with severe pancreatitis, as identi…

Enteral nutritional support by gastrostomy tube in children with cancer.

We examined the use of gastrostomy tubes in malnourished children with cancer as part of our ongoing efforts to improve their supportive care. Patients were examined on the basis of percentage of weight loss and percentage of desirable body weight. Twenty-five patients underwent gastrostomy tube placement followed by aggressive enteral nutritional support. Gastrostomy tubes were placed at a mean of 3.5 months (range, 0.3 to 8 months) after diagnosis; mean weight loss had been 10.1% (range, to 21%) of desirable body weight. There were no immediate postoperative complications. Gastrostomy tube f…

A case of hyperemesis gravidarum treated with artificial nutritional support.

A case of severe hyperemesis gravidarum is described in which the parturient required nutritional support. Initially a trial of enteral feeding was attempted but was unsuccessful. Subsequent parenteral nutrition allowed the remainder of the pregnancy to continue and a live infant was delivered by caesarean section at 34 weeks gestation. Criteria for using artificial feeding, the nutritional requirements of a pregnant woman and the potential hazards to the pregnancy by giving artificial nutritional support are reviewed.

[Parenteral and enteral nutritional support for patients with liver cirrhosis].

In liver cirrhosis, metabolic processes deteriorate, and serum amino acid imbalances may develop. Aromatic amino acids accumulate and branched-chain amino acids (BCAA) deficiencies develop, leading to hepatic encephalopathy and malnutrition. According to Fischer's reports, parenteral nutritional support with BCAA rich solution (Aminoleban (Otsuka), Morihepamin (Morishita)) is useful in patients with hepatic encephalopathy. Enteral nutritional support with BCAA rich solution (Aminoleban EN (Otsuka), Hepan-ED (Morishita)), together with low protein diet, may correct the specific metabolic abnorm…

Specialized nutritional support in the cancer patient: is it worthwhile?

Over a 7 year period, 85 cancer patients were managed by the nutrition service of St. Joseph's Hospital, Toronto. All these patients were nutritionally depleted, had obstruction to the gastrointestinal tract, or had postoperative complications such as enterocutaneous fistulas, evisceration or intraabdominal sepsis, which left total parenteral nutrition (TPN) as the only means of achieving positive nitrogen balance. A prospective study started in 1970 has found that when cancer was resectable and TPN was started preoperatively and continued postoperatively (24 patients-group 1) until the patien

Evidence supporting early nutritional support with parenteral amino acid infusion.

Postnatal growth of extremely low birth weight (ELBW) infants remains poor and does not come close to approximating rates of in utero growth. There is good evidence that early deficiencies in protein may be an important contributor to the poor growth outcomes observed in this population. Protein losses are inversely related to gestational age, and ELBW infants lose 1% to 2% of their total endogenous body protein stores each day that they receive glucose alone. It is now abundantly clear from a variety of studies that providing intravenous amino acids to sick premature infants in early postnata

Predictive factors for prolonged nutritional support after oesophagogastric cancer resection.

INTRODUCTION: There remains great variation in the use of perioperative feeding adjuncts following oesophagogastric cancer resections with unknown clinical benefit. The aim of this study was to examine which preoperative clinicopathological factors were associated with prolonged use of adjuvant nutritional support after oesophagogastric cancer surgery and to evaluate the associated costs. METHODS: A cohort study of 518 patients undergoing oesophagogastric resection and receiving perioperative parenteral nutrition was undertaken. Preoperative clinicopathological characteristics were evaluated …

Nutritional Support: The Use of Antioxidants in Inflammatory Bowel Disease.

The problem of treating inflammatory bowel disease continues to be a topic of great interest for researchers. Despite the complexity surrounding their treatment and strategies to prolong periods of remission, there is a promising exploration of various compounds that have potential in combating inflammation and alleviating symptoms. Selenium, calcium, magnesium, zinc, and iron are among these compounds, offering a glimpse of hope in the treatment of IBD. These essential minerals not only hold the promise of reducing inflammation in these diseases, but also show the potential to enhance immune

Malnutrition and Nutritional Support in Alcoholic Liver Disease: a Review.

Malnutrition is associated with alcoholic liver disease (ALD) and related complications such as hepatic encephalopathy and increased rate of infections. Avoidance of prolonged fasting and overly restrictive diets is important to avoid poor nutrition. Adequate intake of calories, protein, and micronutrients via frequent small meals and evening supplements and/or enteral and parenteral nutrition when indicated has been associated with reduced mortality and morbidity in patients with ALD. Modification of protein/fat sources and composition in addition to probiotic supplementation are promising in

Systematic Nutritional Support in Allogeneic Hematopoietic Stem Cell Transplant Recipients.

Allogeneic hematopoietic stem cell transplantation (HSCT) has become an established treatment modality for various hematological diseases. However, in allogeneic HSCT, patients often suffer from severe gastrointestinal complications caused by the conditioning regimen and acute/chronic graft-versus-host disease, which requires support by multidisciplinary nutritional support teams (NST). In addition, pretransplantation nutritional status can affect the clinical outcome after allogeneic HSCT. Therefore, it is important to refer the patient to a NST when becoming aware of nutritional problems bef…
