PURPOSE: We report the outcome of a staged approach to the intra-abdominal testicle and provide baseline data on operative time, postoperative course and testicular survival.
MATERIALS AND METHODS: We retrospectively reviewed the records of 441 boys (547 undescended testes). There were 105 boys (24%) with 124 nonpalpable testes (23%). All patients underwent laparoscopy. We report on a patient with persistent müllerian duct syndrome and normal external genitalia who had embryonal cancer of the testis 16 years after neonatal bilateral orchiopexy. In previous cases the testes of these patients have not been considered predisposed to form tumors. However, the occurrence of a testis tumor has been reported in 8 patients with this syndrome. The specific factors resulting in tumor formation in such patients are uncertain. Until they are clarified we suggest that these patients should be observed carefully for the possible development of testis tumor.

A 1-year-old boy presented for evaluation of bilateral undescended testes. Diagnostic laparoscopy was conducted, and uterine tissue with bilateral gonads was detected in the rectovesical fossa. Of 22 boys with an intra-abdominal testis 8 (12 testes) underwent the 2-stage Fowler-Stephens orchiopexy. During stage 1 the testicular artery and internal spermatic vein were ligated in situ 2 to 3 cm. superior to the intra-abdominal testis. The testicular vessels were transected inferior to the ligatures 6 months later and the testis was brought to the scrotum with the standard Fowler-Stephens orchiopexy technique.
