Organizing pneumonia pattern in the follow-up CT of Legionella-infected patients.

The main aim of this study was to describe the appearance of the CT pattern of organizing pneumonia in Legionella-infected patients. Serial CT scans obtained from five sporadic cases of Legionella pneumophila pneumonia were retrospectively reviewed. The mean time of follow-up was 14 days. Chest CT was analyzed with regard to frequency and appearance of CT patterns of pulmonary abnormalities. Consolidation and ground-glass opacities, with or without an air bronchogram, were the most common abnormalities detected in CT scans during follow-up patients with L. pneumophila pneumonia. Two patterns w…

[Pneumocystis jiroveci infection associated with organizing pneumonia in a kidney transplant patient].

The authors report the association of organizing pneumonia (OP) and a Pneumocystis jiroveci infection in a woman who benefited from a kidney transplant 13 years before and was under corticoids, cyclosporine and mycophenolate mofetil. The diagnosis was based on progressive dyspnoea with fever with an alteration in the general state associated with diffuse micronodular pneumopathy suggesting bronchiolitis. The conformation was obtained by the analysis of the alveolar bronchial washings and the histological examination of the distal biopsies revealing endo-alveolar vegetant fibromas. Transbronchi…

Fatal acute fibrinous and organizing pneumonia in a child with juvenile dermatomyositis.

Acute fibrinous and organizing pneumonia, a recently described form of diffuse acute lung injury, sometimes affects adults with inflammatory myopathy. We describe a child with juvenile dermatomyositis who had development of acute fibrinous and organizing pneumonia. There does not appear to be a successful method of treatment, particularly in severe cases with respiratory failure.

[Primary pulmonary cryptococcosis exhibiting the radiological characteristics of bronchiolitis obliterans organizing pneumonia].

A 31-year-old male was admitted to Toranomon Hospital because of a cough and bilateral patchy infiltrates shown on the chest radiograph. He had been well prior to admission. Chest CT scan revealed patchy areas of air-space consolidation with air-bronchogram and adjacent ground-glass opacities, suggestive of bronchiolitis obliterans organizing pneumonia (BOOP). Transbronchial lung biopsy specimen confirmed the formation of epithelioid cell granulomas without necrosis and the coexistence of organizing pneumonia. The titer of serum cryptococcal antigen increased to 1:256. According to these findi…

TB or not TB: cavitary bronchiolitis obliterans organizing pneumonia mimicking pulmonary tuberculosis.

Two patients with subacute symptoms and signs compatible with pulmonary tuberculosis (TB) had right upper lobe cavitary infiltrates shown on chest radiography. In both patients, purified protein derivative and microbiologic testing excluded TB, and tissue examination yielded typical histologic changes of bronchiolitis obliterans organizing pneumonia (BOOP). Glucocorticoid therapy led to clinical and radiologic resolution. Though probably rare in this situation, BOOP should be considered in the differential diagnosis of patients presenting with clinical and radiologic features of pulmonary TB.

Organizing pneumonia related to common variable immunodeficiency. case report and literature review.

A 68-year-old woman suffering from common variable immunodeficiency (CVI) developed a typical picture of organizing pneumonia. Causative factors other than CVI were eliminated. Several antibiotic regimens failed to improve the patient's condition, while the clinical manifestations rapidly disappeared under steroid therapy, with complete radiological recovery, but relapsed after steroid withdrawal. Finally, organizing pneumonia was definitely demonstrated by pathological findings obtained by open lung biopsy. Interestingly, pathological examination exhibited two other well-known CVI-associated …

Bronchiolitis obliterans with organizing pneumonia (BOOP) heralding anti-Jo-1-positive polymyositis.

We report a 51-year-old man who presented with 3 weeks of polyarthritis with fever, nonproductive cough, bibasilar crackles, tachypnea, and hypoxia. Initial laboratory data showed an increased erythrocyte sedimentation rate, rheumatoid factor, and anti-Jo-1 antibody. Imaging studies showed bilateral lower lobe infiltrates of the lung. A transbronchial biopsy specimen revealed characteristic findings for bronchiolitis obliterans organizing pneumonia (BOOP). About 6 months later, he developed profound proximal muscle weakness with a dramatic increase in creatine phosphokinase and aldolase and a …

Cryptogenic organizing pneumonia after withdrawal of systemic corticosteroids for chronic eosinophilic pneumonia and severe asthma under benralizumab treatment.

A 79-year-old woman with severe asthma developed chronic eosinophilic pneumonia (CEP). After CEP resolved with oral prednisolone at 30 mg/day, prednisolone was tapered and discontinued under introduction of benralizumab for her severe asthma. However, 8 weeks later, symptoms and bilateral patchy infiltrates on chest radiography appeared. Lymphocytosis without eosinophilia was seen in bronchoalveolar lavage fluids, and transbronchial biopsy indicated organizing pneumonia. Cryptogenic organizing pneumonia (COP) was diagnosed and resolved with prednisolone at 30 mg/day. Prednisolone was tapered t…

[Organized pneumonia].

Organizing pneumonia is a condition first described in 1941. Its symptoms are nonspecific (fever, arthromyalgia, etc.), and its diagnosis relies primarily on histology, when this is not feasible, it is based on a combination of clinical, laboratory, and radiological findings, in the absence of a differential diagnosis particularly active infections. It is a rare condition that may be secondary to another condition or primary and referred to as cryptogenic. Numerous causes of organized pneumonia have been described: post-infectious, associated with connective tissue diseases, drug-induced, etc.…

Lymphangioleiomyomatosis with superimposed organizing pneumonia diagnosed by transbronchial lung cryobiopsy: A case report.

Lymphangioleiomyomatosis (LAM) is a cystic lung disease characterized by rounded, thin-walled cysts uniformly distributed within normal lung parenchyma on high-resolution chest computed tomography. We report a case of diffuse cystic lung disease with multiple alveolar opacities. Transbronchial lung biopsy showed organizing pneumonia, but LAM cells were not observed. She was diagnosed with cryptogenic organizing pneumonia (COP) and treated with corticosteroid therapy. LAM was definitively diagnosed by transbronchial lung cryobiopsy about two years after completing corticosteroid therapy for COP…

Acute fibrinous and organizing pneumonia after bone marrow transplantation, an underrecognized, severe condition.

INTRODUCTION: Allogeneic bone marrow transplantation can lead to various pulmonary complications, including acute fibrinous organizing pneumonia. This condition is rare and presents with aggressive clinical features, distinct from other forms of organizing pneumonia, such as cryptogenic organizing pneumonia. METHOD: A literature review using the PubMed, Embase, Lilacs, and Cochrane databases was conducted to analyze cases of acute fibrinous organizing pneumonia following transplantation focusing on clinical features, therapeutic approaches, and outcomes. CASE REPORT: The case of a 62-year-ol…

Cryptogenic organizing pneumonia complicated by pulmonary embolism following glucocorticoid therapy: a case report.

BACKGROUND: Cryptogenic organizing pneumonia (COP), a rare interstitial lung disease, can mimic community-acquired pneumonia (CAP), often leading to delayed diagnosis. This case highlights the importance of recognizing COP in elderly patients and brings attention to pulmonary embolism (PE) as a potential but underrecognized complication associated with glucocorticosteroid therapy, providing novel insights into hypercoagulability risks during treatment. CASE PRESENTATION: An 80-year-old woman from Xinjiang presented with a 4-week history of cough, dyspnea, and weight loss that was unresponsive…

A case of drug-induced organizing pneumonia caused by amikacin liposome inhalation suspension.

Inhaled liposomal antimicrobials are known to cause hypersensitivity pneumonitis. Amikacin liposome inhalation suspension (ALIS) is a promising novel antimicrobial agent against refractory Mycobacterium avium complex infections. The frequency of drug-induced lung injury caused by ALIS is relatively high. To date, no reports of ALIS-induced organizing pneumonia diagnosed by bronchoscopy are available. We report a case of a 74-year-old female patient presenting with non-tuberculous mycobacterial pulmonary disease (NTM-PD). She was treated with ALIS for refractory NTM-PD. Fifty-nine days after st…

Synovitis, acne, pustulosis, hyperostosis, osteitis syndrome with invisible organizing pneumonia: A case report.

We report the case of a 59-year-old female patient, presenting with pustular rash on both hands and pain in the lumbosacral part and left lower limb. A magnetic resonance imaging examination of the left leg was undertaken and the result showed that a malignant lesion with bone destruction of the left femoral shaft could not be excluded. Subsequently, bone tumor was excluded by pathological examination. Lung computed tomography scan showed patchy consolidation and cord shadow in the middle left lung. Subsequently, lung cancer was excluded by pathological examination, and the histopathological c

Paraneoplastic acute fibrinous and organizing pneumonia from lymphoma completely responding to bendamustine-rituximab.

Acute fibrinous and organizing pneumonia (AFOP) is a rare histopathological pattern of lung injury characterized by prominent fibrin deposition in alveolar spaces. It may be idiopathic or associated with medications, connective tissue disease, infection, environmental exposures, transplantation, and malignancy. There is no proven treatment but multiple reports describe response to corticosteroids. We report the case of a 65-year-old male never-smoker with a 15-month history of dry cough, dyspnoea, anorexia, and night sweats only partially responsive to doxycycline and oral prednisolone. Comput…

Concomitant Radiation Recall Dermatitis and Organizing Pneumonia following Breast Radiotherapy: A Case Report.

PURPOSE: Radiation recall dermatitis (RRD) is a rare complication that occurs after completion of radiation therapy (RT) and initiation of a precipitating agent, most commonly chemotherapeutic medications. Various theories attempt to explain the mechanism, including activation of the body's inflammatory pathways through nonimmune activation. Likewise, radiation-induced organizing pneumonia (RIOP) is an infrequent but potentially life-threatening complication of RT that, while not fully understood, is suspected to be partly an autoimmune reaction. PATIENT: We present the case of a 71-year-old …

Belimumab in refractory organizing pneumonia associated with systemic lupus erythematosus: a case report.

Organizing pneumonia is an inflammatory lung entity that presents with a huge variety of clinical, radiological and pathological patterns. Organizing pneumonia can be idiopathic or secondary to other diseases. Corticosteroid therapy is usually the first-line treatment showing clinical improvement in most cases. This report presents the case of a 56-year-old woman with systemic lupus erythematosus who was diagnosed with an organizing pneumonia and showed a poor response to steroid and azathioprine treatment. We considered the use of belimumab, which resulted in excellent clinical and radiologic…
