Three-dimensional analysis of osteophyte formation on distal radius following scaphoid nonunion.

BACKGROUND: The purposes of this study were to quantitatively analyze osteophyte formation of the distal radius following scaphoid nonunion and to investigate how fracture locations relate to osteophyte formation patterns. METHODS: Three-dimensional surface models of the scaphoid and distal radius were constructed from computed tomographic images of both the wrists of 17 patients' with scaphoid nonunion. The scaphoid nonunions were classified into 3 types according to the location of the fracture line: distal extra-articular (n = 6); distal intra-articular (n = 5); and proximal (n = 6). The o…

Localization of bone morphogenetic protein-2 in human osteoarthritic cartilage and osteophyte.

OBJECTIVES: To examine the localization of bone morphogenetic protein (BMP)-2 mRNA and protein in human osteoarthritic (OA) articular cartilage and osteophyte. DESIGN: Five normal, four growing and 14 OA human cartilage samples, graded histomorphologically by Mankin Score, were studied by in situ hybridization and immunohistochemistry for the expression of BMP-2. RESULTS: BMP-2 mRNA was present in chondrocytes in neonatal growing articular cartilage, but was scarcely present in normal adult articular cartilage. In OA articular cartilage, BMP-2 mRNA and protein were detected in both clusterin…

Involvement of nitric oxide in chondrocyte cell death in chondro-osteophyte formation.

OBJECTIVE: To examine the nitric oxide (NO) production relevant to chondrocyte cell death in order to elucidate the mechanism of chondro-osteophyte formation in osteoarthrotic joints. DESIGN: Human chondro-osteophytes were obtained during total hip arthroplasty. Expression of inducible nitric oxide synthase (iNOS) mRNA was determined by in-situ hybridization. Localization of iNOS and nitrotyrosine at protein level were examined by immunohistochemistry. Cell death of chondrocytes were confirmed by both TUNEL method and transmission electron microscopy. RESULTS: The various populations of prol…

Posterior decompression of the vertebral artery narrowed by cervical osteophyte: case report.

BACKGROUND: Symptomatic vertebral artery compression caused by cervical spondylosis usually is caused by compression of the artery by osteophytes arising from the uncinate process. Compression from facet joint osteophytes is seldom reported. CASE DESCRIPTION: A 69-year-old male developed symptomatic vertebral artery stenosis secondary to an osteophyte arising from the superior facet of the sixth cervical vertebra posterior to the artery. A posterior decompression of the vertebral artery with removal of the offending facet joint complex relieved the patient's transient neurologic events. CONC…

Cervical osteophytes resulting in dysphagia: A case report with literature review.

Dysphagia may result from either organic or structural conditions, whether malignant or benign, or functional disorders, primarily involving motility or sensory perception. However, its occurrence due to cervical osteophytes is a rare phenomenon. This report presents a case of anterior cervical osteophytes inducing dysphagia. An 81-year-old man presented with complaints of weakness, fever, dysphagia, loss of appetite, and weight loss. Esophagogastroduodenoscopy showed a bulge in the posterior pharyngeal wall. A computed tomography scan revealed anterior cervical osteophytes extending from C2 t…

The effect of retroaortic left renal vein on lumbar osteophytes formation.

PURPOSE: Assess whether a Retroaortic left renal vein (RLRV) affects vertebral osteophyte formation in the lumbar spine, compared to normal anatomy left renal vein. METHODS: We conducted a retrospective case-control study. Computed tomography (CT) scans of individuals with a RLRV (study group) were compared to age- and gender-matched normal anatomy CT scans (control group). L1 to L4 vertebral levels were appreciated for: left renal vein level, osteophyte presence and the aorta-vertebral distance (AVD) at the left renal vein level. Univariate analyses were conducted using Chi-square test and F…

Lethal mesenteric perforation by osteophytes after blunt abdominal trauma.

A 50 year-old homeless man was found dead the day after he had sustained blunt abdominal trauma during a physical assault. Autopsy revealed no apparent injury to the abdominal wall, but showed a massive hemoperitoneum resulting from a large (8 cm) tear of the mesenteric root. It also revealed prominent and diffuse spinal osteophytes predominating in the lumbar region, where they were fused and formed a large anterior ossified excrescence. The diagnosis of diffuse idiopathic skeletal hyperostosis was established in the presence of continuous ossification along the anterior aspect of five contig…

Osteophytes in the Cervical Vertebral Bodies (C3-C7)-Demographical Perspectives.

Vertebral osteophytes are an age-dependent manifestation of degenerative changes in the spine. We aimed to determine the prevalence and severity of cervical osteophytosis in a large study population. To do so, we developed a grading system for osteophytosis, enabling the assessment of their presence and severity in the cervical spine, and applied it to the analysis of dried cervical vertebral bodies (C3-C7) from 273 individuals. Statistical analyses were carried out per motion segment, while testing for the effect of age, sex, and ethnicity. The highest prevalence of osteophytes was found in m…

Fate of osteophytes and sclerosis in fused segments after lumbar fusion.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To investigate the fate of sclerosis and anterior osteophytes in the fused segments after instrumented lumbar fusion for degenerative lumbar disorders. SUMMARY OF BACKGROUND DATA: Sclerosis and osteophytosis are well-known radiographical findings, but little is known of their significance with regard to spontaneous resorption after spine fusion. METHODS: Thirty patients (9 males, 21 females; 60 vertebra; mean age of 66.9 yr [45-86 yr]) were divided into a posterolateral fusion group (n = 14, 28 vertebrae) and a posterior lumbar interbody …

The cam-type deformity--what is it: SCFE, osteophyte, or a new disease?

Cam-type deformity of the proximal femur is a risk factor for the development of cam-type femoroacetabular impingement and a prearthrotic co … entity from residual or silent slipped capital femoral epiphysis, as well as osteoarthritis-induced osteophytes. In this paper we examine the different pathogenetic aspects of the proximal femur that contribute to cam-type deformity and/or symptomatic cam-type femoroacetabular impingement.

Dysphagia due to anterior cervical osteophytes complicated with hypopharynx abscess.

This report concerns a case of an 80-year-old man with progressive dysphagia and stridor. Patient's clinical and radiological evaluation revealed a mass of the hypopharynx and a large cervical osteophyte. Microlaryngoscopy under general anaesthesia demonstrated the presence of a hypopharynx abscess that was drained. A tracheotomy was performed in order to secure the airway, and 6 weeks later, the patient was decannulated and returned to full consistency diet.

Hypertrophic anterior cervical osteophytes causing dysphagia and airway obstruction.

Hyperostosis of anterior cervical vertebral osteophytes can produce otolaryngological symptoms ranging from mild dysphagia, dysphonia, and foreign body sensation to severe food impaction and stridulous dyspnea. Airway compromise necessitating a tracheostomy is very rare. We discuss the case of an elderly man who presented with progressive dysphagia and a large hypopharyngeal mass as his initial manifestations of hypertrophic anterior cervical osteophytes. After a biopsy of the mass, the patient went into airway distress due to bilateral vocal fold fixation by the enlarging mass and consequentl…

The ultrastructure of the peri-articular osteophytes - an evaluation by scanning electron microscopy.

OBJECTIVE: Osteophytes are intra-articular osteochondral tissues, which are usually found at the margins of degenerating synovial joints. The aetiology or pathogenesis of this tissue has been a subject of protracted debate. The aim of this study was to offer a possible aetiology and or pathogenesis of this expendable yet important osteochondral tissue using the scanning electron microscopy to evaluate the structure of the cartilage mantle of osteophytes and the relationship of this mantle with that of the adjoining normal articular cartilage. METHODS: Sections of periarticular osteophytes and…

Osteophyte at the sacroiliac joint as a cause of sciatica: a report of four cases.

Four cases of sciatica due to osteophytes impinging on the sciatic nerve at the sacroiliac joint are reported. Of these 4 cases, 2 were treated conservatively and the other 2 required surgical excision of the osteophyte. The report highlights the importance of keeping this uncommon etiology in the differential diagnosis of sciatica.
