Is this really trigeminal neuralgia? Diagnostic re-evaluation of patients referred for neurosurgery.

OBJECTIVES: Patients with facial pain are sometimes referred for neurosurgical treatment with a poorly documented diagnosis of trigeminal ne … nding that was clearly overestimated by the referring neurologists in seven patients. CONCLUSIONS: Overdiagnosis of trigeminal neuralgia was common in patients referred for neurosurgery, mostly due to insufficient clinical evaluation combined with an overestimation of MRI-detected NVCs. The clinical diagnosis should be critically re-evaluated before recommendi …

Presumed Urinary Tract Infection in Patients Admitted with COVID-19: Are We Treating Too Much?

Despite the low rates of bacterial co-/superinfections in COVID-19 patients, antimicrobial drug use has been liberal since the start of the COVID-19 pandemic. Due to the low specificity of markers of bacterial co-/superinfection in the COVID-19 setting, overdiagnosis and antimicrobial overprescription have become widespread. A quantitative and qualitative evaluation of urinary tract infection (UTI) diagnoses and antimicrobial drug prescriptions for UTI diagnoses was performed in patients admitted to the COVID-19 ward of a university hospital between 17 March and 2 November 2020. A team of infe…

Comparing the effectiveness of prostate cancer screening protocols: European Association of Urology- and European Randomized Study of Screening for Prostate Cancer-based strategies.

Prostate cancer (PC) screening reduces PC mortality but also causes burden such as overdiagnosis and unnecessary biopsies. The European Association of Urology (EAU) recently proposed a risk-adapted screening protocol incorporating prostate-specific antigen (PSA)-based intervals, a risk calculator (RC), and magnetic resonance imaging (MRI), though its long-term outcomes remain unquantified. We constructed the MIcrosimulation SCreening Analysis-PSA model by adapting the existing MIcrosimulation SCreening Analysis-PROstate microsimulation framework to simulate individual PSA trajectories. The mod…

Modeling the natural history of ductal carcinoma in situ based on population data.

BACKGROUND: The incidence of ductal carcinoma in situ (DCIS) has increased substantially since the introduction of mammography screening. Ne … lts data. Models estimated mean sojourn time (MST) in the preclinical screen-detectable DCIS state, overdiagnosis, and the risk of progression from preclinical screen-detectable DCIS. RESULTS: Without biopsy and surgical excision, the majority of DCIS (64-100%) in the preclinical screen-detectable state progressed to IBC in submodels assuming no DCIS regressio …

Current Challenges in Imaging-Based Cancer Screening, From the AJR Special Series on Screening.

The early detection of cancer confers many significant benefits for patients, primarily by enabling less invasive and more effective treatme … harms need to be assessed. These harms, although specific to the modality and cancer, often include overdiagnosis, overtreatment, and false-positive examinations. As radiology technology improves and new tools become available, the risk-to-harm ratios of imaging-based screening will shift, and screening recommendations will need to adapt accordingly. Radiologis …

Pseudolipomatosis in Endometrial Specimens Does Not Represent Uterine Perforation.

BACKGROUND: Specimens of endometrial biopsies can sometimes present with an artifact within blood, composed of optically clear vacuoles mimicking adipose tissue, pseudolipomatosis. This artifact can be mistaken for adipose tissue and lead to an overdiagnosis of uterine perforation. CASE: We describe the case of pseudolipomatosis seen within the evacuated products of conception from a missed abortion. RESULTS: Areas of vacuolization in the blood clot mimicked adipose tissue. However, the vacuoles varied in size and did not contain adipocytes. CONCLUSION: Familiarity with this artifact will l…

Diagnostic inflation: causes and a suggested cure.

There have been a striking diagnostic inflation and a corresponding increase in the use of psychotropic drugs during the past 30 years. DSM-5, scheduled to appear in May 2013, proposes another grand expansion of mental illness. In this article, we will review the causes of diagnostic exuberance and associated medical treatment. We will then suggest a method of stepped care combined with stepped diagnosis, which may reduce overdiagnosis without risking undertreatment of those who really need help. The goal is to control diagnostic inflation, to reduce the harms and costs of unnecessary treatmen…

Red flags in global autism data: a forensic analysis of prevalence patterns and official aid dependencies.

INTRODUCTION: The literature extensively examines the global incidence rate of autism, emphasizing the need to scrutinize reported figures for potential anomalies, particularly addressing overdiagnosis concerns. METHODS: Our forensic analysis employing Benford's Law and Mean Absolute Deviation indicates significant statistical irregularities and potential overdiagnosis, especially post-DSM-5 implementation, suggesting diagnostic criteria changes drive upward trends. The segmented analysis reveals this relationship intensified in low-income countries post-DSM-5 while remaining non-significant …

Incidence and mortality trends after introduction of lung cancer screening in a community-based healthcare system.

BACKGROUND: Lung cancer screening (LCS) enables early cancer detection but raises concerns about overdiagnosis, particularly in community-based LCS settings where evidence remains limited. This study examined trends in lung cancer incidence and mortality following the introduction of LCS in a community-based setting and used these trends to assess the potential of LCS-associated overdiagnosis. METHODS: This retrospective study leveraged data and outcomes from the Kaiser Permanente Colorado (KPCO) LCS program between 2014 and 2023. Trends in lung cancer incidence and mortality were assessed by…

The expert eye - Navigating ambiguity in clinical breast radiology.

Drawing on phenomenological concepts of embodiment and the theory of clinical judgment, we examine how radiologists navigate the inherent risk of overtesting and overdiagnosis in breast cancer screening. Based on 12 weeks of fieldwork at a breast radiology department, we explore how radiologists navigate clinical ambiguity when women are recalled for further diagnostics following an abnormal screening mammogram. Radiologists expressed a strong sense of ethical obligation and responsibility, shaped by the structural and moral expectations of the screening programme, which fostered a precautiona…

[Tongue tie: to cut or not to cut?].

Cleavage of tongue tie (ankyloglossia) in newborns is controversial and has known varying degrees of popularity over time. The last decade frenotomy is advocated and increasingly performed in infants with breastfeeding problems. However, evidence for an association between ankyloglossia and breastfeeding is dubious. Moreover, evidence for the effect of frenotomy on breastfeeding is lacking. Well-conducted controlled trials are sparse and methodological quality of reported trials is weak and merely hindered by lack of equipoise. Thus, the tongue tie and frenotomy are prone for overdiagnosis and…

The effect of screening on melanoma incidence and biopsy rates.

BACKGROUND: Cutaneous melanomas are common cancers in white-skinned populations, and early detection is promoted as a means of reducing morbidity and mortality. There is concern that increased skin screening is leading to overdiagnosis of indolent melanomas with low risk of lethality. The extent of melanoma overdiagnosis associated with screening is unknown. OBJECTIVES: To estimate possible overdiagnosis by comparing subsequent melanoma incidence and biopsy rates among people subjected to skin screening those who were not. METHODS: We recruited 43 762 residents of Queensland, Australia, aged…

[Cancer screening: blessing or curse?].

Cancer screening seems intuitively logical: early detection to prevent worse outcomes. However, this article reveals the complexity behind this assumption. Cancer's linear growth model- on which early detection was based - proves outdated. Cancers grow at vastly different rates; some even stop or disappear spontaneously. The main problem is overdiagnosis: in Dutch breast cancer screening, approximately 2,000 women are treated annually for cancers that would never cause symptoms. Epidemiological data show a persistent increase in stage I cancers without a decline in stage IV cases - a sign of o…

[Prostate cancer screening : to be recommended in 2024?].

The Prostate Specific Antigen (PSA) blood test is still the only screening tool for prostate cancer. It is recommended between the ages of 50 and 69 as part of a shared decision making process between a patient and his or her doctor using a decision aid, as the test carries a significant risk of overdiagnosis. If a patient wishes to be screened, either because he is at higher risk, or because he places greater importance on a modest reduction in cancer-related mortality, the frequency of screening depends on his age, family history, and whether he is part of a high-risk group. The use of multi…

Thirty percent of ductal carcinoma in situ of the breast in Japan is extremely low-grade ER(+)/HER2(-) type without comedo necrosis.

Background Overdiagnosis in mammography (MMG) is a problem. Combination of MMG and ultrasonography for breast cancer screening may increase overdiagnosis. Most cases of overdiagnosis are low-grade ductal carcinoma in situ (LGD), but no reports have focused on them. Materials and methods We immunostained 169 ductal carcinoma in situ (DCIS) cases for ER, PgR, HER2 and Ki67 and classified them into 4 subtypes: ER(+)/HER2(-), ER(+)/HER2(+), ER(-)/HER2(-) and ER(-)/HER2(+). The Ki67 index was used to evaluate the grade of malignancy and examined for correlations with each ER/HER2 subtype and the nu…

Update on Pediatric Overuse.

As concerns over health care-related harms and costs continue to mount, efforts to identify and combat medical overuse are needed. Although much of the recent attention has focused on health care for adults, children are also harmed by overuse. Using a structured PubMed search and manual tables of contents review, we identified important articles on pediatric overuse published in 2015. These articles were evaluated according to the quality of the methods, the magnitude of clinical effect, and the number of patients potentially affected and were categorized into overdiagnosis, overtreatment, an…
