Craniofacial rehabilitation demands a multidisciplinary approach where prosthodontists play a pivotal yet often underutilized role in the continuum of care. Traditionally, prosthetic rehabilitation is considered a postsurgical step, leading to delays in functional restoration and compromised outcomes. This editorial emphasizes the need to redefine prosthodontics as an integral component of craniofacial surgical planning, from the operating room to the dental chair. By fostering collaboration between surgeons, prosthodontists, and digital design teams at the preoperative stage, patient-specific

OBJECTIVE: The objective of this investigation was to assess and compare the fracture resistance of roots filled with AH Plus, Total Fill and AH Plus bioceramic sealers using single cone, warm vertical compaction (WVC), and soft-core techniques.
METHODS: This study utilized the palatal root canals of eighty extracted human maxillary first molars. All roots were sectioned to maintain a uniform root length of 11 mm. The samples were mechanically prepared using EdgeFile X7 rotary files to size 40/0.04. Eight teeth were left unfilled as a control group, while the remaining teeth were classified i

Reconstruction of the hard palate defects is among the most challenging problems for plastic surgeons. Prosthetic obturations and local flaps for small defects have been used, whereas numerous regional and free flaps have been described for larger defects. The search for the ideal method offering a natural palatal structure is still ongoing. Five male patients with a mean age of 30.4 years experiencing hard palate defects due to congenital cleft palate or tumor excisions were repaired by prelaminated calvarial osteofascial flap. The mean defect size was 3.14 × 2.48 cm. Both of the surfaces of

INTRODUCTION: The case of a newborn female patient with unilateral cleft lip, alveolus and palate is presented with a markedly sunken nasal wing at the cleft side. This finding may lead to a very unsatisfactory aesthetic result after primary cheiloplasty and nasoplasty.
PATIENT AND METHOD: Five months prior to surgery, the patient was treated with a palatal plate and additionally with a nasoalveolar conformer for 4 months thereof. The device consisted of an acrylic pelotte (attached to the maxillary plate with a wire) which lifted the nasal dome and was activated regularly. Under this presurg

In patients with bilateral cleft lip and palate (BCLP), orthodontic and facial orthopedic treatments alone occasionally fail to resolve problems because of the absence of alveolar and palatal hard and soft tissue or contraction resulting from previous surgical repair. Prosthetic reconstruction of the anterior maxilla is important for these patients. Patients with BCLP have premaxillary displacement in up to 3 dimensions. It may be difficult to make impressions without deformation. This clinical report describes a method for positioning the mobile premaxilla during impression making for a patie

In 48 patients a systematized therapeutic model plan for the preoperative and postoperative orthodontic treatment of patients with unilateral or bilateral cleft lips and palates is pursued. In addition to stimulation and growth control of the maxillary alveolar segments in a transverse direction and at the segment poles, increasing sagittal development is observed in a later developmental phase.

This article presents a technique for fabricating a modified nasoalveolar molding appliance using 0.032-inch titanium-molybdenum alloy wire for nasal stents incorporating a double loop. The nasal stents are included in the acrylic molding plate at the time the appliance is inserted. The acrylic retention button used in a conventional appliance is replaced by a simple retention hook fabricated using titanium-molybdenum alloy wire. This technique is an effective alternative to a conventional appliance, and it simplifies the appliance-modification process during follow-up visits.

The development of articulation before surgical closure of the hard palate was compared in 75 preschool children with cleft lip and palate and 40 preschool children born without clefts. The children were aged 2 years to 5 years 11 months. The patients had significantly poorer articulation skills than the controls at each age level. Substitutions were the most frequent error, and they did not decrease with age in the patients. Fistula size and a history of speech therapy were significant factors in the articulation error scores only in 5-year-olds. No advantage in articulation proficiency was f

Self-inflicted gunshot wounds (GSW) to the palate result in complex bony and soft tissue trauma to the mid and upper face. Patients who survive these injuries are faced with significant speech and feeding difficulties. Upper and midface fractures open reduction and internal fixation (ORIF) is required for many of these patients, and consideration to incision planning is critical in order to preserve a primary option for oroantral fistula repair. The temporoparietal fascia (TPF) flap is an excellent option for primary palate repair as it is often exposed in the operative field during facial fra

Maxillary necrosis can occur due to bacterial infections such as osteomyelitis, viral infections, such as herpes zoster or fungal infections, such as mucormycosis, aspergillosis etc. Mucormycosis is an opportunistic fungal infection, which mainly infects immunocompromised patients. Once the maxilla is involved, surgical resection and debridement of the necrosed areas can result in extensive maxillary defects. The clinician is to face many a challenge in order to replace not only the missing teeth, but also the lost soft tissues and bone, including hard palate and alveolar ridges. The prosthesi

Patients with extensive head and neck injuries due to trauma and/or extensive surgical procedures often exhibit a severely limited ability to open the mouth. For the dentist involved in prosthodontic treatment of such patients, restricted mouth opening commonly leads to compromised impressions and prostheses especially in cases where both hard and soft palatal parts have been resected. In prosthodontic treatment, the loaded impression tray is the largest item requiring intraoral placement. During impression procedures, wide mouth opening is required for proper tray insertion and alignment. Thi

Oral rehabilitation of adult patients with cleft lip and palate is related to the severity of the anatomical and functional alterations that hamper the proper closure of the nasopharynx. The ideal treatment is closure by bone graft and orthodontics. However, when surgery is not possible or when the patient does not wish to undergo surgery, a palatal prosthesis may offer the best solution in most clinical situations. The authors of this article propose a new classification to help the practitioner with decision making and prosthetic treatment planning for residual palatoalveolar cleft defects.

Technical difficulties in the construction of hard palate obturators following oncologic surgery are due to the recording limitations of the entire defect area, plus prosthesis base instability during recording of maxillomandibular relationships. This article describes a time-saving technique that ensures stable and precise recording bases. A light-polymerizing acrylic resin layer is used for making the first impression of the defect, while simultaneously obtaining an acrylic resin impression tray and base for recording maxillomandibular relationships. Adhesive paper copies are used for the ar

Very early, the new-born no more listen the phonemes which are not used around him. In the same way, he does not listen the phonemes impossible to pronounce by himself, because he is not concerned. He does not engrave then. Consequently, we must give him, quickly, complete anatomical means of pronunciation, and train him by early intensive imitation-interchange mechanism. For those two reasons, we propose to complete immediately the early surgical reparation of the soft palate by a thin obturating plate and by an early orthophonic parental guidance.

A patient had an adenocystic carcinoma that involved both maxillary sinuses, the entire hard palate, and the lower third of both nasal chambers. Treatment consisted of bilateral maxillectomy and a split-thickness skin graft applied to the inside of the cheeks. A dental obturator constructed to fit undercuts in the surgical cavity allowed speech and swallowing. Because the patient objected to the sunken appearance of the upper lip, she obtained the central incisor portion of her late husband's upper denture and bonded this segment to the dental appliance. This achieved satisfactory cosmesis.

OBJECTIVES/HYPOTHESIS: The management of palatal defects resulting from the extirpation of benign and malignant lesions uses a variety of methods, with the optimal techniques allowing maximal postoperative function with minimal morbidity. The palatal island flap is an effective, reliable technique for reconstructing postablative oral cavity defects.
METHODS: All patients who underwent palatal resections for benign or malignant lesions at a tertiary care, referral-based head and neck cancer center since 1995 were eligible. Ten patients were identified whose surgical defects were reconstructed

BACKGROUND: Restoration of speech after surgical resection for oropharyngeal cancer traditionally includes maxillofacial prosthetic intervention. Relatively few publications with objective speech outcomes exist. The purpose of this study was to evaluate speech outcome relative to the size of the surgical defect, the type of speech prosthesis, and the height and position of the speech bulb in relation to the posterior pharyngeal wall in the nasopharynx.
METHODS: Fifty-five patients treated at the Memorial Sloan-Kettering Cancer Center Dental Service who underwent ablative cancer therapy were e
