INTRODUCTION: Anatomicaly preformed variable-angle locking plates are technologically mature and appear to be optimal for the fixation of distal radius fractures. However, there is still much argument about whether volar plating is equivalent to dorsal (buttressing) plating, especially in the management of intra-articular extension fractures. This biomechanical study was performed to determine, in a simple intra-articular fracture model, whether dorsal or volar plate constructs would be more stable.
MATERIALS AND METHODS: Six pairs of fresh frozen radii were examined with dual-energy absorpti

OBJECTIVE: To report the MR imaging findings of painful injured metacarpophalangeal (MCP) joints of the fingers.
DESIGN AND PATIENTS: MR imaging of 39 injured MCP joints in 38 patients was performed after a mean delay of 8.8 months. The MR images were obtained with the fingers in extended and flexed positions using T2-weighted and T1-weighted sequences before and after intravenous injection of a gadolinium compound. Ten patients were treated surgically. Mean clinical follow-up was 1.8 years.
RESULTS: Tears of the collateral ligaments were the most common lesion (30/39), most being radial in

RATIONALE: Tendon adhesion is a relatively uncommon complication after distal radius fracture surgery.
PATIENT CONCERNS: A 51-year-old woman underwent open reduction and volar plate fixation for a distal radius fracture at another hospital approximately 4 weeks ago. Upon presentation to our hospital, active flexion of the thumb interphalangeal (IP) joint was preserved, but active extension was absent; additionally, passive extension of the thumb IP joint could not be achieved by the examiner.
DIAGNOSES: Plain radiography and computed tomography demonstrated that 1 distal screw of the volar p

We present an economic evaluation using data from the Distal Radius Acute Fracture Fixation Trial (DRAFFT) to compare the relative cost effectiveness of percutaneous Kirschner wire (K-wire) fixation and volar locking-plate fixation for patients with dorsally-displaced fractures of the distal radius. The cost effectiveness analysis (cost per quality-adjusted life year; QALY) was derived from a multi-centre, two-arm, parallel group, assessor-blind, randomised controlled trial which took place in 18 trauma centres in the United Kingdom. Data from 460 patients were available for analysis, which in

Current diagnostic criteria and therapeutic guidelines for injuries to the collateral ligaments of the proximal interphalangeal (PIP) joint are imprecise and vague. Laxity determinations, failure analysis, radiographic stress testing, and microscopic dissections were performed on 112 PIP joints. The lateral collateral ligament (LCL) is the primary restraint to varus and valgus angulation of the PIP joint. Its palmar fibers are tight in joint extension and provide the first line of resistance to lateral angulation. Failure of the LCL almost always occurs proximally in a sequential fashion that

BACKGROUND: Extra-articular ulnar shortening osteotomy is a common procedure for the surgical treatment of ulnar impaction syndrome. Several techniques for this osteotomy have been developed to avoid the morbidity associated with a standard transverse osteotomy. However, these techniques require special instrumentation and are expensive. The purpose of this study was to evaluate the outcome of step-cut ulnar shortening osteotomy without special jigs for ulnar impaction syndrome.
METHODS: A retrospective study of 164 consecutive patients who underwent step-cut ulnar shortening osteotomy betwee

Volar melanotic macules are asymptomatic, tan-brown to brownish-black macules found on the palms, fingers, soles, and/or toes of dark-skinned individuals and are infrequently reported in light-skinned individuals. Herein, we report the acquisition of multiple, brownish-black volar digital macules in a 63-year-old white woman and avid gardener that were noted after the onset of pruritic, chronic hand dermatitis over a 10-month period. She was healthy without any medical conditions or medications. No family history of intestinal polyposis or mucosal melanosis existed. At presentation, she had le

The purpose of this study was to investigate the sex-based differences in the carpal arch morphology. Carpal arch morphology was quantified using palmar bowing and area of the arch formed by the transverse carpal ligament. The carpal arch was imaged at the distal and proximal tunnel levels using ultrasonography in 20 healthy young adults (10 women and 10 men). It was found that females had a smaller carpal arch height compared to men at both distal and proximal levels (p<0.05) and smaller carpal arch width only at the proximal level (p<0.05) but not distally. Palmar bowing index, the carpal ar

We present an 18 year-old patient with a volar dislocation of the metacarpophalangeal joint of the thumb. Open reduction was performed through a dorsal incision. Because of the soft tissue interposition such as dorsal capsule, volar plate, dislocated extensor pollicis longus and brevis tendons, ruptured ulnar collateral ligament; open reduction, soft tissue and ligament repair are recommended for this type of thumb dislocations.

BACKGROUND: Fractures of the distal radius are common, with volar locking plates being increasingly used in their treatment. They aim to provide stable internal fixation and are designed to mirror the natural anatomy. Current volar plate designs incorporate a volar cortical angle (VCA) of 25 degrees.
HYPOTHESIS: The aim of this study is to determine whether the VCA in uninjured distal radii corresponds accurately with modern volar plate designs.
MATERIALS AND METHODS: A retrospective radiological analysis utilizing Computed Tomography scans to assess the VCA of 100 distal radii. Each distal

Magnetic resonance imaging (MRI) with small surface coils is a well established method for the diagnostic evaluation of finger masses. Until now, histological examination has been required to reliably assess tumor extent and infiltration of surrounding structures. Ultra-high-field MR microscopy (MRM) allows evaluation of anatomical structures and pathologies with submillimeter resolution. This study describes the diagnostic prospects and potential of MRM based on the ex-vivo examination of different finger pathologies. Ten human digits were examined by ex-vivo MRM at 7.1 Tesla (ClinScan, Bruke

Congenital swan neck deformity is a rare condition, secondary to congenital laxity of the palmar plate of the proximal interphalangeal joint. Here, we describe a new surgical method which is based on the palmar plate reinforcement using the Mitek mini anchor system (Mitek Surgical Products Inc., Norwood, MA, USA). Four fingers of a patient with congenital swan neck deformities were corrected with this technique at two stages. First, the anchor was inserted into the volar surface of the proximal phalanx. The two sutures of the anchor system were then crossed the PIP joint in a V fashion. Two ho

Thumb extensor injuries and bony avulsion in the distal phalanx of the thumb are rare compared with other fingers. The most reported complications are infection, nail deformity, joint incongruity, implant failure, recurrent flexion deformity, and residual pain. This report presents a case of 30-year-old man suffering from an injury in the left thumb distal phalanx with a displaced comminuted intra-articular fracture of the distal phalanx of the left thumb. The nail plate was interposed between the dorsal and palmar fragments. The interposition of the nail plate in the bony mallet thumb has not

There are multiple surgical techniques to stabilize the bone in digital replantation. Various criteria need to be considered when choosing appropriate bony fixation including operative time, ease of exposure, stability, limitation of the area of dissection, and reproducibility. We describe our technique using internal fixation with a low-profile plate on the palmar aspect of the proximal or middle phalanx during replantation of a digit. This technique allows coverage of the plate with no risk of compromising the dorsal venous anastomosis. In our experience, using this technique we have observe

PURPOSE: To determine whether the number of distal locking screws significantly affects stability of a cadaveric simulated distal radius fracture fixed with a volar locking plate.
METHODS: We created AO/ASIF type C2 fractures in 10 matched pairs of human fresh-frozen cadaveric wrists and then fixed them using volar locking plates. The number of distal locking screws used was 4 screws or 7 screws in each wrist of the matched pair. We loaded the stabilized fractures cyclically to simulate 6 weeks of postoperative stressing during a therapy protocol and then loaded them to failure. Failure was d

INTRODUCTION: A distal radius fracture is a common injury that affects people of all ages. However, there are various therapy approaches that are controversial. Casting with or without closed reduction has long been considered a feasible option for treating distal radius fractures; alternative options include K-wire fixation and ligamentotax are with external fixation. However, with limited function and impairments, the results are frequently poor. "Several studies have found that distal radius fractures treated with external fixation have good anatomical and functional outcomes, although ther
