The case for routine parathyroid hormone monitoring.

Parathyroid hormone (PTH) is a uremic toxin with multiple systemic effects including bone disorders (renal osteodystrophy), myopathy, neurologic abnormalities, anemia, pruritus, and cardiomyopathy. Hyperparathyroidism is common in CKD and results in significant morbidity and mortality if left untreated. Clinical practice guidelines from the Kidney Disease Improving Global Outcomes initiative broadened the optimal PTH range to >2 and <9 times the upper limit of normal for the assay measured. Furthermore, the guidelines recommend following trends in PTH to determine the appropriate therapy. Thes…

Parathyroid hormone applications in the craniofacial skeleton.

Parathyroid hormone (PTH) is known for its ability to 'build' bone, with research in this area centered on its use as an osteoporosis therapeutic. Recent interest has developed regarding its potential for regenerative applications such as fracture healing and osseous defects of the oral cavity. Many years of investigation using murine gene-targeted models substantiate a role for signaling at the PTH/PTH-related protein (PTHrP) receptor (PPR) in intramembranous bone formation in the craniofacial region as well as in tooth development. Pre-clinical studies clearly support a positive role of inte…

Role of parathyroid hormone monitoring during parathyroidectomy.

BACKGROUND: The value of intraoperative parathyroid hormone (IOPTH) monitoring in parathyroidectomy has been previously debated. METHODS: This retrospective analysis was of 240 patients, operated on for primary hyperparathyroidism (PHPT) between January 2002 and January 2006 and categorized into 3 groups by preoperative and intraoperative modalities: group 1 (n = 109), technetium 99m sestamibi (MIBI), ultrasonography, and IOPTH; group 2 (n = 102), ultrasonography and MIBI; and group 3 (n = 29), ultrasonography and IOPTH. Sensitivity and specificity were calculated. RESULTS: In group 1, IOPTH…

Significance of elevated parathyroid hormone after parathyroidectomy.

OBJECTIVE: To review the prevalence of parathyroid hormone elevation after parathyroidectomy for primary hyperparathyroidism and to discuss possible mechanisms. METHODS: A Medline search of the English-language literature published between 1990 and 2009 was performed using the search terms "elevated PTH after parathyroidectomy." All of the identified articles reported either prospective or retrospective studies without control groups. Studies that included patients with secondary or tertiary hyperparathyroidism were not reviewed. RESULTS: Within 1 week to 5 years after parathyroidectomy, 9% …

[Intraoperative parathyroid hormone determination for primary hyperparathyroidism].

Intraoperative parathyroid hormone measurement (IOPTH) has proved to be an important promoter for focused and minimally invasive parathyroidectomy procedures in primary hyperparathyroidism. IOPTH enables multiglandular disease to be excluded with a high degree of certainty at the time of operation. The choice of the cut-off value for IOPTH as the criterion for success is of utmost importance with respect to the prognosis for operative success (biochemical healing). Advantages and disadvantages of the variety of existing IOPTH success criteria are confusing and their assessment is contradictory…

Therapy of hypoparathyroidism with intact parathyroid hormone.

UNLABELLED: Hypoparathyroidism, a disorder characterized by low parathyroid hormone (PTH), is generally treated with oral calcium and vitamin D supplementation. We investigated the effects of PTH(1-84) treatment in 30 hypoparathyroid subjects for 24 months. PTH(1-84) treatment in hypoparathyroidism significantly reduced supplemental calcium and 1,25-dihydroxyvitamin D requirements without generally altering serum and urinary calcium levels. INTRODUCTION: Hypoparathyroidism, a disorder characterized by low PTH, is associated with hypocalcemia, hypercalciuria, and increased bone mineral density…

Hormone replacement after thyroid and parathyroid surgery.

BACKGROUND: Hypothyroidism and hypocalcemia are common after thyroid and parathyroid surgery. In this article, the authors provide clinically-oriented recommendations to help surgeons, general practitioners, internists, and endocrinologists give their affected patients adequate hormone replacement therapy. METHODS: Selective evaluation of original articles and reviews that were retrieved by a PubMed search over the years 1980 to 2010, as well as of the recommendations of medical societies including the Endocrine Society (USA), the German Society for Endocrinology (Deutsche Gesellschaft für En

Parathyroid hormone reserve in 22q11.2 deletion syndrome.

OBJECTIVE: We hypothesized that most patients with 22q11.2 deletion and a history of hypocalcemia have inadequate parathyroid function, manifested by intact parathyroid hormone levels below normal. We aimed to evaluate intact parathyroid hormone levels both during normocalcemia and at hypocalcemia, in this population. STUDY DESIGN: Retrospective chart review of 103 patients with 22q11.2 deletion born since 1997 and cared for at the Children's Hospital of Philadelphia. Calcium and intact parathyroid hormone drawn simultaneously were recorded, along with clinical presentation at hypocalcemia. …

Parathyroid hormone regulates histone deacetylases in osteoblasts.

Parathyroid hormone (PTH) functions as an essential regulator of calcium homeostasis and as a mediator of bone remodeling. We have already shown that PTH stimulates the expression of matrix metalloproteinase-13 (MMP-13), which is responsible for degrading components of extracellular matrix. We have hypothesized that histone deacetylases (HDACs) are involved with PTH-induced MMP-13 gene expression in the osteoblastic cell line, UMR 106-01. We have shown that PTH profoundly regulates HDAC4 in UMR 106-01 cells through a PKA-dependent pathway, leading to removal of HDAC4 from the MMP-13 promoter a…

Perioperative parathyroid hormone levels in thyroid surgery.

OBJECTIVE: Perioperative hypocalcemia from temporary parathyroid gland dysfunction is common after thyroid surgery. No reliable cutoff values for parathyroid hormone (PTH) and the subsequent possibility of developing hypocalcemia exist. The purpose of this study is to determine a criterion for predicting hypocalcemia based on different PTH levels as cutoff values. STUDY DESIGN: Retrospective chart review. METHODS: A centralized database of intraoperative PTH levels was analyzed. PTH values approximately 10 minutes after excision of the thyroid gland and in the recovery room were obtained; se…

Therapy of male osteoporosis with parathyroid hormone.

Definable causes of male osteoporosis account for only about 60% of the osteoporotic population. Those for whom no etiology is readily apparent are said to have primary or idiopathic male osteoporosis. In these individuals, histomorphometric studies indicate that this is a disorder that is more typically characterized by low turnover. Although antiresorptive agents such as alendronate have been shown to increase bone mass in men, the rationale for an anabolic agent that can stimulate bone formation is clear. The most attractive anabolic agent at this time is parathyroid hormone (PTH) administe…

Parathyroid hormone gene polymorphisms in primary hyperparathyroidism.

OBJECTIVE: Genetic contributions to bone mineral density (BMD) and bone turnover are well known. In the present study, we analysed the relationship between restriction fragment length polymorphisms of the PTH gene and the development of primary hyperparathyroidism (pHPT) as well as its severity. PATIENTS: Seventy-nine pHPT patients and 104 age-matched healthy controls were analysed. DESIGN AND MEASUREMENTS: PTH genotypes were determined by polymerase chain reaction and BstB I or Dra II restriction fragment length polymorphisms. The presence and absence of BstB I or Dra II restriction sites o

Parathyroid hormone-related protein and skin cancer.

Parathyroid hormone-related protein (PTHrP) is now well established as the major, if not the sole mediator of the humoral hypercalcaemia of malignancy. This is the phenomenon of elevated blood calcium in the absence of bony metastases in patients with certain solid tumours, most commonly squamous cell carcinoma (SCC) of the lung, but also occurring in SCC at other sites including skin, and other cancers including melanoma. Although discovered because of its hormonal action in cancer, there is at present no indication that PTHrP acts as a circulating hormone in normal animals or in humans after…

Clinical performance of parathyroid hormone immunometric assays.

Three immunometric assays of parathyroid hormone (PTH)--a commercial immunoradiometric assay, an in-house immunoradiometric assay, and an immunochemiluminometric assay--were evaluated in 50 patients with surgically proven primary hyperparathyroidism. Of these patients, 43 had increased values with the commercial assay (sensitivity, 86%), whereas 45 patients had increased concentrations with both the in-house immunoradiometric and the in-house immunochemiluminometric assays (sensitivities, 90%). Because of the results of this comparison study, we confidently chose the immunochemiluminometric as…

Mechanisms of parathyroid hormone resistance in pseudohypoparathyroidism.

The problem with PHP is that it is almost certainly a heterogeneous condition encompassing a number of defects within the PTH receptor-adenylate cyclase complex. The clinical manifestations of PTH resistance are amplified and extended by a number of associated abnormalities. These include a low intracellular calcium concentration, secondary hyperparathyroidism, reduced production of 1,25-(OH)2D and perhaps the secretion of an abnormal PTH peptide. Almost every aspect of PTH function seems involved and this makes PHP a fascinating condition in which to test current views of the role of PTH in t

Effects of parathyroid hormone on urinary acidification.

Recollection micropuncture experiments were carried out in plasma-replete euvolemic thyroparathyroidectomized rats to examine the effects of a purified bovine parathyroid hormone (PTH) infusion on urinary acidification. After a 60-min equilibration period, PTH administration had the following effects: reabsorption in the proximal convoluted tubule was inhibited 13% for total CO2, 31% for chloride, and 28% for water; early distal delivery, however, remained unchanged for bicarbonate, chloride, and water principally as a result of stimulation of bicarbonate and chloride absorption in Henle's loo…
