[Etiology, therapy and prophylaxis of bovine parturient paresis (hypocalcaemia)].

Due to the sudden increase of calcium demand at the onset of lactation many high yielding dairy cows experience a certain level of hypocalcaemia following parturition. The incidence of hypocalcaemia (parturient paresis) increases with age but also depends on many other factors such as the acid-base status and the availability of calcium as well as other minerals and trace elements. Hypocalcaemia can easily be treated by supplementation of calcium parenterally or orally, nonetheless, prophylaxis of the condition should be the main focus in modern dairy farming, in order to avoid its negative ef…

[Spontaneous regeneration of facial nerve paresis caused by surgery].

Facial nerve paresis is a known complication to surgery in the facial region. Spontaneous regeneration of such injuries has been described, but little is known about the prognosis and optimal management. This is a case report of two patients with partial facial nerve paresis following surgery. The patients were treated conservatively. Both experienced a regeneration over ten months post-operatively. This suggests a favourable prognosis with conservative treatment in selected cases, which is reassuring for both doctors and patients.

Pupil-Involving Third Cranial Nerve Paresis at High Altitude.

Blomquist, Preston H. Pupil-involving third cranial nerve paresis at high altitude. High Alt Med Biol. 19:286-287, 2018.-Although sixth cranial nerve palsies are a well-recognized entity at high altitude, other cranial nerve palsies due to altitude are much more uncommon. A case of a 55-year-old woman is presented who flew from Dallas, Texas (elevation 176 m) to Breckenridge, Colorado, where she hiked up to 3600 m above sea level. She developed a pupil-involving right third cranial nerve paresis that resolved over the next 3 months. In the absence of significant abnormalities on neuroimaging a…

Neuromuscular compensation mechanisms in vocal fold paralysis and paresis.

OBJECTIVES/HYPOTHESIS: Vocal fold paresis and paralysis are common conditions. Treatment options include augmentation laryngoplasty and voice therapy. The optimal management for this condition is unclear. The objective of this study was to assess possible neuromuscular compensation mechanisms that could potentially be used in the treatment of vocal fold paresis and paralysis. STUDY DESIGN: In vivo canine model. METHODS: In an in vivo canine model, we examined three conditions: 1) unilateral right recurrent laryngeal nerve (RLN) paresis and paralysis, 2) unilateral superior laryngeal nerve (S…

A Rare Complication of Herpes Zoster: Segmental Zoster Paresis.

Herpes zoster is a common presentation in both the community and emergency department; however segmental zoster paresis is a rare complication that can lead to misdiagnosis. We present a case of a 74-year-old Indian gentleman with a background of well controlled diabetes mellitus, hypertension, and ischaemic heart disease who presented with sudden right lower limb weakness. This was preceded by a 5-day history of paraesthesia starting in the right foot and ascending up the right lower limb. On examination, there was a characteristic vesicular rash in the L2/3 region with MRC grading 3/5 in the…

Congenital Oculomotor Nerve Paresis With Isolated Cyclic Pupillary Spasms.

Cyclic oculomotor nerve paresis is a rare and usually congenital disorder. It is characterized by unilateral third nerve paresis with periodic spasms causing eyelid elevation, miosis, and contraction of 1 or more of the extraocular muscles innervated by the third nerve. We report a 20-month-old girl who presented initially with a congenital partial right third nerve paresis without ptosis. She subsequently developed isolated cyclic spasms of the pupil followed several months later by permanent partial ptosis.

Management of bilateral recurrent laryngeal nerve paresis after thyroidectomy.

Bilateral recurrent laryngeal nerve (RLN) injury is rare for benign thyroid lesions (0.2%). After extubation-stridor, respiratory distress, aphonia occurs due to the closure of the glottic aperture necessitating immediate intervention and emergency intubation or tracheostomy. Intra-operative identification and preservation of the RLN minimizes the risk of injury. It is customary to expect RLN problems after thyroid surgery especially if malignancy, big thyroid, distorted anatomical problems and difficult airway that can lead to intubation trauma. Soon after extubating, it is essential to the a

Vocal fold paresis: etiology, clinical diagnosis and clinical management.

PURPOSE OF REVIEW: Recent papers on incidence of unilateral paresis in general and for specific causes; techniques to assist clinical diagnosis; computer and animal modeling leading to new assessment tools; and studies on the effectiveness of clinical management. RECENT FINDINGS: There is much interest in being able to assess unilateral paresis in patients both pre and post-thyroidectomy. Because the gold standard for defining the presence of paresis is by laryngeal electromyography, which is not routinely available, much interest has been placed in finding other measures that correlate with …

Recurrent psychogenic paresis after dural puncture in a parturient.

We describe the case of a 29-year-old parturient who, after undergoing elective cesarean delivery, displayed symptoms of lower extremity wea … ic resonance imaging scan revealed a normal spine with no evidence of hematoma. The lower extremity paresis persisted and a neurologist diagnosed psychogenic paresis, a type of conversion disorder. Interestingly, the patient's postoperative leg paresis was not her first occurrence of neurological dysfunction after dural puncture. At 27 weeks of gestation, …

Entropion in children with isolated peripheral facial nerve paresis.

PURPOSE: Adults with facial nerve paresis (FNP) generally develop ectropion, but a recent report of children with syndromatic FNPs implies that entropion may be more common in this setting than ectropion. This study evaluates eyelid position and other periorbital changes in children with isolated, non-syndromatic FNP. METHODS: Charts were reviewed of 10 sequential children who presented to a major national eye referral centre with isolated FNP of variable aetiology. Severity of FNP was assessed according to the House-Brackmann scale. RESULTS: All 10 patients (4 males and 6 females; mean age …

Hemispheric hypoperfusion in postictal paresis mimics early brain ischemia.

The clinical differentiation between stroke and seizure is usually straightforward but postictal neurological deficits can be mistaken for stroke in case no detailed medical history is available. Up to now, the imaging findings of Todd's paresis are not well described. This case report demonstrates that postictal paresis can be accompanied by a reversible global hemispheric hypoperfusion as measured with perfusion MRI indicating transient but profound cerebrovascular dysfunction in postictal paresis. Extensive postictal perfusion changes must be discriminated from emerging stroke to avoid pote…

Subacute hypoglossal nerve paresis with internal carotid artery dissection.

OBJECTIVES: To describe a case of an isolated hypoglossal nerve palsy in a patient with a spontaneous internal carotid artery dissection (ICAD). This condition is a well-recognized cause of cerebral ischemic stroke in patients younger than 45 years of age. Isolated cranial nerve neuropathy is a rare presentation. More common manifestations include incomplete hemiparesis, hemicrania, Horner syndrome, cervical bruit, pulsatile tinnitus, and multiple cranial nerve palsies. METHODS: A comprehensive literature search (Ovid, MEDLINE) for the presentation, diagnostic evaluation, treatment, and outco…

Modified Harada-Ito procedure in bilateral superior oblique paresis.

BACKGROUND: Patients with bilateral superior oblique palsies may complain of diplopia and torsion, especially in downgaze. They are difficult to manage surgically, although bilateral modified Harada-Ito procedures may be of value. METHODS: A retrospective review was carried out of patients treated with bilateral Fells' modified Harada-Ito procedures for bilateral superior oblique palsy between 1989 and March 2000, the patients being identified from operating records. RESULTS: Twenty-three patients were identified (9 females and 14 males) with a mean age at presentation of 37 years (range 19

Hemostatic alterations in patients with acute, unilateral vestibular paresis.

OBJECTIVES: The etiopathogenesis of acute unilateral peripheral vestibulopathy (APV) still remains a matter of debate; ischemic changes in the circulation of the labyrinth may play a role. We consequently looked for possible hemostasis alterations in a group of patients with APV of an unknown nature. METHODS: We evaluated blood parameters known to be involved in circulation disorders, including total and HDL cholesterol, triglycerides, apolipoprotein A and B, lipoprotein(a), homocysteine, folate, prothrombin time, activated partial thromboplastin time, fibrinogen, D-dimer, antithrombin III, p

Prevalence of laryngeal paresis in dogs undergoing general anaesthesia.

OBJECTIVE: To estimate the prevalence of laryngeal paresis within a selected population of dogs and identify some of the distinguishing characteristics of affected dogs. DESIGN: A prospective study involving laryngoscopic examination of 250 dogs. PROCEDURE: The laryngeal movements of 250 dogs undergoing general anaesthesia were observed. The severity of laryngeal paresis in these dogs was graded (0 = normal laryngeal movements, 4 = bilateral laryngeal paralysis). The following information was also recorded for each dog: age, sex, weight, breed, condition score, anaesthetic protocol, clinical…
