Early complications associated with fixation constructs of operatively treated patella fractures: a retrospective study.

PURPOSE: Although multiple fixation constructs exist for patella fractures, postoperative complications are common, and optimal construct choice remains unclear. This study evaluated construct-specific complication rates in operatively treated patella fractures. METHODS: A retrospective review was conducted of adult patients who underwent surgical fixation of patella fractures (AO/OTA 34) at a Level I trauma center between 2012 and 2022. Patients younger than 18 years, treated with isolated cerclage wiring, or with less than 90 days of follow-up were excluded. Patients were stratified by fixat…

Complications following surgical treatment of patella fractures - a systematic review and proportional meta-analysis.

PURPOSE: The aim of this systematic review and proportional meta-analysis was to identify complications of surgical treatment of patella fractures and to estimate their incidence. We extended existing knowledge on this topic by including several more recent and large-scale studies. METHODS: This systematic review and meta-analysis were performed in accordance with the Cochrane Handbook for systematic reviews of interventions. After searching in PubMed, MEDLINE, EMBASE, Cochrane Library, and OpenGrey, all studies after 2000, with study populations > 100 patients, including only patients > 18 y…

The use of a 30-degree radiolucent triangle during surgery in distal avulsion fractures of the patella.

BACKGROUND: Avoiding patella baja or alta after the Krackow suture technique for distal avulsion fractures of the patella can be challenging. We aim to introduce a simple and reproducible technique using a 30-degree radiolucent triangle involving the contralateral knee to ensure the correct positioning of the patella intraoperatively. METHOD: The radiolucent triangle is positioned under the contralateral knee before operating the injured knee. A strict lateral view is obtained using fluoroscopy as a reference before a Krackow technique is performed on the avulsion fracture of the patella. RE

Fixation of patella fractures with a minimally invasive tensioned wire method: compressive external fixation.

BACKGROUND: Current fixation methods of patella fractures are associated with hardware prominence, infection, and failure of fixation. These complications necessitate repeat surgical treatment, increase costs, and limit function. We investigated whether a novel device can effectively treat patella fractures with few complications. METHODS: A consecutive series of patients with displaced patella fractures with transverse and select other patterns were treated with the technique of compressive external fixation (CEF), a device based on an external tensioned wire construct. Patients were allowed…

Cannulated screw and cable are superior to modified tension band in the treatment of transverse patella fractures.

BACKGROUND: Although the modified tension band technique (eg, tension band supplemented by longitudinal Kirschner wires) has long been the mainstay for fixation of transverse fractures of the patella, it has shortcomings, such as bad reduction, loosening of implants, and skin irritation. QUESTIONS/PURPOSES: We conducted a retrospective comparison of the modified tension band technique and the titanium cable-cannulated screw tension band technique. PATIENTS AND METHODS: We retrospectively reviewed 101 patients aged 22 to 85 years (mean, 56.6 years) with AO/OTA 34-C1 fractures (n = 68) and 34-

Management of Patella Fracture Nonunion and Large Bone Defect After Bone-Patellar Tendon-Bone Autograft: A Case Report.

CASE: A 32-year-old patient was diagnosed with a vertical patella fracture nonunion after a bone-patellar tendon-bone (BTB) anterior cruciate ligament reconstruction. In addition, a 1 × 2 × 1 cm patellar bone defect was noticed at the graft harvesting site. The patient was treated surgically with open reduction and internal fixation and iliac crest bone autograft which resulted in fracture union. CONCLUSION: Many intraoperative and postoperative risk factors for iatrogenic patella fracture when harvesting BTB autograft were identified. Surgeons should be aware of technical skills needed to pr…

Why Mechanism Matters: A Literature Review of Simultaneous Ipsilateral Tibial Tuberosity Avulsion and Patella Fracture With Case Report.

Simultaneous ipsilateral tibial tuberosity avulsion and patella fractures are rare in adults. They are often associated with patients who have underlying bone disease and other medical co-morbidities. Here we describe a case where this injury was attributed to direct trauma and demonstrate our department's management for such an injury. In addition to our case report, we have performed a systematic literature review to identify other cases of the same injury. Only four other cases have been reported. Here we summarise and compare the management and outcome measures reported in each case. All p…

Titanium mesh as a low-profile alternative for tension-band augmentation in patella fracture fixation: A biomechanical study.

OBJECTIVES: We performed a simple biomechanical study to compare the fixation strength of titanium mesh with traditional tension-band augmentation, which is a standard treatment for transverse patella fractures. We hypothesised that titanium mesh augmentation is not inferior in fixation strength to the standard treatment. METHODS: Twenty-four synthetic patellae were tested. Twelve were fixed with stainless steel wire and parallel cannulated screws. Twelve were fixed with parallel cannulated screws, augmented with anterior titanium mesh and four screws. A custom test fixture was developed to s…
