Self-harm and suicide during and after opioid agonist treatment among primary care patients in England: a cohort study.

BACKGROUND: The first 4 weeks after initiation and cessation of opioid agonist treatment for opioid dependence are associated with an increased risk of all-cause mortality and overdose. We aimed to investigate whether the rate of self-harm and suicide among people who were prescribed opioid agonist treatment differs during initiation, cessation, and the remainder of time on and off treatment. METHODS: We did a retrospective cohort study and used health-care records from UK Clinical Practice Research Datalink, linked to mortality and hospital admission data, for adults (age 18-75 years at coho

Physical activity promotion for multimorbid patients in primary care settings: a protocol for a systematic review evaluating health benefits and harms.

BACKGROUND: To date multimorbidity has not received much attention in health policies, even though multiple chronic diseases put high demands on the health care system in industrial nations. Enormous costs of care and a physically, mentally, and socially reduced quality of life are common consequences of multimorbidity. Physical activity (PA) has a positive preventive and therapeutic effect on common non-communicable diseases. The objective of this study will be to evaluate the health benefits and harms of PA interventions for sedentary adults with multimorbidity in primary care settings. MET

The Association Between Self-Reported Awake Oral Behaviors and Orofacial Pain Depends on the Belief of Patients That These Behaviors Are Harmful to the Jaw.

AIMS: To examine if the existence of an association between self-reported awake oral behaviors and orofacial pain depends on the belief of patients that these behaviors are harmful to the jaw and to investigate if an additional variable (ie, somatic symptoms, depression, and/or anxiety) indirectly affects the association between the causal attribution belief and the report of awake oral behaviors. METHODS: Prior to the first clinical visit, patients referred to a specialized clinic for complaints of orofacial pain and dysfunction completed a digital questionnaire. Data of 329 patients diagnos

Do the benefits of using calcitriol and other vitamin D receptor activators in patients with chronic kidney disease outweigh the harms?

The primary indication for administration of calcitriol or other vitamin D receptor activators (VDRA) in chronic kidney disease (CKD) is secondary hyperparathyroidism (SHPT). Prevention and treatment of SHPT appears important, as imbalances in mineral metabolism are associated with renal osteodystrophy, and higher parathyroid hormone (PTH) levels are associated with increased rates of mortality and morbidity in CKD patients. There is, however, a lack of controlled trial data that show lowering PTH with calcitriol/VDRA equates to improved clinical outcomes. Recent randomized controlled trials h

Which Harms to Patients Affect Family-Building Decision-Making in States in Which Legislatures Ban or Restrict Abortion Care?

AbstractAbortion bans and restrictions harm patients and are not evidence based. Bans express poor understanding of the clinical complexity of many patients' need for early intervention and need to avoid mortality or morbidity. Restrictions delay clinically indicated care and avoidable complications and contribute to poor physical and mental health outcomes; they also intrude in family decision-making and contribute to poor neonatal outcomes. This commentary on a case considers how bans and restrictions ignore patients' views about their and their family's quality of life and ignore patients'

When Fruit Turns Harmful: Late Diagnosis of Hereditary Fructose Intolerance in a Pediatric Patient-A Case Report and Literature Review.

INTRODUCTION: Hereditary fructose intolerance is a rare but potentially severe and fatal disorder if it is not recognized promptly. It is caused by biallelic mutations in the ALDOB gene, which encodes the Aldolase B enzyme. Deficiency leads to intracellular accumulation of fructose-1-phosphate, causing secondary inhibition of gluconeogenesis and glucogenolysis and, consequently, hypoglycemia along with hepatic and renal dysfunction. The clinical presentation is variable and often nonspecific, making diagnosis challenging. CASE REPORT: A 12-year-old boy, born small for gestational age, with a

Assessment of Personality Profiles Among Patients Presenting With Deliberate Self-Harm at a Tertiary Care Hospital: A Cross-Sectional Study.

Background Deliberate self-harm (DSH) represents intentional self-inflicted physical injury encompassing both non-suicidal self-injurious acts and suicide attempts, constituting a significant psychiatric emergency with profound morbidity implications. Individuals with personality disorders demonstrate substantially elevated suicide attempt rates compared to those without personality pathology. Comprehensive psychosocial assessment remains fundamental for evaluating DSH presentations. While research has investigated socioeconomic determinants and personality disorders among adolescent populatio

Monitoring patients with acute respiratory failure during non-invasive respiratory support to minimize harm and identify treatment failure.

Non-invasive respiratory support (NRS), including high flow nasal oxygen therapy, continuous positive airway pressure and non-invasive ventilation, is a cornerstone in the management of critically ill patients who develop acute respiratory failure (ARF). Overall, NRS reduces the work of breathing and relieves dyspnea in many patients with ARF, sometimes avoiding the need for intubation and invasive mechanical ventilation with variable efficacy across diverse clinical scenarios. Nonetheless, prolonged exposure to NRS in the presence of sustained high respiratory drive and effort can result in r

Impaired Decision-Making Capacity in a Patient With Oral Squamous Cell Carcinoma: Weighing the Benefits and Harms of Treatment Against Objection.

Impairment of medical decision-making capacity is common in acutely ill patients especially those with solid malignancies. Many of these patients lack advance directives and healthcare proxies leaving clinicians with unclear guidance on subsequent treatment decisions and few existing mechanisms to address this issue. We present a case of a 55-year-old man with recurrent oral squamous cell carcinoma who lacked decision-making capacity due to cognitive impairment. He did not have an advance directive or documented healthcare proxy. He declined curative-intent cancer-directed therapy. The dilemma

Impact of Real-Time Comorbidity Capture on Clinical Escalation and Harm Prevention in Hospitalized Patients: A Benchmarking and Statistical Gap Analysis.

Background Systemic documentation deficiencies in hospitalized patients with high-acuity conditions such as systemic inflammatory response syndrome (SIRS) can compromise clinical decision-making, risk adjustment, and patient safety. Despite well-established evidence linking chronic conditions like chronic kidney disease (CKD), diabetes mellitus (DM), and liver failure to adverse outcomes, these comorbidities are often underrepresented in electronic health records (EHRs). This study introduces a statistically rigorous, informatics-assisted framework to benchmark and quantify real-world document

Benefit-Harm Analysis of Earlier Initiation of Triple Therapy for Prevention of Acute Exacerbation in Patients with Chronic Obstructive Pulmonary Disease.

Rationale: Reducing the risk of exacerbation is a fundamental goal in managing stable chronic obstructive pulmonary disease (COPD). Guidelines recommend triple therapy (inhaled corticosteroids, long-acting muscarinic antagonists, and long-acting β-agonists) only as a stepup from dual therapy (long-acting muscarinic antagonists and long-acting β-agonists) for patients at continued high risk of exacerbation, because of the trade-off of an increased risk of pneumonia associated with inhaled corticosteroid-containing therapies. However, there is little evidence on the optimum timing of initiating

Risk of self-harm in patients with eating disorders: English population-based national record-linkage study, 1999-2021.

OBJECTIVE: Anorexia nervosa (AN) and bulimia nervosa (BN) are eating disorders associated with high rates of self-harm (SH). This is the first national study in England to quantify this association in a hospital population. METHOD: A retrospective cohort study using a linked national dataset of Hospital Episode Statistics for 1999-2021. The exposure cohort included individuals aged <35 years admitted to hospital with a diagnosis of AN or BN. The reference cohort included hospital controls. We calculated the rate ratio (RR) of SH in each cohort. The individuals in the two cohorts were matched

Patient-level interventions to reduce alcohol-related harms in low- and middle-income countries: A systematic review and meta-summary.

BACKGROUND: Disease and disability from alcohol use disproportionately impact people in low- and middle-income countries (LMICs). While varied interventions have been shown to reduce alcohol use in high-income countries, their efficacy in LMICs has not been assessed. This systematic review describes current published literature on patient-level alcohol interventions in LMICs and specifically describes clinical trials evaluating interventions to reduce alcohol use in LMICs. METHODS AND FINDINGS: In accordance with PRISMA, we performed a systematic review using an electronic search strategy fro

Harm Reduction in Male Patients Actively Using Anabolic Androgenic Steroids (AAS) and Performance-Enhancing Drugs (PEDs): a Review.

Anabolic androgenic steroid (AAS) and performance-enhancing drug (PED) use is a prevalent medical issue, especially among men, with an estimated 2.9-4 million Americans using AAS in their lifetime. Prior studies of AAS use reveal an association with polycythemia, dyslipidemia, infertility, hypertension, left ventricular hypertrophy, and multiple behavioral disorders. AAS withdrawal syndrome, a state of depression, anhedonia, and sexual dysfunction after discontinuing AAS use, is a common barrier to successful cessation. Clinical resources for these patients and training of physicians on manage

Risk of harm in synthetic and biological intervention trials in patients with inflammatory arthritis: protocol for a metaepidemiological study focusing on contextual factors.

INTRODUCTION: Inflammatory arthritis (IA) conditions, including rheumatoid arthritis, psoriatic arthritis and axial spondyloarthritis, are characterised by inflammatory infiltration of the joints. Biological disease-modifying antirheumatic drugs (bDMARDs) and targeted synthetic disease-modifying antirheumatic drugs (tsDMARDs), respectively, reduce the effects of proinflammatory cytokines and immune cells to ameliorate disease. However, immunosuppression can be associated with high rates of serious adverse events (SAEs), including serious infections, and maybe an increased risk of malignancies

Angiotensin-converting enzyme inhibitors and angiotensin receptor blockers may be harmful in patients with diabetes during COVID-19 pandemic.

The novel coronavirus disease 2019 (COVID-19) outbreak once again demonstrated the importance of the renin-angiotensin system (RAS) in patients with diabetes. Activation of the RAS increases in patients with diabetes. The virus attaches to the ACE2 enzyme at low cytosolic pH values and enters into the cell and causes infection. Especially in the presence of diabetes mellitus and accompanying comorbid conditions such as hypertension, obesity, old age, and smoking, cytosolic pH is low, thus the virus easily may enter the cell by attaching to ACE2. ACEIs and ARBs lead to a reduction in angiotensi
