Cyanosis and Raynaud's phenomenon: the relation to underlying disease and venous abnormalities.

Matched groups of 7 to 8 patients with primary Raynaud's phenomenon, systemic sclerosis, and undifferentiated connective tissue disease and cold tolerant normal control subjects were studied by simultaneous digital strain gauge plethysmography and laser Doppler capillary velocimetry during two controlled cycles of hand warming and cooling. Nailfold capillaroscopic assessments and measures of in vivo platelet activation were performed. Triphasic color changes were present in all patients with systemic sclerosis, in 6 of 8 with undifferentiated connective tissue disease, but in only 1 of 7 with …

Persistence of static lung hyperinflation in adults with cystic fibrosis treated with elexacaftor-tezacaftor-ivacaftor.

BACKGROUND: This study sought to describe the prevalence of lung hyperinflation in adults (≥18 years) with cystic fibrosis (awCF) and the effects of elexacaftor-tezacaftor-ivacaftor (ETI) on hyperinflation. METHODS: Consecutive awCF who initiated ETI at Cochin Hospital (Paris, France) and had performed body plethysmography within 12 months prior to ETI initiation were included. Abnormal lung volumes (including total lung capacity (TLC) and residual volume (RV)) were defined as > upper limit of normal (ULN) using the Global Lung Initiative reference values. Lung hyperinflation was defined as R…

Airway resistance and airflow dynamics after fat injection into vocal folds.

OBJECTIVES: The aim of this prospective study was to verify whether vocal fold fat augmentation (VFFA) modifies upper airway patency. To the best of our knowledge, this is the first study analyzing the impact of VFFA on laryngeal resistance to airflow. METHODS: Twenty-one consecutive patients 16 to 74 years of age underwent 24 VFFA operations because of glottic incompetence due to laryngeal hemiplegia (13 patients) or vocal fold tissue defects (8 patients). Flow-volume loop spirometry and body plethysmography were performed before and 1 to 6 months after surgery. RESULTS: There were no signi…

Assessment of functional abnormalities in infants and children with lung disease.

Lung function testing in infants and children has to cover a wide palette of various investigational procedures and the fact that babies can predominantly only be tested asleep and young children may not cooperate accurately must be taken into account. There is still a life-span between 2 and 5 years in which lung function testing is impossible. Different techniques for infants lung function testing (spirometry, squeeze-jacket method, forced oscillation technique, occlusion technique, whole-body infant-plethysmography, multibreath nitrogen washout (MBNW), interruption technique), which helps a…

Diminished pulmonary function in pectus excavatum: from denying the problem to finding the mechanism.

BACKGROUND: Recently, technical improvement in the ability to measure lung function and the severity of chest deformity have enabled progress in understanding the mechanism of limitations of lung function in pectus excavatum. METHODS: After establishing that most patients with pectus excavatum do have symptoms of exercise intolerance, easy fatigability, and shortness of breath with exertion, lung function has been evaluated by a variety of methods in different centers. Spirometry, plethysmography, exercise testing, oculo electronic plethysmography, and imaging methods have been used to assess…

The pressure/volume relationship of the calf: a measurement of vein compliance?

OBJECTIVES: The role of compliance changes in the patho-physiology of venous disease is not well known mainly because of difficulty to measure compliance of veins in situ. This study suggests a method to determine the calf pressure/volume relationship by utilizing venous occlusion plethysmography combined with dorsal vein pressure. DESIGN: Comparison between two techniques of measuring calf pressure/volume relationship using air plethysmography with validation against popliteal vein diameter changes detected by duplex ultrasound. SETTING Vascular laboratory. MATERIALS: In 6 normal and 6 radi…

Work of Breathing in Premature Neonates: Noninvasive Neurally-Adjusted Ventilatory Assist versus Noninvasive Ventilation.

BACKGROUND: We tested whether work of breathing in premature newborns estimated by phase angle (θ) by using respiratory inductance plethysmography is decreased during neurally-adjusted ventilatory assist (NAVA) noninvasive ventilation (NIV) versus NIV alone. METHODS: NAVA NIV and NIV were applied in random order while using respiratory inductance plethysmography to measure the phase angle. RESULTS: Patient-ventilator asynchrony was decreased during NAVA NIV; however, the phase angle was not different between the modes. A large number of repeated assists with switches to backup were found whe…

Methods of evaluation and quantification of microangiopathy in high perfusion microangiopathy (chronic venous insufficiency and diabetic microangiopathy).

Noninvasive macro- and microcirculatory tests provide quantitative information that offers answers to most questions posed in venous diseases. Duplex scanning is used to assess the macrocirculation, and microcirculatory methods are used to assess and quantify venous microangiopathy. Laser Doppler flowmetry is used to assess perfusion. Transcutaneous Po(2) and Pco(2) measurements are used to study venous hypertension, and strain-gauge plethysmography is used to assess capillary filtration. In venous hypertension, fluid filtration into the extracapillary compartment is increased. The increase in…

Acute lipodermatosclerosis is associated with venous insufficiency.

BACKGROUND: Acute lipodermatosclerosis is characterized by pain and tenderness in the medial aspect of the leg. It is thought to be the result of venous insufficiency and to be the acute counterpart of chronic lipodermatosclerosis, a hallmark of venous disease. However, there is no direct evidence linking acute lipodermatosclerosis to venous disease. OBJECTIVE: Our purpose was to determine whether acute lipodermatosclerosis is associated with venous insufficiency. METHODS: With air plethysmography, we determined the venous filling index, the ejection fraction, and the residual volume fractio…

A respiratory signature of disease progression in the Pompe rat.

Early-onset Pompe disease occurs due to mutations in the acid α-glucosidase (GAA) gene that result in the absence of functional GAA protein. This results in widespread glycogen accumulation and cardiorespiratory failure early in life. We used a Gaa null (Gaa-/-) rat Pompe model and concurrent analysis of diaphragm electromyography (EMG) and plethysmography respiratory waveforms to determine whether a unique "respiratory signature" would develop over the lifespan. Intramuscular wires enabled diaphragm EMG recordings during whole body plethysmography. Measurements were taken from 4 to 10 mo of a…

Changes in lung volumes and gas trapping in patients with large hiatal hernia.

BACKGROUND AND AIMS: Studies assessing hiatal hernia (HH)-related effects on lung volumes derived by body plethysmography are limited. We aimed to evaluate the effect of hernia size on lung volumes (including assessment by body plethysmography) and the relationship to functional capacity, as well as the impact of corrective surgery. METHODS: Seventy-three patients (70 ± 10 years; 54 female) with large HH [mean ± standard deviation, intra-thoracic stomach (ITS) (%): 63 ± 20%; type III in 65/73] had respiratory function data (spirometry, 73/73; body plethysmography, 64/73; diffusing capacity, 7…
