Action of deferoxamine against Pneumocystis carinii.

We found earlier that deferoxamine (DFO), a drug used for treatment of iron overload, is active against a rat model of Pneumocystis carinii pneumonia (PCP). We had assumed a mode of action by deprivation of nutritional iron; however, data here show that DFO penetrates P. carinii, causing irreversible damage, thus indicating a different mode of action. Penetration was demonstrated by showing DFO uptake by high-pressure liquid chromatography analysis. By using calcein-AM as an indicator, exposure to DFO was shown to cause a reduction in P. carinii cytoplasmic free iron. Exposure to >or=100 micro…

Diagnostic strategies in Pneumocystis carinii pneumonia.

Pneumocystis carinii (P. carinii) remains a major pulmonary pathogen for the immunocompromised patient. In HIV infected patients, P. carinii represents the most commonly diagnosed cause of pneumonia. In the AIDS patient, empiric therapy based on clinical presentation has its proponents. However, this approach has been associated with a worse overall prognosis for the at risk patient. Because P. carinii can not be cultured, specific identification relies on examining respiratory specimens ranging from expectorated sputum to bronchoscopy with bronchoalveolar lavage (BAL). The low sensitivity of …

Pneumocystis carinii pneumonia: pitfalls of prophylaxis.

Pneumocystis carinii pneumonia (PCP) occurs commonly in immunocompromised patients. Sulfamethoxazole-trimethoprim (SMX-TMP) is effective prophylaxis, although PCP may still occur despite apparently adequate use. We report three cases of PCP which highlight some of the pitfalls of prophylaxis.

Pneumocystis carinii interactions with respiratory epithelium.

Ultrastructural studies indicate that the attachment of Pneumocystis carinii to alveolar epithelial cells is a central feature during development of this pneumonia. P. carinii attaches preferentially to type I alveolar epithelial cells, and rarely binds to type II cells. Extensive in vitro investigations support roles for both fibronectin and vitronectin, which bind to the organism, and mediate P. carinii attachment to respiratory epithelial cells through corresponding integrin receptors. Additional studies indicate that P. carinii cytoskeletal rearrangement occurs after initial binding of the…

Pneumocystis carinii pneumonia masquerading as tuberculosis.

Recent laboratory studies indicate that genetic diversity exists in human strains of Pneumocystis carinii. Structural and functional variability in infecting strains could result in differences in host-parasite interactions and the natural history of P carinii pneumonia. We report 5 unusual cases in which the clinical presentation mimicked tuberculosis. All patients were cared for at a university-based public hospital clinic in Los Angeles, Calif, during a 2-year period. These patients were chronically ill, had lost weight, and each had cavities or cystic spaces as the primary radiographic fin…

Diagnostic strategies for Pneumocystis carinii pneumonia.

Pneumocystis carinii is an opportunistic organism that is a common cause of pneumonia in immunocompromised patients. Its life cycle begins when cysts rupture and release sporozoites, which mature into trophozoites that eventually form cysts. The diagnostic methods for P. carinii pneumonia (PCP) have progressed from open lung biopsy to bronchoalveolar lavage (BAL) and induced sputum analysis (ISA). Detection of P. carinii organisms is done with various stains that highlight sporozoites, trophozoites, or the cyst wall. Noninvasive, cost-effective methods to aid in the diagnosis of PCP have been …

Granulomatous Pneumocystis carinii in AIDS patients.

Seven cases of Pneumocystis carinii pneumonia with granulomatous reaction in patients infected with the human immunodeficiency virus are described. The patients were all adult men between the ages of 32 and 45 years, with different high-risk factors. Clinically, all the patients presented with a history of non-productive cough and shortness of breath. Two of the patients had a past history of pulmonary pneumocystosis. Radiologically, six patients had diffuse pulmonary infiltrates and one nodular pulmonary infiltrate. Transbronchial lung biopsies were obtained in four patients and open lung bio…

Successful chemoprophylaxis for Pneumocystis carinii pneumonitis.

In a randomized, double-blind, placebocontrolled study to evaluate the efficacy of trimethoprim-sulfamethoxazole for the prevention of Pneumocystis carinii pneumonia, we studied 160 patients with cancer who were at high risk for this pneumonia over a two-year period. Seventeen of the 80 patients receiving a placebo acquired P. carinii pneumonitis, whereas none of the 80 given 150 mg of trimethoprim and 750 mg of sulfamethoxazole per square meter per day had the infection P less than 0.01). Bacterial sepsis, pneumonia other than that caused by P. carinii, acute otitis media, upper-respiratory-t…

Clinical performance of the fully automated quantitative LeaDEA Pneumocystis PCR Kit for the diagnosis of Pneumocystis jirovecii pneumonia.

Compared with microscopy, quantitative real-time PCR (qPCR) is more sensitive for the diagnosis of Pneumocystis jirovecii pneumonia (PcP) and may also help distinguish PcP from colonization based on fungal burden. In this retrospective cohort study, we evaluated the performance of the LeaDEA Pneumocystis PCR Kit, a single-packaged reagent system designed for use on a fully automated molecular testing platform, using respiratory specimens from immunocompromised patients. The analytical and clinical performance of the LeaDEA assay on the GeneLEAD VIII platform was compared with that of a previou…
