PURPOSE: The purpose of the study was to investigate clinical outcomes following a mini-open posterior technique.
METHODS: Patients who received mini-open popliteal cyst excisions between April 1999 and April 2010 were identified. Charts were retrospectively reviewed for postoperative complications, cyst recurrence, previous aspiration, re-operation, intraoperative findings, cyst size, comorbidities, and co-surgeries. Visual Analogue Pain Scale and Rauschning's symptomatic knee criteria were collected prospectively to assess functional outcomes.
RESULTS: Twenty-two legs in 21 patients were i

In the treatment of patients with popliteal cysts (Baker's cysts), we generally need to address the intra-articular pathologies, most commonly, medial meniscal tears and concomitant connecting valvular mechanisms responsible for the formation and recurrence of the cyst. We introduce here an arthroscopic technique that can treat the associated intra-articular pathology and correct the valvular mechanism of the capsular fold. The most important step is to locate the opening connection between the joint cavity and the popliteal cyst at the posteromedial compartment. The opening is found at the po

Popliteal cysts which rupture to mimic a deep vein thrombosis (DTV) can be seen in rheumatoid arthritis and other inflammatory arthritides. Gout with cystal synovitis and ruptured popliteal cyst has only been sporadically reported and nowhere has this occurred more than once.

A patient was admitted with a clinical suspicion of left sural thrombophlebitis. Analysis of the clinical features and the results of rheoplethysmography, phlebography, arteriography and, most importantly, ultrasonography led to the diagnosis of popliteal synovial cyst and demonstrated the vascular repercussions of this lesions responsible for the swollen leg. A review of the literature confirms the frequently misleading clinical features of popliteal cysts. The authors present a strategy for the investigation of these cases, based on vascular functional investigations and in particular ultras

Forty-one knee joints presenting with significant popliteal cysts were examined clinically and arthrographically. The fluid was aspirated from the cysts and replaced by contrast medium of low viscosity. On knee flexion the contrast fluid spontaneously passed over into the joint in 16 cases (no valve mechanism), in 7 small amounts of fluid were forced into the joint when the cysts were manually compressed, and in 18 no fluid passage to the joint was observed on manipulation (valve mechanism). When a valvular action was established, effusion in the joint was rare and the fluid from the cyst was

Report on 38 Baker cysts found in 300 knee arthrographies performed for clinically suspected meniscus lesions. In order not to miss these cysts, fluoroscopy and/or X-ray documentation immediately following the instillation of contrast material and air into the knee joint is mandatory, because these contrast agents tend to disappear fairly rapidly, leaving the lesion poorly visible towards the end of the examination. Furthermore, the clinical significance and the therapeutic aspects of Baker cysts are discussed.

We report a case of knee pyarthrosis in a 54-year-old woman with rheumatoid arthritis and a popliteal cyst. The onset of infection coincided with a cortisone injection. Initial management consisted of arthroscopic irrigation and debridement (I&D) on 2 consecutive occasions without resolution of the infection. Only after open excision of the popliteal cyst in conjunction with I&D of the knee joint proper did the infection resolve. This is the first reported case of a patient requiring excision of a popliteal cyst to clear pyarthrosis of the knee after failure of arthroscopic I&D. Consideration

Despite increasing knowledge of etiopathogenesis, therapy, and recurrence rate of popliteal cysts, they nevertheless occasionally represent clinical problems. We report the case of a 58-year-old rheumatoid arthritis patient in whom a giant recurrent cyst developed very shortly after primary excision. Reports of such large popliteal cysts are rare and very few cases were reported in rheumatoid arthritis patients. Moreover, no such giant recurrent cysts formed so shortly after primary excision. Thus, its occurrence may be partially ascribed to the specific dynamics of fluid flows caused by the a

Popliteal cysts in children differ from those in adults. They are considered to be less frequent and usually appear in the absence of intra-articular lesions. However, their prevalence in asymptomatic children is unknown. We present a prospective epidemiological study of 168 asymptomatic children under 15 years of age hospitalised for various pathologies. Children with knee problems and rheumatic diseases were excluded from the study. The children were examined ultrasonographically with a 7.5 MHz linear scanner. A popliteal cyst could be identified in 4 patients. The prevalence of asymptomatic

The case of an 11-year-old girl presenting with a symptomatic popliteal cyst is described. There was no previous knee trauma nor could any inflammation be found. On MRI a concomitant retropatellar cartilage lesion had been detected preoperatively and was confirmed and treated by arthroscopy. The prevalence and treatment of popliteal cysts in children are discussed. Whereas in adults popliteal cysts are generally accompanied by intra-articular lesions, the presence of a concomitant intraarticular lesion in children is an extremely rare finding. We conclude that intraarticular lesions in the pre

Positive correlations obtained between the presence of anterior femoral erosion, enlargement of the gastrocnemius-semimembranous bursa and osteoarthritis in the knee suggests that these are all manifestations of a degenerative process developing with increasing age. Popliteal cysts are considered to act as a decompressing factor for raised intraarticular knee pressure. The association of popliteal cysts with femoral erosions suggests that both conditions may be attributed to similar mechanisms, namely, hydrarthrosis and raised joint pressure secondary to degenerative articular disease. The dou

Grey-scale ultrasonography will detect reliably the presence of clinically significant popliteal cysts, fluid collections which do not fill by arthrography, and will frequently demonstrate ruptured cysts and the soft tissue changes resulting from a recent leak. Forty-eight knees, in 25 patients with popliteal and/or calf pain were examined by ultrasonography followed by arthrography. Popliteal cysts were demonstrated in 40% (19/48) by ultrasound and in 46% (22/48) by arthrogram. For comparison between arthrography and ultrasonography chi2 = 8.58 and contingency coefficient, phi = 0.42 (p less

Ultrasound scanning techniques detected popliteal cysts in 14 of 24 knees affected with rheumatoid arthritis and associated with an anterior effusion. Serial scans demonstrated the persistence of cysts when effusion was not controlled by intraarticular steroid and lidocaine, and regression of cyst following control of effusion with anterior synovectomy. As the technique is noninvasive, painless, and reproducible, ultrasound scanning should be the technique of choice for the detection and assessment of popliteal cysts.

Popliteal cysts are common lesions, primarily characterized by soft tissue swelling, which may occasionally present with calcifications or small osteochondromas, and can rarely lead to restricted movement. We report a case involving a spherical coral-like synovial chondroma located within a popliteal cyst, which caused significant pain and limited the flexion and extension of the knee joint. Notably, the appearance of the synovial chondroma was strikingly similar to that of spherical coral, and its distinctive imaging features are previously unreported in the literature. Following the excision

ABSTRACT: This study describes an arthroscopic technique for popliteal cyst excision and reports preliminary outcomes data. We retrospectively identified patients with symptomatic popliteal cysts who underwent popliteal cyst excision at our institution between 2013 and 2020. Arthroscopic popliteal cyst excision was performed using dual posteromedial portals, expansion of the valvular mechanism, and cyst wall resection. International Knee Documentation Committee (IKDC) scores were collected preoperatively and at follow-up. We calculated the proportions of patients meeting Patient Acceptable Sym
