Breast Cancer Incidence Rates in Ghanaian and US Black Women From 2013 Through 2015.

IMPORTANCE: Uncertainty exists about whether women of West African ancestry have increased rates of estrogen receptor (ER)-negative breast tumors, which commonly have earlier age onsets than ER-positive tumors and are aggressive. High-quality population-based studies in Africa with tumor pathology data are generally unavailable, and there is speculation that younger population age structures may explain the increased frequencies of reported ER-negative tumors. OBJECTIVE: To estimate age-standardized and age-specific rates of ER-negative breast cancer among Ghanaian women and non-Hispanic Blac

Age-Dependent Assortativeness in Herpes Simplex Virus Type 1 Oral Transmission in the United States: A Mathematical Modeling Analysis.

BACKGROUND: Herpes simplex virus type 1 (HSV-1) is a highly infectious, globally prevalent lifelong infection. Despite advancements in understanding its epidemiology, the assortativeness in the age-dependent transmission patterns remains unclear. This study aimed to estimate the degree of assortativeness in age group mixing for oral-to-oral HSV-1 transmission within the United States (US) population. METHODS: An age-structured mathematical model was employed to describe HSV-1 transmission dynamics in the US population, incorporating its different modes of transmission. The model was fitted to

Differing time trends for alcohol misuse and alcohol use disorder among Veteran men: A comparison of population surveillance and clinical data.

INTRODUCTION: Data on time trends are important to track the health burden of alcohol use disorder (AUD) and plan for alcohol-related healthcare needs. Our aim was to estimate time trends in AUD for male VA patients and to assess whether trends in documented diagnosis rates were similar to trends in unhealthy alcohol use measured through clinical screening or trends in AUD prevalence among US veterans in the general population. MATERIALS AND METHODS: We used VA electronic health record data (10/1/2009-9/30/2019) to measure AUD diagnosis rates and clinical alcohol screening data to measure unh

The unintended health effects of US COVID-19 lockdowns: a systematic review.

INTRODUCTION: US lockdowns and school closures implemented during the COVID-19 pandemic were intended to mitigate viral transmission and protect public health. However, the broader health effects of these interventions remain unclear. METHODS: We conducted a systematic review of peer-reviewed studies that assessed the impact of US lockdowns and school closures on health-related outcomes excluding COVID-19 transmission and mortality. RESULTS: A total of 132 studies met inclusion criteria, yielding 454 unique outcomes. Lockdowns and school closures were associated with detrimental health effec

Diurnal variation in serum alanine aminotransferase activity in the US population.

GOALS AND BACKGROUND: Serum alanine aminotransferase (ALT) activity has been reported to be greater in the afternoon than the early morning, but data are scarce. We examined diurnal variation of ALT in a national population-based sample. STUDY: Participants in the 1999 to 2008 US National Health and Nutrition Examination Survey were randomly assigned to morning (AM; n = 4474 adolescents, 11,235 adults) or afternoon/evening (PM; n = 4887 adolescents, 11,735 adults) examinations. We examined ALT distributions graphically and compared both geometric mean ALT and the prevalence of elevated ALT, d

The role of elastography in the assessment of thyroid nodules in acromegaly.

BACKGROUND: Nodular thyroid disease is a frequent finding seen in patients with acromegaly. Ultrasound-elastography (US-E) appears to be a helpful tool for the diagnosis of thyroid cancer. The aims of this study were to evaluate thyroid nodules in acromegaly and to assess the diagnostic accuracy of US-E in detecting thyroid cancer in this population. METHODS: US-E was applied to 166 nodules detected in 102 acromegalic patients and to 105 nodules found in 95 nonacromegalic subjects. The lesions were classified according to the elasticity scores (ES) as soft (ES 1-2) or hard (ES 3-4). RESULTS:

Potential impact of newborn screening for cystic fibrosis on child survival: a systematic review and analysis.

OBJECTIVE: To estimate the population impact of child mortality as a result of cystic fibrosis (CF) potentially preventable by newborn screening. STUDY DESIGN: A systematic literature review of mortality in children with classic CF without meconium ileus (MI) in screened and unscreened cohorts was extended by contacting investigators for unpublished data. In addition, survival in US states with and without newborn screening (NBS) programs for CF was compared using data from the Cystic Fibrosis Foundation Patient Registry (CFFPR). RESULTS: Among non-US studies, CF-related mortality risk to ap

Diseases Common in Persons With Cystic Fibrosis Among CFTR Heterozygotes.

IMPORTANCE: Cystic fibrosis is one of the most commonly diagnosed autosomal recessive disorders in the US. It is estimated that more than 10 million individuals are heterozygous for a pathogenic CFTR gene variant in the US (heterozygotes). The phenotypic risk of these heterozygotes is not well defined, particularly among populations of predominantly non-European genetic ancestry. Understanding disease risk across each population can improve management strategies for all. OBJECTIVE: To examine associations of diseases across the phenome with CFTR heterozygotes. DESIGN, SETTING, AND PARTICIPAN

Trends in Female Breast Cancer Incidence among Japanese, Korean, and US Populations: An Age-Period-Cohort Analysis.

BACKGROUND: Breast cancer incidence has historically been higher in Western countries; however, rates have increased substantially in East Asia, with variation by age and generation. Direct comparisons between native Asians and their US counterparts remain limited. We examined the effects of age-period-cohort (APC) on breast cancer incidence among native Japanese women, with comparisons with Japanese Americans, native Korean women, Korean Americans, and US White and Black women. METHODS: We analyzed population-based cancer registry data from Japan (1985-2019), Korea (1993-2017), Hawaii (1988-

Heterogeneous associations of socioeconomic status with metabolic disease in racial and ethnic subgroups in the United States: A cross-sectional cohort study in NHANES and All Of Us.

BACKGROUND: Unfavorable socioeconomic status (SES) is associated with adverse health outcomes and is believed to at least partially mediate racial and ethnic health disparities such that some clinical risk algorithms now incorporate SES measures. However, whether the associations of improved SES with improved health are uniform across US racial and ethnic subpopulations is unknown. METHODS AND FINDINGS: Among adult participants in the National Health and Nutrition Examination Survey 1999-2018 and the All of Us cohort v7, we used logistic regression to examine the association between SES measu

Mercury exposure and risks from dental amalgam in the US population, post-2000.

Dental amalgam is 50% metallic mercury (Hg) by weight and Hg vapour continuously evolves from in-place dental amalgam, causing increased Hg content with increasing amalgam load in urine, faeces, exhaled breath, saliva, blood, and various organs and tissues including the kidney, pituitary gland, liver, and brain. The Hg content also increases with maternal amalgam load in amniotic fluid, placenta, cord blood, meconium, various foetal tissues including liver, kidney and brain, in colostrum and breast milk. Based on 2001 to 2004 population statistics, 181.1 million Americans carry a grand total o

Performance of the 2015 US Preventive Services Task Force Screening Criteria for Prediabetes and Undiagnosed Diabetes.

BACKGROUND: In 2015, The US Preventive Services Task Force (USPSTF) recommended screening for prediabetes and undiagnosed diabetes (collectively called dysglycemia) among adults aged 40-70 years with overweight or obesity. The recommendation suggests that clinicians consider screening earlier in people who have other diabetes risk factors. OBJECTIVE: To compare the performance of limited and expanded screening criteria recommended by the USPSTF for detecting dysglycemia among US adults. DESIGN: Cross-sectional analysis of survey and laboratory data collected from nationally representative sa
