Risk factors for portal vein system thrombosis after partial splenic embolisation in cirrhotic patients with hypersplenism.

AIM: To determine risk factors for portal venous system thrombosis (PVST) after partial splenic artery embolisation (PSAE) in cirrhotic patients with hypersplenism. MATERIALS AND METHODS: Between March 2014 and February 2022, 428 cirrhotic patients with hypersplenism underwent partial splenic artery embolisation and from these patients 208 were enrolled and 220 were excluded. Medical records of enrolled patients were collected. Computed tomography (CT) images were reviewed by two blinded, independent radiologists. Statistical analyses were performed by using SPSS. RESULTS: Progressive PVST w

Are hepatic portal venous system components distributed equally in the liver? A multidetector computerized tomography study.

OBJECTIVE: We aimed to evaluate the relationships between the splenic index, right and left hepatic lobe volumes, diameters of splenic vein (SV), superior mesenteric vein (SMV) and the portal vein (PV) by Multidetector Computerized Tomography (MDCT). We also investigated indirect signs of portal venous flow pattern using these parameters. MATERIAL AND METHODS: Following their contrast thoracoabdominal and abdominal 64-MDCT examinations, the images of 100 cases (61 males and 39 females) were evaluated retrospectively. For each case, the splenic index, total hepatic volume, left and right hepat

Abnormalities of the umbilico-portal venous system in Down syndrome: a report of two new patients.

Congenital anomalies of the umbilical and portal venous system are rare vascular malformations which are often associated with anomalies of the heart and gastrointestinal tract. Association with chromosomal disorders has been sporadically reported. We now report on two patients with trisomy 21 and congenital anomalies of the umbilico-portal system. A male fetus showed absence of the intrahepatic portal vein (PV) and ductus venosus with a direct communication between portal sinus and inferior vena cava exhibiting an umbilicosystemic total shunt during the fetal life and a portosystemic total sh…

Abnormalities of the fetal central veins and umbilico-portal system: prenatal ultrasonographic diagnosis and proposed classification.

OBJECTIVES: Anomalies of the fetal venous system are poorly documented and their pathogenesis is not well understood. The present study was undertaken to review the spectrum of fetal central veins and umbilico-portal system anomalies, and to propose a classification system. METHODS: A 7-year restrospective survey was conducted. RESULTS: Nineteen fetuses showed abnormal connection between central veins and the fetal heart. Three fetuses showed abnormal connections of the cardinal veins, two of which had interruption of the inferior vena cava, and one had isolated persistent left superior vena…

Portal system thrombosis after splenectomy for neoplasm or chronic hematologic disorder: is routine surveillance imaging necessary?

PURPOSE: This study was undertaken to assess the frequency and sequelae of portal system thrombosis (PST) after splenectomy in patients with cancer or chronic hematologic disorders and to determine whether routine surveillance imaging for this potential complication is warranted. METHOD: The radiology reports of 203 consecutive patients with cancer or chronic hematologic disorders who underwent splenectomy between January 1990 and January 1997 were reviewed. Imaging examinations and medical records were reviewed for those in whom PST was found after splenectomy. RESULTS: One hundred twenty-t…

Imaging of the portal venous system in patients with cirrhosis: MR angiography vs duplex Doppler sonography.

OBJECTIVE: To determine the relative performances of MR angiography and duplex Doppler sonography in the evaluation of the portal venous system in patients with portal hypertension, we compared two-dimensional time-of-flight MR angiography and duplex Doppler sonography in 60 sequential patients with cirrhosis. All 60 subsequently underwent liver transplantation. SUBJECTS AND METHODS: MR imaging and duplex Doppler sonography were performed on the same day, and in all patients the imaging findings were validated at surgical exploration. Five transplantation candidates had surgical portosystemic

An anterograde tracing study of the vagal innervation of rat liver, portal vein and biliary system.

In order to investigate the distribution and structure of the vagal liver innervation, abdominal vagal afferents and efferents were selectively labeled by injecting WGA-HRP or Dil into the nodose ganglia, and DiA into the dorsal motor nucleus, respectively. Vagal afferent fibers produced characteristic terminal-like structures at three locations in the liver hilus: 1. Fine varicose endings preferentially surrounding, but not entering, the numerous peribiliary glands in the larger intra and extrahepatic bile ducts 2. Large, cup-shaped terminals in almost all paraganglia 3. Fine varicose endings

[Early detection of hepatobiliary involvement in children with mucoviscidosis using duplex sonography of the portal system].

To investigate early signs of hepatobiliary disease in CF, we measured portal, splenic, superior mesenteric vein and hepatic artery diameter … ortal vein flow showed no relevant differences in CF-patients versus controls. It is concluded that portal system abnormalities, especially a decrease in Vmax and TAV of portal vein shown by duplex sonography, may be earlier indicators of CF liver disease than biochemical and clinical signs.

Anatomical variations in the tributaries of the normal feline extrahepatic portal system: evaluation with CT angiography.

ObjectivesIt is often suggested in the literature that the anatomy of the extrahepatic portal vein (EHPV) in dogs and cats is similar. Nevertheless, variations and contradictions in the tributaries of the EHPV in cats have been described. Therefore, the aim of this study was to describe the normal anatomical variations of the tributaries of the EHPV in a large cohort of cats.MethodsA retrospective, descriptive, cross-sectional study of abdominal CT angiography (CTA) scans was performed. Cats that underwent CTA between January 2020 and July 2024 were reviewed in consensus by three observers. Ca

Partial splenic embolization treats recurrent left pleural effusions in a patient with portal venous system thrombosis.

A 56-year-old female with thrombocythemia complicated by portal venous system thrombosis presented with recurrent left pleural effusions after failed recanalization via mechanical thrombectomy and stenting at an outside center. With no other cause, splenic vein thrombosis and left-sided portal hypertension was suggested as a possible etiology. Partial splenic embolization was performed with immediate decrease in effusions and resolution by 8 weeks. Portal and splenic venous system thrombosis may cause recurrent pleural effusions from left-sided portal hypertension and fluid leakage across diap

Schistosoma mansoni: acquired resistance in mice by implantation of young irradiated worms into the portal system.

In two distinct experiments, immature S. mansoni worms (LE strain, Belo Horizonte, Brazil), aged 20 days, obtained from the portal system of white outbred mice, were irradiated with 14 and 4 Krad, respectively. Afterwards, the worms were directly inoculated into the portal vein of normal mice. Inoculation was performed with 20 irradiated worms per animal. Fifty days after inoculation, the mice that received 4 and 14 Krad-irradiated worms and their respective controls were infected with S. mansoni cercariae (LE strain), by transcutaneous route. Twenty days after this challenge infection, the an…

A 0D-3D multi-scale model of the portal venous system coupled with the entire cardiovascular system applied to predict postsplenectomy hemodynamic metrics.

Splenectomy is a common surgery for portal hypertensive patients with splenomegaly. Although splenectomy is able to effectively relieve the complications of portal hypertension, it also increases the risk of portal venous system thrombosis remarkably. Previous studies demonstrated that the hemodynamic metrics of the portal venous system could be employed in predicting the risk of postsplenectomy thrombosis, and 3D models were utilized to simulate the blood flow in the portal venous system. Aiming to reflect the global effect of splenectomy and better simulate the hemodynamic metrics, in this s
