Left paratracheal pressure versus cricoid pressure for successful laryngeal mask airway insertion in adult patients: a randomized, non-inferiority trial.

BACKGROUND: Cricoid pressure (CP) is used to prevent pulmonary aspiration of regurgitated gastric contents and gastric insufflation during positive-pressure ventilation. However, CP impedes the successful insertion of laryngeal mask airway (LMA). Left paratracheal pressure (LPP), a maneuver of applying backward digital force at the lower left paratracheal level, was recently introduced as an alternative to CP. We assessed whether LPP is non-inferior to CP in successful LMA insertion on the first attempt in adult patients undergoing general anesthesia. METHODS: In this non-inferiority randomiz

Epidemiology, ventilation management and outcome in patients receiving intensive care after non-thoracic surgery - Insights from the LAS VEGAS study.

INTRODUCTION AND OBJECTIVES: Information about epidemiology, ventilation management and outcome in postoperative intensive care unit (ICU) patients remains scarce. The objective was to test whether postoperative ventilation differs from that in the operation room. MATERIAL AND METHODS: This was a substudy of the worldwide observational LAS VEGAS study, including patients undergoing non-thoracic surgeries. Of 146 study sites participating in the LAS VEGAS study, 117 (80%) sites reported on the postoperative ICU course, including ventilation and complications. The coprimary outcomes were two ke

[Sleep apnea syndrome and heart failure].

More than half of all patients with stable heart failure are affected by sleep apnea syndrome. The mechanism of apnea may be either obstructive, by collapse of the upper airways, or central with nocturnal Cheyne-Stokes respiration. Changes in intrathoracic pressure, awakening and hypoxemia all lead to an increase in left ventricular afterload and to sympathetic activation, which can aggravate heart failure and prognosis. Treatment with positive pressure ventilation improves symptoms as well as cardiac function and prognosis. Sleep respiratory disorders are underdiagnosed and undertreated in pa

Synchronized mechanical ventilation for respiratory support in newborn infants.

BACKGROUND: During synchronized mechanical ventilation, positive airway pressure and spontaneous inspiration coincide. Thus, if synchronous ventilation is provoked, adequate gas exchange should be achieved at lower peak airway pressures, potentially reducing barotrauma and hence airleak and chronic lung disease. Synchronous ventilation can be achieved by manipulation of rate and inspiratory time during conventional ventilation and employment of patient assisted ventilation. OBJECTIVES: To compare (i) the efficacy of synchronized mechanical ventilation, delivered as high frequency positive pre

[Cardiopulmonary effects of CPPV (continuous positive pressure ventilation) and IRV (inverse ratio ventilation) in experimental myocardial ischemia].

Continuous positive pressure ventilation (CPPV) is an established therapy for treatment of acute respiratory failure (ARF). However, cardiac performance may be severely disturbed due to elevated intrathoracic pressure, inducing a decrease in cardiac output (CO) and oxygen delivery (DO2). Alternatively, mechanical ventilation with prolonged inspiratory to expiratory duration ratio (inversed ratio ventilation IRV) has been successfully used in ARF. No data are available about IRV in acute haemodynamic oedema. Thus, the cardiopulmonary effects of CPPV (positive end-expiratory pressure [PEEP] = 10

Adult respiratory distress syndrome: improved oxygenation during high-frequency jet ventilation/continuous positive airway pressure.

The role of high-frequency jet ventilation (HFJV)/continuous positive airway pressure (CPAP) and HFJV/intermittent mandatory ventilation (IMV) in the treatment of surgical patients with the adult respiratory distress syndrome were evaluated. To compare the efficacy of HFJV to IMV at a constant FiO2 and positive end-expiratory pressure, patients in surgical intensive care were randomized to receive IMV/CPAP therapy or one of three modes of HFJV: (1) HFJV/CPAP alone, (2) HFJV/CPAP + IMV (1), or (3) HFJV/CPAP + IMV (2). Each patient served as his own control. During comparison of HFJV/CPAP + IMV

Prevention of gastric inflation during mask ventilation in newborn infants.

Ten premature infants who had recovered from the respiratory distress syndrome were subjected to cricoid compression during mask ventilation. Each infant was ventilated with an AMBU face mask for 3 min with the nasogastric (NG) tube, without the NG tube, and without the NG tube but applying cricoid pressure during ventilation. The results of the study indicated that, similar to insertion of an NG tube, cricoid compression can prevent gastric inflation during mask ventilation.

"Tailored" vs. "one-size-fits-all": current knowledge of individualized PEEP and constant PEEP strategy during the perioperative period.

Postoperative pulmonary complications (PPCs) are dramatically associated with prolonged hospitalization and increased mortality rates in surgical patients. Positive end-expiratory pressure (PEEP), a standardized lung-protective strategy, has been proven to effectively improve perioperative oxygenation and pulmonary compliance. However, constant high PEEP may induce alveolar hyperventilation and elevate pulmonary vascular resistance, while fixed low PEEP may not prevent atelectasis and even cause PPCs. Currently, an individualized PEEP (iPEEP) approach is found to be superior to the constant PE

Noninvasive ventilation for respiratory failure: When does it work and when it does not?

Noninvasive positive pressure ventilation (NIV) is a positive pressure ventilation method employed across various disease processes, utilizing noninvasive interfaces such as helmets and facemasks rather than invasive methods such as endotracheal intubation. The benefits of NIV are significant in both the acute care setting, such as improving work of breathing and avoiding the need for endotracheal intubation, as well as in the chronic care setting, improving quality of life and mortality. While new guidelines broaden the application of NIV across various disease areas and introduce emerging mo

[Experimental vocal cord abduction impairment with an artificial vocal cord].

Non-invasive positive pressure ventilation (NPPV) has recently been applied to the patients with multiple system atrophy (MSA) with various respiratory complications including vocal cord abduction impairment and respiratory disturbance by the central origin. Any consensus guidelines on setting up the inspiratory positive airway pressure (IPAP) and expiratory one (EPAP), however, have not been raised yet. To investigate this problem, we made the upper airway tract model with moderately and severely narrow glottis using a training/test lung and the artificial vocal cord which was developed for a

[Ventilatory assist therapy].

NPPV has been shown to improve arterial blood gas values, prevent symptoms resulting from alveolar hypoventilation, and decrease hospitalization in patients with chronic respiratory failure with COPD. Regarding acute exacerbation of COPD, the utility of NPPV has been established so NPPV will be regarded first-line therapy as ventilatory assist therapy. On the other hand, with respect to chronic stable COPD, the utility of NPPV isn' t established yet. However NPPV cases have been increasing rapidly in these years, as new home respiratory care technique. From now on, we have to evaluate the obje

Noninvasive positive pressure ventilation in acute respiratory failure: does it improve outcomes?

Studies have shown that noninvasive positive pressure ventilation (NPPV) is well tolerated and safe, and that it improves oxygenation in some patients with acute respiratory failure. By obviating the need for endotracheal intubation in certain conditions, it results in fewer complications, shorter hospital stays, and consequently, lower mortality rates and costs of care.
