The paper reports a retrospective study performed between January 1996 and January 1999 which comprised 423 patients who underwent eye cataract surgery. Thirty-seven children aged two to ten years old sustained unilateral traumatic cataract. Aspiration of the traumatic lens was followed by intraocular lens implantation in these 37 eyes. METHODS: Iris retractors or a Malyugin Ring (Microsurgical Technology, Redmond, WA) were used for mechanical pupil dilation before laser capsulotomy and lens fragmentation. The laser treatment can be performed with or without use of ophthalmic viscosurgical devices (OVD).

Cataracta secundaria ("Elschnig pearls") following extracapsular cataract extraction with IOL implantation can be treated by Neodymium-YAG laser capsulotomy or by surgical peeling. The major benefit of the peeling technique is the preservation of an intact posterior capsule. We retrospectively reviewed the peeling procedures (n = 224, mean age: 67 years) performed in our hospital from 1987 to 1990. Cataracts are well known to be associated with myotonic dystrophy. Less well known are the phenomena of recurrent posterior capsule opacification and capsulorhexis contracture post cataract surgery. Two cases are described herein of postoperative capsular complications requiring multiple capsulotomies in patients with myotonic dystrophy. It is proposed that a common aetiology may underlie both posterior capsule opacification and capsulorhexis contracture in myotonic dystrophy cases.

Surgical technique and results of surgical correction for substantial flexion contracture of the knee in 15 children with spina bifida or other spinal conditions with lower extremities paresis is presented. Extensive posterior capsulotomy, occasionally supplemented with posterior cruciate sectioning is necessary to correct considerable flexion contracture of the knee. The technique is also effective in congenital flexion contracture with no articular surfaces deformity. Serial redressing and casting are imperative in postoperative management.

Fifty cases of clubfeet (38 children) were evaluated 10 years after surgery. Turco's method was used in 22 cases, while 24 were managed with Achilles tendon lengthening and dorsal capsulotomy. Five patients developed retinal detachments within one month of undergoing neodymium-YAG laser posterior capsulotomy. In four patients rhegmatogenous retinal detachments developed in association with typical aphakic breaks; in the fifth patient a previously stable extramacular traction detachment extended into the fovea.
