Recent advances in our understanding of the anatomy and physiology of accommodation have contributed to current concepts of the possible constituent factors in presbyopia, largely supplanting the long-held belief that presbyopia is due to sclerosis of the crystalline lens. In this review, the author examines epidemiologic, basic scientific, and clinical evidence for a multifactorial decrease in accommodative amplitude with age. Methods of measurement, oculomotor effects of presbyopia, and approaches to correcting it are also considered.

OBJECTIVE: Presbyopia, an age-related ocular disorder, is characterized by the loss in the accommodative abilities of the human eye. Conventional methods of correcting presbyopia divide the field of view, thereby resulting in significant vision impairment. PURPOSE: To predict the potential clinical visual acuity of different presbyopia treatments.
METHODS: The defocus transfer function (DTF) is a mathematical technique for simultaneously illustrating the optical transfer function for all levels of defocus. Through-focus clinical visual acuity measurements were superimposed onto the DTF to determine the corresponding retinal contrast and map out a modulation threshold curve. The process is performed for an apodized diffractive and a zonal refractive intraocular lens, as well as an opaque annular ring implant that extends depth of focus.

PURPOSE: To assess the safety, efficacy, and stability of conductive keratoplasty (CK) in the treatment of presbyopia.
METHODS: Ten near plano presbyopic patients (6 women and 4 men) underwent unilateral CK in the non-dominant eye to improve their near vision. Mean age was 51 +/- 3.1 years (range: 46 to 56 years). The surgeries were the author's first 10 CK procedures performed. The postoperative target for these eyes ranged from -1.25 to -1.75 diopters (D).

Forced vergence fixation disparity curves were measured at distance and near on an asymptomatic presbyopic adult population (average age 62.7 +/- 7.0 years, range 51 to 79). Approximately 42% of the 62-subject sample had the same curve types at distance and near (type I-38.4%; type II-3.5%). Different curve types were found in 58.1% of the sample.
