Pharmacological Treatment of Alcohol use Disorder in Patients with Psychotic Disorders: A Systematic Review.

BACKGROUND: Patients with psychotic disorders (PD) often have comorbid alcohol use disorder (AUD), which is typically treated pharmacologically. Up till now, no systematic review has examined the effectiveness and safety of AUD treatment in PD patients. OBJECTIVES: This study aimed to systematically review the literature on (1) the effects of pharmacological treatments for AUD on drinking outcomes, (2) the side effects of the drugs, and (3) the effects of polypharmacy in patients with comorbid AUD and PD. METHODS: Bibliographic searches were conducted in MEDLINE, Embase, Cochrane Central Reg

Comorbid Diagnosis of Psychotic Disorders in Borderline Personality Disorder: Prevalence and Influence on Outcome.

BACKGROUND: A diagnosis of psychotic disorder is traditionally considered incompatible with borderline personality disorder (BPD), even though patients sometimes fulfill the diagnostic criteria for both disorders. How often this happens is barely known, as is the influence of comorbid psychotic disorders on the outcome of BPD. Since studies on isolated auditory verbal hallucinations in patients with BPD indicate that these perceptual symptoms have severe consequences and are associated with suicidal behavior and hospitalization, patients with comorbid psychotic disorders are unlikely to fare b

The prevalence and correlates of self-harm in pregnant women with psychotic disorder and bipolar disorder.

Women with severe mental illness are at increased risk of suicide in the perinatal period, and these suicides are often preceded by self-harm, but little is known about self-harm and its correlates in this population. This study aimed to investigate the prevalence of suicidal ideation and self-harm, and its correlates, in women with psychotic disorders and bipolar disorder during pregnancy. Historical cohort study using de-identified secondary mental healthcare records linked with national maternity data. Women pregnant from 2007 to 2011, with ICD-10 diagnoses of schizophrenia and related diso

The misdiagnosis of bipolar disorder as a psychotic disorder: some of its causes and their influence on therapy.

OBJECTIVES: Looking at chart records bipolar disorder is often misdiagnosed as a psychotic disorder but no study has ever systematically looked into the reasons. One reason for misdiagnoses could be that clinicians use heuristics like the prototype approach in routine practice instead of strictly adhering to the diagnostic criteria. Using an experimental approach we investigated if the use of heuristics can explain when a diagnosis of psychotic disorder is given instead of bipolar disorder. We systematically varied information about the presence or absence of specific symptoms, i.e. hallucinat

Associations of past conduct disorder with personality disorders in 'non-psychotic' psychiatric inpatients.

The aim was to investigate associations of a history of features of DSM-III-R conduct disorder (CD) with features of DSM-III-R personality disorders (PDs) and psychopathy, in inpatient psychiatric practice. Fifty-six psychiatric inpatients, without a history of specified 'psychoses', were assessed by the SCID structured interview for DSM-III-R PDs and the 'Psychopathy Checklist Revised' (PCL-R). In a sample in which 59% had borderline PD, significant associations between a history of CD criteria and the adult features of antisocial PD (APD) were relatively specific compared with other PDs, but

Psychotic symptoms in patients with borderline personality disorder and concurrent axis I disorder.

OBJECTIVE: Whether psychotic symptoms are part of the fundamental psychopathology of borderline personality disorder remains in dispute. The goal of the study was to examine the incidence and nature of psychotic symptoms in a sample of patients with the disorder. METHODS: The inpatient psychiatric records of 92 patients with a discharge diagnosis of borderline personality disorder, some of whom had comorbid affective disorder or substance abuse disorder, were examined to obtain data on the presence of psychotic symptoms (narrowly defined as delusions and auditory and visual hallucinations), t

Do maternal psychotic symptoms predict offspring's psychotic disorder? Findings from the Helsinki High-Risk Study.

The Helsinki High-Risk (HR) Study is a follow-up study of 179 offspring born to mothers with DSM-IV-TR diagnoses of schizophrenia, schizoaffective disorder, other schizophrenia spectrum disorders, and affective psychoses. Mothers comprised all female patients born between 1916 and 1948 who had been treated with hospital diagnoses of schizophrenia, schizophreniform, or schizoaffective psychoses in any mental hospital in the city of Helsinki up to 1974, and who had given birth in Helsinki between 1960 and 1964. In this report we conducted a principal factor analysis of maternal symptoms using 12

The association between psychotic mania, psychotic depression and mixed affective episodes among 14,529 patients with bipolar disorder.

BACKGROUND: Psychotic and mixed affective episodes are prevalent in the course of bipolar disorder. Despite many studies on the implications of psychotic mania (PM), psychotic depression (PD) and mixed affective episodes (MAE), relatively little is known about the relationship between the three subtypes. The present study aimed to investigate whether the occurrence of PM, PD and MAE were associated with one another. METHODS: This is a nationwide register-based, historical prospective cohort study. Data was obtained from the Danish Psychiatric Central Research Register. Subjects were defined a

A systematic review and meta-analysis of suicidality in psychotic disorders: Stratified analyses by psychotic subtypes, clinical setting and geographical region.

We studied the prevalence of suicide attempts and cumulative incidence of completed suicide in schizophrenia (SZ), schizoaffective disorder (SZAF), delusional disorder (DD) and first-episode psychosis (FEP). A systematic review was performed using Scopus and PubMed databases (1990- July 2020). A random effects meta-analysis was conducted. Stratified analyses were conducted by diagnosis, clinical setting and geographical region. The prevalence of attempted suicide was 20.3% for SZ, 46.8% for SZAF, 11.1% for DD and 12.5% for FEP. Suicide attempts rates were higher for outpatient samples than for

Polyunsaturated fatty acids and psychotic disorders: integrating mechanistic, genetic, and clinical evidence.

Polyunsaturated fatty acids (PUFAs), including omega-3 (n-3) and omega-6 (n-6) essential fatty acids, play critical roles in brain development and function, serving as precursors to a wide range of bioactive lipid mediators involved in immune and inflammatory signaling. Emerging evidence implicates PUFA dysregulation in schizophrenia and bipolar disorder, severe psychiatric conditions that remain incompletely treated by current therapeutics. In this review, we first outline the biological roles of PUFAs relevant to neurodevelopment and psychiatric risk, with particular emphasis on their intera

Exploring Combination Therapy for the Management of Psychotic Disorders: A Comprehensive Review.

Schizophrenia is a severe brain disorder impacting approximately 1% of the overall world population. A large number of FDA-approved antipsychotics are available in the market for the management of schizophrenia and other related disorders. However, 20-30% of patients exhibit treatment resistance, known as pharmacoresistant or refractory schizophrenia, where monotherapy proves to be ineffective. This review describes the various prevalent psychotic disorders, the antipsychotic mechanism of action, and drug resistance in monotherapy. The review highlights the significance of combination drug the

Turkish adaptation study of the Metacognition Assessment Scale-Abbreviated in psychotic disorders.

BACKGROUND: Metacognition refers to the ability to recognize one's own and others' feelings and thoughts and to utilize this information to develop and understanding of oneself and others. Given that metacognitive deficits are considered a key treatment target in schizophrenia spectrum disorder, the availability of reliable tools to measure these skills is essential. As an interviewer-rated scale, the Metacognition Assessment Scale-Abbreviated (MAS-A) offers advantages over self-report scales in assessing metacognitive capacity. This study aims to evaluate the validity and reliability of the T

Pharmacotherapy of Psychotic Disorders in Türkiye: Insights From a National Prescription Database.

OBJECTIVE: Psychotic disorders, affecting 3-7 per 1000 individuals, significantly impair mental functioning. Although antipsychotic medications represent the mainstay of treatment, prescribing practices can vary based on patient characteristics and across different countries. Currently, national epidemiological data on psychotropic prescribing patterns in Türkiye are lacking, and understanding these variations is crucial for informing treatment guidelines and optimizing patient care. METHODS: Data from the Turkish Prescription Information System (PIS) for the year 2019 were analyzed to evalua

The relationship between oxidative stress and psychotic disorders in 22q11.2 deletion syndrome.

BACKGROUND: 22q11.2 Deletion syndrome (22q11.2DS) is the most common microdeletion syndrome in humans. This condition is associated with a wide range of symptoms including immune and neuropsychiatric disorders. Notably, psychotic disorders including schizophrenia have a prevalence of ∼ 30%. A growing body of evidence indicates that neuroinflammation and oxidative stress (OS) play a role in the pathophysiology of schizophrenia. In this study, we aim to assess the interaction between 22q11.2DS, OS and schizophrenia. METHODS: Blood samples were collected from 125 participants (including individu

Sex-Dependent Shared and Nonshared Genetic Architecture Across Mood and Psychotic Disorders.

BACKGROUND: Sex differences in incidence and/or presentation of schizophrenia (SCZ), major depressive disorder (MDD), and bipolar disorder (BIP) are pervasive. Previous evidence for shared genetic risk and sex differences in brain abnormalities across disorders suggest possible shared sex-dependent genetic risk. METHODS: We conducted the largest to date genome-wide genotype-by-sex (G×S) interaction of risk for these disorders using 85,735 cases (33,403 SCZ, 19,924 BIP, and 32,408 MDD) and 109,946 controls from the PGC (Psychiatric Genomics Consortium) and iPSYCH. Romantic relationships, sexuality, and psychotic disorders: A systematic review of recent findings.

UNLABELLED: For individuals with a psychotic disorder, dating can present several challenges and lead many to be excluded from intimate relationships. These difficulties may stem from a number of factors, including impairments in social and sexual functioning. Although scientific interest in this topic is mounting, the last quantitative review of the literature dates back to 2003. OBJECTIVES: The aim of this systematic review was to collect, evaluate, and synthesize quantitative data from studies published in the last 15 years on romantic relationships and sexuality in the context of a psycho

Endocannabinoid system in psychotic and mood disorders, a review of human studies.

Despite widespread evidence of endocannabinoid system involvement in the pathophysiology of psychiatric disorders, our understanding remains rudimentary. Here we review studies of the endocannabinoid system in humans with psychotic and mood disorders. Postmortem, peripheral, cerebrospinal fluid and in vivo imaging studies provide evidence for the involvement of the endocannabinoid system in psychotic and mood disorders. Psychotic disorders and major depressive disorder exhibit alterations of brain cannabinoid CB1 receptors and peripheral blood endocannabinoids. Further, these changes may be se

Accelerated Aging of Functional Brain Networks Supporting Cognitive Function in Psychotic Disorders.

BACKGROUND: Across networks, connectivity within the frontoparietal network (FPN) and cingulo-opercular network (CON) exhibits reductions earliest during healthy aging, contributing to cognitive impairment. Individuals with psychotic disorders demonstrate evidence of accelerated aging across multiple biological systems. By leveraging a large sample of patients with psychosis from early to chronic illness stages, this study sought to determine whether the CON and FPN exhibit evidence of accelerated aging in psychotic disorders, confirm associations between network efficiency and cognition, and
