Assessment of Out-of-Pocket Expenditure of HER2-Positive Breast Cancer Patients in a Tertiary Cancer Center and Private Clinics in the Philippines.

BACKGROUND: The survival advantage of HER2-positive breast cancer from targeted treatment is commonly undermined by catastrophic health expenditure (CHE), particularly in resource-limited areas. Recognizing that financial catastrophe leads to non-adherence to treatment and dissaving practices, we examined the out-of-pocket (OOP) expenses of patients with HER2-positive breast cancer. OBJECTIVE: The study aimed to estimate the median total per-cycle out-of-pocket expenditure of HER2-positive breast cancer treatment from the patient perspective, in public and private clinics, evaluate the associ

Cost savings from the provision of specific methods of contraception in a publicly funded program.

OBJECTIVES: We examined the cost-effectiveness of contraceptive methods dispensed in 2003 to 955,000 women in Family PACT (Planning, Access, … ntraceptives, $1.43. CONCLUSIONS: All contraceptive methods were cost-effective-they saved more in public expenditures for unintended pregnancies than they cost to provide. Because no single method is clinically recommended to every woman, it is medically and fiscally advisable for public health programs to offer all contraceptive methods.

Impact of a New York City supportive housing program on Medicaid expenditure patterns among people with serious mental illness and chronic homelessness.

BACKGROUND: A rapid increase of Medicaid expenditures has been a serious concern, and housing stability has been discussed as a means to reduce Medicaid costs. A program evaluation of a New York City supportive housing program has assessed the association between supportive housing tenancy and Medicaid savings among New York City housing program applicants with serious mental illness and chronic homelessness or dual diagnoses of mental illness and substance use disorder, stratified by distinctive Medicaid expenditure patterns. METHODS: The evaluation used matched data from administrative reco

Burden and cost of communicable, maternal, perinatal and nutrition deficiency diseases in India.

BACKGROUND: Globally 36% of deaths and 42% of Disability Adjusted Life Years (DALYs) are due to communicable, maternal, perinatal and nutritional disorders (CMPND). We examined the state-wise disease burden and treatment cost for these diseases in India for 2017. METHODS: DALYs for CMPND was obtained from National Disease Burden Estimate (NBE) Study and the expenditure was determined from the unit level records of persons who reported hospitalization for one or more CMPND in National Sample Survey (NSS)-75th Round. RESULTS: The top conditions resulting in high DALYs for India were perinatal

Beyond teaching and research: the hidden economic value of university-affiliated physicians in Italian public hospitals.

University-hospital conventions within the Italian Servizio Sanitario Nazionale, regulated by D.Lgs. 517/1999, allow academic clinicians to serve as clinical directors in public hospitals. While the educational and scientific benefits are well described, systematic economic evaluation of these arrangements is lacking. A 12-month Budget Impact Analysis was conducted comparing two staffing models: (i) a hospital-employed clinical director fully remunerated by the regional health service, and (ii) a university-affiliated professor whose base salary is funded by the university under national conve

Exploratory analysis of regional factors influencing early childhood caries status in 5-year-old children: evidence from national oral health surveys of China, 2005-2015.

BACKGROUND: Currently, regional disparities of early childhood caries (ECC) remained underexplored. This study aimed to investigate the associations of regional factors with temporal variations of ECC among 5-year-old children in China using data from the 3rd National Oral Health Survey of China (the 2005 survey) and the 4th National Oral Health Survey of China (the 2015 survey). METHODS: Post-stratification-weighted data combining the two rounds of cross-sectional National Oral Health Surveys were used for establishing negative binomial regression models, with the number of decayed, missing

Association between governmental spending on social services and health care use among low-income older adults.

Prior research demonstrates that local government spending on social policies, excluding health care, is linked to improved population health. Whether such spending is associated with better access to primary care and reduced acute care utilization remains unclear. In this cross-sectional study, we evaluated the associations between county-level social spending and individual-level health care utilization among low-income Medicare beneficiaries, aged ≥65 years, from 2016 to 2018. We linked claims data to 4 categories of county-level government expenditures from the US Government Finance Databa

Private health investments under competing risks: Evidence from malaria control in Senegal.

This study exploits the introduction of high subsidies for anti-malaria products in Senegal in 2009 to investigate whether malaria prevents parents from investing in child health. A simple model of health investments under competing mortality risks predicts that private expenses to fight malaria and other diseases should increase in response to anti-malaria public interventions. We test and validate this prediction using original panel data from a household expenditure survey combined with geographical information on malaria prevalence. We find that health expenditures in malarious regions cat

No Cost Sharing for Public Assistance Recipients and Health Service Usage in Japan.

IMPORTANCE: In Japan, the public assistance (PA) system for low-income individuals provides medical aid that covers all health care costs (no cost sharing). However, little is known about how health care service utilization changes after PA certification. OBJECTIVE: To examine the trajectories of outpatient medical and dental care utilization among PA recipients before and after PA certification. DESIGN, SETTING, AND PARTICIPANTS: Longitudinal data from a municipality in Japan participating in the Longevity Improvement Fair Evidence Study from 2017 to 2022 were used. Generalized estimating e

Competing paradigms of obesity pathogenesis: energy balance versus carbohydrate-insulin models.

The obesity pandemic continues unabated despite a persistent public health campaign to decrease energy intake ("eat less") and increase energy expenditure ("move more"). One explanation for this failure is that the current approach, based on the notion of energy balance, has not been adequately embraced by the public. Another possibility is that this approach rests on an erroneous paradigm. A new formulation of the energy balance model (EBM), like prior versions, considers overeating (energy intake > expenditure) the primary cause of obesity, incorporating an emphasis on "complex endocrine, me

Long-Term Health and Economic Impact of a Community-Based, Gene-Guided, Nutrition Program: The Sakado Folate Project in Japan.

BACKGROUND/OBJECTIVES: Precision nutrition informed by genetic profiling has been proposed to improve public health outcomes; however, long-term, community-based evidence remains limited. This study evaluated the long-term health and economic impacts of the Sakado Folate Project. METHODS: Since 2006, residents participating in the Sakado Folate Project received gene-guided nutritional counseling focused on folate intake and related lifestyle factors. Target genes included methylenetetrahydrofolate reductase (MTHFR), angiotensinogen (AGT), adrenoreceptor B3 (ADRB3), and uncoupling protein 1 (U

Gender equity and public health outcomes: The COVID-19 experience.

This paper extends the growing research on the impact of gender equity on public health outcomes using the ongoing Covid-19 pandemic as its research setting. Specifically, it introduces a conceptual model incorporating the impact of gender equity and human development on women's representation in legislature and public health expenditure, and their combined impact with human environment (population density, aging population and urban population) on important public health outcomes in the Covid-19 context, including the total number of tests, diagnosed, active and critical cases, and deaths. The American Cancer Society's National Prostate Cancer Detection Project.

As a significant public health problem, prostate cancer meets nearly all the criteria for screening. While concerns about incomplete natural history, progression rates and need for better prognostic factors are valid, important social and public health issues also need consideration. If future expenditures for terminal cancer care are minimized via reductions in therapy choices or coverage, no economic benefit for prostate cancer screening should exist. Narrowly focused attempts at cost reduction could inappropriately discourage highest risk groups from participating in early detection program

Slovakia: Health System Review.

This analysis of the Slovak health system reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance. Slovakia, a central European country with a population of 5.4 million, continues to face significant health and health care system challenges. Slovakia's health system is founded on universal coverage with compulsory health insurance, a broad benefits package and a competitive insurance model. Although life expectancy improved between 2000 and 2019, the COVID-19 pandemic reversed gains, and in 2023 Slovak life expectancy remai

Trends in governmental expenditure on vaccination programmes in the Netherlands, a historical analysis.

BACKGROUND: Health economic evaluations are often required before implementing a vaccination programme. Such evaluations rarely consider the historical context of a vaccination programme. We review the financial history of vaccination programmes in the Netherlands, and compare these to demographic and macroeconomic developments as well as avoided mortality burden. METHODS: Previously uncatalogued historical expenditures on the Dutch National Immunisation Programme (NIP) and influenza vaccination were obtained from official reports. Costs were adjusted for inflation using Consumer Price Indice

Resource allocation in Pakistan's health sector: a critical appraisal and a path toward the Millennium Development Goals.

Pakistan is trying hard to sustain its progress toward the Millennium Development Goals. However, because of a lack of political commitment to innovative solutions to improve its financing mechanism, the health system is unable to provide even essential and basic services to the people. The country, with more than 70% of the population living on less than two US dollars a day, largely depends on direct taxes for its revenue. Because of inadequate financing, the quality of government services is inexcusably poor; therefore, a majority of people seek healthcare in the private sector. This has le

Public health and national security: the critical role of increased federal support.

Protecting the public's health historically has been a state and local responsibility. However, the growing threat of bioterrorism has highlighted the importance of a strong public health infrastructure to the nation's homeland security and has focused increased attention on the preparedness of the public health system. As a result, federal public health funding has increased exponentially since the anthrax attacks of late 2001, and Congress has passed sweeping new federal legislation intended to strengthen the nation's public health system. This heightened level of federal interest and suppor
