Bridging crisis recovery and long-term transformation of the health system in Lebanon: evidence from key informant interviews and global lessons.

BACKGROUND: Since late 2019, Lebanon’s health system has faced immense strain an unprecedented financial and economic crisis, aggravated by the COVID-19 pandemic, and the Beirut Port explosion which damaged or destroyed a third of the capital, Beirut. This has created an urgent need for reform, heightened by the subsequent conflict (2023–2026) and its additional toll on health infrastructure and personnel. METHODS: A purposive sample of 27 key informants was chosen, based on their expertise in public health, hospital management, healthcare financing, and policy-making. Informants included poli

International migration of Africa-born health workers: a scoping review of drivers and policies, 2010-2024.

BACKGROUND: Africa faces a critical shortage of human resources for health (HRH), crucial for improving population health. We described drivers of and policies to manage the migration of Africa-born HRH from source (African) countries in Africa to destination (non-African) countries. METHODS: We conducted a scoping review, using the Arksey and O'Malley framework, using data from peer-reviewed articles (2010-2024) and gray literature from institutions like Africa Centers for Disease Control (CDC) and World Health Organization (WHO). We used push-pull theory to identify economic, professional,

Building Local Research Capacity for Global Pandemic Preparedness: Lessons from WHO Unity Studies and Their Expansion in India.

The rapid onset and progression of the COVID-19 pandemic highlighted the critical necessity for standardized, timely epidemiological investigations to generate actionable evidence for public health policy. The World Health Organization (WHO) Global Influenza Surveillance and Response System (GISRS) and Unity Studies and Investigations initiative (Unity Studies) provides the standardized framework to address these critical knowledge gaps. This manuscript reflects upon the Hamdard Institute of Medical Sciences and Research (HIMSR)'s experience as an active site implementing three WHO Unity proto

Healthcare Infrastructure and Resource Barriers to Preventing Mother-to-Child Transmission of HIV in Ghana: Insights from a Qualitative Study.

BACKGROUND: The prevention of the mother-to-child transmission (PMTCT) of HIV is a vital strategy in reducing paediatric HIV infections. However, the delivery of PMTCT services is frequently impeded by resource constraints within the healthcare systems. This study investigates the systemic barriers affecting PMTCT implementation in Ghana and examines the disconnection between health policy design, priority setting, and on-the-ground realities. METHODS: The study employed the qualitative approach using a case study research design. The purposive sampling technique was used in selecting the hea

Qualitative assessment of maternal healthcare services: Infrastructure, staffing, and quality of care in Lejweleputswa District Municipality, South Africa.

Maternal healthcare performance varies widely across South Africa, shaped by uneven resources, staffing constraints, and service-delivery pressures. This study explored how healthcare providers experience the conditions that influence maternal care, focusing on infrastructure, staffing, and quality-of-care processes. Using a qualitative case study design, semi-structured interviews were conducted with nurses, midwives, and medical officers working in public-sector maternity services. Data were analysed using an interpretative phenomenological approach. Participants described persistent infrast

Partnerships and organizational capacity drive sustainability of integrated hypertension-HIV care in primary health care in Uganda.

BACKGROUND: The co-occurrence of HIV and non-communicable diseases (NCDs), particularly hypertension, is a growing global health issue. As people living with HIV (PLHIV) live longer due to antiretroviral therapy (ART), they become more susceptible to NCDs like hypertension due to chronic inflammation, long-term drug side effects, and metabolic factors that compound their risk. The integrated HIV/HTN project implemented a feasible and cost-effective multi-component intervention in several public health facilities in Uganda. In this study, we sought to evaluate the program sustainability of the

U.S. Funding Cuts and Health System Challenges in Community-Based HIV Programmes in Rural KwaZulu-Natal, South Africa: A Qualitative Study.

Community-based organisations (CBOs) play a critical role in implementing HIV programmes, many of which have historically relied on United States (U.S.) donor funding. Recent U.S. funding cuts have disrupted community-based HIV programmes, underscoring the need to understand implementers' experiences to support sustainable service delivery. This study explored how funding instability affected community-based HIV programmes in rural KwaZulu-Natal, South Africa. A qualitative, exploratory design was employed using interviews and focus group discussions with purposively selected youth frontline w

A study on the impact of digital infrastructure development on the health of low-income rural residents: based on panel data from 2010 to 2022.

The health status of low-income rural residents is intricately linked to social equity and justice and the realization of the goal of common prosperity. Based on the CFPS data from 2010 to 2020 and the list of "Broadband China" demonstration cities, this paper employs a multi-period Difference-in-Differences (DID) approach to empirically analyze the impact of digital infrastructure construction on the health of low-income rural residents and its mechanism. The results indicate that: (1) Digital infrastructure construction had a significant positive impact on the health of rural low-income resi

The Unequal Burden of Environmental and Health Costs of Energy-Intensive Artificial Intelligence.

We examined environmental and public health implications of energy-intensive artificial intelligence (AI) infrastructure, emphasizing health equity, workers, and vulnerable communities. We analyzed evidence on AI infrastructure across its life cycle, including data-center electricity demand, fossil-fuel power, water use, heat, noise, critical-mineral extraction, occupational exposures, and electronic waste (e-waste). AI infrastructure can expose nearby communities and workers to air pollution, heat, noise, water stress, hazardous mining conditions, and e-waste toxicants. In 2023, US data cente

The Cost and Public Health System Effects of Active Monitoring and Illness Response for Ebola Virus Disease: A Case Evaluation of Georgia.

In August 2014, the World Health Organization declared the Ebola virus disease epidemic in West Africa a public health emergency of international concern. After 2 imported cases of the disease were identified in the United States in autumn 2014, the Centers for Disease Control and Prevention recommended that all jurisdictions begin active monitoring of travelers at risk of developing Ebola virus disease for 21 days from the last day of a potential exposure to minimize the risk of disease transmission. Here we describe the infrastructure development, monitoring processes, total planned expendit

Optimizing Georgia's public health workforce: a study on demographics, engagement, and capacity building.

BACKGROUND: A well-functioning public health system relies on a robust workforce. Comprehensive data on the workforce, such as number, distribution, and key characteristics, are crucial for evidence-based workforce planning and development. However, few comprehensive public health workforce assessments exist, especially in low- and middle-income countries. Public health reforms over the years and needs identified during the COVID-19 pandemic prompted this assessment in Georgia. METHODS: A survey of the core public health workforce, including employees at central and regional units of the Nati

Local variation in public health preparedness: lessons from California.

Since September 2001 Congress has allocated approximately dollars 3 billion to strengthen the public health infrastructure. To achieve this goal, the U.S. Centers for Disease Control and Prevention (CDC) allocates funding to states, which distribute funds to local jurisdictions. Evidence-based measures to assess public health preparedness are lacking. We used an expert-panel process to develop performance measures, based on the ten essential public health services. We developed and conducted tabletop exercises in California to evaluate preparedness to detect and respond to a hypothetical small…

Infection prevention and control in low- and middle-income countries: policy, practice, and implementation challenges in sub-Saharan Africa.

SUMMARYInfection prevention and control (IPC) programs are essential for safe healthcare; yet, implementation across sub-Saharan Africa remains constrained by chronic underfunding, weak water, sanitation, and hygiene (WASH) systems, limited laboratory capacity, and shortages of trained staff. Healthcare-associated infections (HCAIs) are presumed common, underreported, and often severe, with a disproportionate burden among hospitalized neonates and postpartum women, and they pose substantial risks to healthcare workers during outbreaks. Gram-negative pathogens such as Klebsiella, Escherichia co

Sustainability of infectious disease elimination: a scoping review on integrating treatment and surveillance within health systems.

BACKGROUND: Global infectious disease elimination programmes have historically relied on vertical, donor-driven approaches. However, amid reduced global funding and increased emphasis on country ownership and sustainability, interest in infectious disease-specific activities integrated into routine health systems is growing. Yet evidence on how integration has been implemented remains fragmented. This scoping review takes a multi-disease approach to assess the extent and nature of evidence on the integration of infectious disease elimination and eradication programmes into routine health syste

Reassessing the public health value of regional cancer registries in the era of Japan's National Cancer Registry: a learning health system perspective.

Cancer registries are integral to public health infrastructure and support not only the monitoring of cancer incidence and mortality but also cancer control planning, quality assurance of screening programs, evaluation of healthcare delivery, prognostic assessment, and development of research infrastructure. Japan's National Cancer Registry (NCR) was launched in 2016, and many long-standing regional cancer registries were subsequently integrated into the national system; however, the Kanagawa Cancer Registry (KCR) continues to operate independently. This Perspective examines how a regional can…
