Incidence of puerperal metritis and associated risk factors in dairy cows in Hawassa, Southern Ethiopia.

Metritis has considerable economic impact on dairy herd profitability due largely to reduced reproductive performance. Studies in Ethiopia only report prevalence, with no available data on the incidence of metritis. The present prospective cohort study aimed to estimate the incidence of puerperal metritis, identify risk factors, and isolate the causative bacteria in dairy cows in Hawassa. Daily follow-up of 120 dairy cows from parturition to 21 days postpartum was carried out from November 2019 to February 2021. Of these, 21 developed puerperal metritis with an incidence rate of 0.94 cases per

[Milk fever].

Infectious complications following delivery were, in the past, attributed to "milk fever": these were milk congestion, milk deposits, rancid milk, etc., that were held responsible. The milk was reabsorbed into the blood of the patient and settled in the peritoneum ("milk peritonitis"), in the broad ligaments (pelvic abscess), in the thighs (phlebitis) and also in the breasts (breast abscess). This belief, originated by Aristotle, was accepted by excellent authors like Andre Levret (1703-1780), one of the most famous French obstetricians and Nicolas Puzos, at the same time. More recently, autho

Peripartum cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is a rare type of cardiomyopathy of unknown aetiology associated with significant mortality and morbidity and characterized by heart failure in late pregnancy or puerperium. Recently PPCM workshop committee has recommended inclusion of echocardiographic features of LV dysfunction to redefine PPCM. Subsequent pregnancies are associated with a very high mortality in these patients and hence should be avoided. Women with PPCM continue to have significant mortality despite the use of conventional drugs for managing heart failure. Use of newer drugs such as immunogl

Sheehan's syndrome presenting as postpartum seizures.

We report a case where a patient presented with generalised tonic-clonic seizures secondary to nausea, vomiting and dehydration. She had suffered a postpartum haemorrhage six months previously. On laboratory assessment hyponatraemia and low hormone concentrations suggested pituitary failure. The diagnosis was confirmed by magnetic resonance imaging of the head, which showed a partially empty sella turcica. Given the severity of the morbidity in this case we emphasise that Sheehan's syndrome should be suspected in women presenting with postpartum seizures.

Bridge to recovery with a thoratec biventricular assist device for postpartum cardiomyopathy.

A 25-year-old African-American woman developed shortness of breath 7 days after a spontaneous vaginal delivery. She was found by echocardiogram to have a left ventricular ejection fraction of 10% and was diagnosed with postpartum cardiomyopathy. Despite medical therapy including diuretics, an ace inhibitor, and a beta blocker, over the next 5 months, the patient had more than 12 hospital admissions for congestive heart failure. After accepting her in transfer, we placed her on multiple IV inotropes and observed no improvement. We then implanted a Thoratec Biventricular Assist Device. During 26

An unusual case of peripartum cardiomyopathy in a parturient with preeclampsia.

Here we report an unusual development of peripartum cardiomyopathy (PPCM) in a parturient woman with preeclampsia. A 36-year-old nulliparous parturient woman underwent elective cesarean section for delivery of twins under spinal anesthesia. Both preoperative workup and past history were unremarkable except for proteinuria and hypertension for 1 week. Approximately 4 hours after cesarean section, progressive orthopnea developed. Chest plain film showed acute pulmonary edema, bilateral pulmonary infiltration with interstitial patches, and cardiomegaly. Postpartum cardiomyopathy was diagnosed aft

[Peripartum cardiomyopathy].

The Peripartum Cardiomyopathy is a rare form of heart disease, of uncertain etiology, more common in black and multiparous women, older than thirty years old. Is defined as development of maternal congestive heart failure, in the last month of pregnancy or within five months after delivery, with documented left ventricular systolic dysfunction, in the absence of a demonstrable cause for heart failure in a previously healthy woman. The diagnosis is commonly established with chest radiography, electrocardiogram and echocardiography. Treatment consist in medical therapy with inotropic support, af

Cerebrovascular accident with quadriplegia following postpartum eclampsia.

A 26-year-old woman (para 1 + 0) was managed at the National Hospital, Abuja, Nigeria for postpartum eclampsia with quadriplegia following referral from a peripheral hospital with a history of a solitary tonic/clonic seizure and unconsciousness. Her antenatal period had been uneventful until she presented with labour pains, where examination revealed an elevated blood pressure. She fitted once after delivery and remained unconscious for more than 12 h, hence the referral. The patient was managed in the intensive care unit (ICU), where she was found to be quadriplegic. She received mechanical v

Posterior reversible encephalopathy syndrome: a case study.

A young woman 4 days postpartum was admitted after experiencing two seizures. Her mentation waxed and waned until, after several hours, staff were unable to arouse her with voice or touch. A computed tomography scan demonstrated considerable white-matter edema. The patient's condition declined to a coma. She remained comatose despite therapeutic interventions to control increased intracranial pressure. To her family, her condition was a source of anguish. To the physicians and nurses, she was a puzzle. The final diagnosis was posterior reversible encephalopathy syndrome, which was related to a

Clinical manifestations of postpartum thyroid disease.

Postpartum thyroiditis (PPT) occurs in 5%-9% of unselected postpartum women; hyperthyroidism and hypothyroidism develop, the latter being permanent, in up to 25 %-30% of women. PPT is strongly associated with antithyroid peroxidase (anti-TPO) antibodies, but 50% of anti-TPO positive women do not develop thyroid dysfunction. Symptom analysis has shown that lack of energy and irritability were the most frequent hyperthyroid symptoms whereas lack of energy, aches and pains, poor memory, dry skin, and cold intolerance were the significant hypothyroid features. Some of these symptoms were more freq

Periarteritis nodosa presenting as a breast lesion: report of a case and review of the literature.

We describe a 34-year-old woman with periarteritis nodosa (PAN) presenting as a breast lesion. Localized involvement of the breast is an unusual manifestation of PAN. To date, 10 cases have been reported: all were in women with an age range of 45-78 years (mean 63). In most cases, breast lesions were an isolated finding, and the prognosis was favourable, setting them apart from the more common form of systemic PAN. The case presented is unusual in that vasculitis developed in the postpartum period, and was associated with cutaneous PAN-like lesions elsewhere on the body, and digital artery occ

[Reversible severe ventricular dysfunction in female patient with eclampsia and HELLP syndrome].

A 23 year-old primigravid woman presented eclampsia and HELLP syndrome (hemolysis, elevated liver enzymes, low platelets) with severe left ventricular dysfunction after having a cesarean section. In a few weeks, clinic disappeared and ventricular function normalized. The importance of differential diagnosis between ventricular dysfunction in eclampsia and peripartum myocardiopathy is discussed.

Intestinal obstruction complicating pregnancy.

Intestinal obstruction is a rare but serious complication of pregnancy with significant maternal and fetal mortality. The reported incidence of intestinal obstruction complicating pregnancy varies widely, from 1 in 66,431 to 1 in 1,500 deliveries. A retrospective review of 66 cases of intestinal obstruction complicating pregnancy and the puerperium, including 2 cases from our institution, revealed that the most common causes of mechanical obstruction were adhesions (58%), volvulus (24%), and intussusception (5%). Seventy-seven percent of the patients with obstruction due to adhesions had under
