Neuroimaging of dementia in 2013: what radiologists need to know.

The structural and functional neuroimaging of dementia have substantially evolved over the last few years. The most common forms of dementia, Alzheimer disease (AD), Lewy body dementia (LBD) and fronto-temporal lobar degeneration (FTLD), have distinct patterns of cortical atrophy and hypometabolism that evolve over time, as reviewed in the first part of this article. The second part discusses unspecific white matter alterations on T2-weighted and fluid-attenuated inversion recovery (FLAIR) images as well as cerebral microbleeds, which often occur during normal aging and may affect cognition. Injectable corticosteroid and local anesthetic preparations: a review for radiologists.

Corticosteroids and local anesthetics are some of the most commonly administered medications in radiology departments. These medications have marked variability in their formulations, which may increase their adverse event profile for specific procedures. In particular, certain corticosteroid preparations are associated with adverse central nervous system (CNS) sequelae. This is most likely due to distal embolization by particulate formulations. Nonparticulate steroid formulations are not associated with such events. Local anesthetics have severe CNS and cardiac adverse effects if injected int

Outcome of 350 implanted chest ports placed by interventional radiologists.

PURPOSE: To determine the outcome of implanted chest ports placed by interventional radiologists. MATERIALS AND METHODS: Between June 1993 and July 1996, a single institution placed 350 implanted chest ports in 346 patients by means of the subclavian vein approach. The medical records of these patients were reviewed to determine the outcome of the ports. Ports were implanted for chemotherapy (n = 341), blood transfusion (n = 7), or antibiotics (n = 2). RESULTS: Immediate complications were seven (2%) pneumothoraces and one (0.3%) hematoma. Four (1.1%) of the pneumothoraces necessitated hospi

Musculoskeletal ultrasound in traumatic emergencies: what radiologists need to know.

While conventional radiography (CR) and computed tomography (CT) remain diagnostic staples in emergency settings, ultrasound (US) is a critical tool offering a rapid, dynamic, and noninvasive alternative for acute musculoskeletal (MSK) pathologies assessment. Prompt identification of conditions such as unstable fractures, soft tissue or muscular hematomas, joint effusions, and tendon ruptures is essential to prevent or mitigate long-term patient morbidity. This review summarizes the role of US in the emergency department, providing a practical overview of targeted scanning techniques and key s

Venous excess doppler ultrasound: What radiologists should know in 2026.

Systemic venous congestion is increasingly recognized as a key contributor to organ dysfunction and adverse outcomes, particularly in the cardiorenal and critical care settings. Elevated right-sided cardiac pressures are transmitted retrogradely to the hepatic, splanchnic, and renal venous systems, leading to impaired organ perfusion, increased interstitial pressure, and complications such as acute kidney injury. Conventional clinical markers and biomarkers of congestion lack sensitivity for detecting early or subclinical venous hypertension. In contrast, ultrasound, particularly when incorpor

The CLEAR model: a structured communication protocol for breast radiologists.

BACKGROUND: Communication in diagnostic breast imaging remains a critical yet frequently neglected aspect of radiological training. Traditionally centered on technical image interpretation, radiological education often leaves practitioners unprepared to navigate the severe emotional and psychological burden of a suspected breast cancer diagnosis. OBJECTIVES: This paper advocates for the communicative autonomy of the breast radiologist, arguing that direct, empathetic communication is not a secondary skill, but a core clinical act that directly influences both the psychological wellbeing and t

Neuroendocrine Malignancies of the Cervix: What Radiologists Need to Know.

Neuroendocrine malignancies of the cervix (NEM) are an aggressive tumor with early metastasis and recurrence, resulting in poor prognosis and 5-year mortality ranging from 10% to 25% for later stages of disease. These are a rare malignancy of middle-aged and elderly women with an incidence of <2%. The clinical presentation is like the common cervical cancer-squamous cell carcinoma, presenting with nonspecific vaginal bleeding. Pap smear has very low sensitivity and accuracy for NEM, leading to delayed diagnosis. Due to its low incidence, the lack of specific clinical findings, and the limited

CT presentations of pancreatic lymphoma: what radiologists need to know.

Pancreatic lymphoma is a rare entity that can mimic other pancreatic pathologies, particularly pancreatic ductal adenocarcinoma (PDAC), making accurate radiologic recognition essential as management differs significantly. Pancreatic lymphoma is treated with chemotherapy and generally carries a more favorable prognosis, whereas PDAC typically requires surgical intervention. Through a series of illustrative cases, many of which were initially referred to a PDAC multidisciplinary conference but ultimately proven to be lymphoma, we highlight the diverse CT imaging spectrum of pancreatic lymphoma,

MRI assessment of velopharyngeal dysfunction: A comprehensive primer for radiologists.

Velopharyngeal dysfunction (VPD) is a condition where the velopharyngeal mechanism fails to close properly, resulting in nasal air escape during speech and hypernasal resonance. VPD is common in patients with cleft palate and presents significant challenges, often necessitating advanced imaging techniques for surgical treatment planning. Recent advancements in magnetic resonance imaging (MRI) have enhanced the capability to obtain detailed static images of velopharyngeal structures at rest and during sustained phonation, which are comparable or superior to traditional technologies. MRI is part

Mucin-producing cystic hepatobiliary neoplasms: Key diagnostic information for radiologists.

Mucin-producing cystic hepatobiliary neoplasms are rare neoplasms which include mucinous cystic neoplasms of the liver and intraductal papillary mucinous neoplasms of the bile duct. Mucinous cystic neoplasms of the liver are characterised by the presence of ovarian-like stroma, and are usually benign. They present as large, multiloculated, cystic masses without biliary communication and are more common among middle-aged women. Intraductal papillary mucinous neoplasms of the bile duct are premalignant lesions originating in the bile duct and presenting with biliary communication. There are 3 gr

Multidisciplinary Approach to Chronic Thromboembolic Pulmonary Hypertension: Role of Radiologists.

Management of chronic thromboembolic pulmonary hypertension (CTEPH) should be determined by a multidisciplinary team, ideally at a specialized CTEPH referral center. Radiologists contribute to this multidisciplinary process by helping to confirm the diagnosis of CTEPH and delineating the extent of disease, both of which help determine a treatment decision. Preoperative assessment of CTEPH usually employs multiple imaging modalities, including ventilation-perfusion (V/Q) scanning, echocardiography, CT pulmonary angiography (CTPA), and right heart catheterization with pulmonary angiography. Comparing diagnosis of midfacial fractures by radiologists and plastic surgeons.

BACKGROUND: Clinicians who manage facial fractures often rely on radiologist interpretations to help with assessment and management. Among treating physicians, facial fractures are categorized into clinically relevant patterns of injury. On the other hand, while radiologists are unsurpassed at identifying individual breaks in the bone, larger fracture patterns are not always conveyed in radiology reports. PURPOSE: This study aims to assess the frequency with which the terminology describing midfacial fracture patterns is concordant among radiologists and treating clinicians. METHODS: The aut

Spontaneous Uterine Leiomyoma Torsion: A Challenging Differential Diagnosis for Radiologists.

UNLABELLED: Uterine leiomyomas are very common gynaecological benign tumours. Spontaneous torsion of a uterine subserosal leiomyoma is a rare cause of acute lower abdominal pain and should be treated immediately with surgery. We report a case of an enlarged subserosal leiomyoma that was first detected by computed tomography (CT) and further confirmed by laparoscopic surgery to be a subserosal leiomyoma with torsion. LEARNING POINTS: Spontaneous torsion of a uterine subserosal leiomyoma is a rare cause of acute lower abdominal pain.Contrast-enhanced CT of the pelvis reveals changes in size, ab

Xanthogranulomatous Cholecystitis: A Diagnostic Challenge for Radiologists, Surgeons, and Pathologists.

Xanthogranulomatous cholecystitis (XGC) is a rare inflammatory disease of the gallbladder characterized by severe proliferative fibrosis and the accumulation of lipid-laden macrophages in areas of destructive inflammation. Misdiagnosis is highly usual, and its macroscopic appearance may often be confused with gallbladder carcinoma. Here we discuss the case of a 56-year-old male who presented in the emergency room with fever, chills, and nausea. The routine laboratory investigations were normal except for elevated white blood cell counts. Abdominal ultrasound showed borderline gallbladder wall

A Stepwise Diagnostic Approach to Cystic Lung Diseases for Radiologists.

Lung cysts are commonly seen on computed tomography (CT), and cystic lung diseases show a wide disease spectrum. Thus, correct diagnosis of cystic lung diseases is a challenge for radiologists. As the first diagnostic step, cysts should be distinguished from cavities, bullae, pneumatocele, emphysema, honeycombing, and cystic bronchiectasis. Second, cysts can be categorized as single/localized versus multiple/diffuse. Solitary/localized cysts include incidental cysts and congenital cystic diseases. Multiple/diffuse cysts can be further categorized according to the presence or absence of associa

Elucidating early CT after pancreatico-duodenectomy: a primer for radiologists.

UNLABELLED: Pancreatico-duodenectomy (PD) represents the standard surgical treatment for resectable malignancies of the pancreatic head, distal common bile duct, periampullary region and duodenum, and is also performed to manage selected benign tumours and refractory chronic pancreatitis. Despite improved surgical techniques and acceptable mortality, PD remains a technically demanding, high-risk operation burdened with high morbidity (complication rates 40-50% of patients). Multidetector computed tomography (CT) represents the mainstay modality to rapidly investigate the postoperative abdomen,
