Management of Distal Ulnar Fracture Combined with Distal Radius Fracture.

The distal ulna is composed of the ulnar styloid, ulnar head, and distal ulnar metaphyseal area. Most of distal ulnar metaphyseal fractures are associated with distal radius fractures and this incidence tends to be greater in osteoporotic elderly. Consideration of the treatment of distal ulna metaphyseal fracture should be addressed after treating a distal radius fracture. If it is stable, cast immobilization is preferred, however, if it shows malalignment or instability, an operative method should be considered. More than half of distal radius fractures are combined with an ulnar styloid frac…

Assessment of a novel biomechanical fracture model for distal radius fractures.

BACKGROUND: Distal radius fractures (DRF) are one of the most common fractures and often need surgical treatment, which has been validated through biomechanical tests. Currently a number of different fracture models are used, none of which resemble the in vivo fracture location. The aim of the study was to develop a new standardized fracture model for DRF (AO-23.A3) and compare its biomechanical behavior to the current gold standard. METHODS: Variable angle locking volar plates (ADAPTIVE, Medartis) were mounted on 10 pairs of fresh-frozen radii. The osteotomy location was alternated within ea…

Differentiating stable buckle fractures from other distal radius fractures: the 1-cm rule.

BACKGROUND: Treatment pathways for isolated distal radius fractures in children are becoming more tailored to specific fracture types and include different management plans for stable buckle fractures versus potentially unstable distal radius fractures. OBJECTIVE: To propose a measurement rule to aid in differentiating stable buckle fractures from other distal radius fractures in children. MATERIALS AND METHODS: A medical record search identified 203 children with closed distal radius fractures during a 14-month period. A consensus of two senior readers served as a reference standard to dist…

Pilon fractures of the wrist. Displaced intra-articular fractures of the distal radius.

22 patients who sustained high energy wrist injuries are reported. These complex injuries resulted in articular disruption of the distal radius. Associated injuries included scapho-lunate dissociation (18%), central die-punch injuries (14%), ulnar fractures (41%) and diastasis at the distal radioulnar joint (23%). Nine fractures (41%) were open and almost a third of patients had other skeletal injuries. All patients were treated by external fixation and reviewed after a mean follow-up of 2 1/2 years. There were no excellent results and only ten good ones (45%). The mean functional impairment w

Monitoring distal radius fractures during rehabilitation using infrared thermography.

INTRODUCTION: Distal radius fractures are among the most common injuries with a relevant rehabilitation process. Following a tissue injury, an inflammatory response occurs, leading to an increase in local temperature. This temperature can be recorded using infrared thermography. This study aimed to determine the degree of skin temperature asymmetry in patients with distal radius fractures during the rehabilitation process, at the beginning and at discharge, as well as to examine the correlation between thermal asymmetry and clinical indicators of injury. METHODS: Infrared images of the forear…

[Basic principles of surgical treatment of distal radius fractures].

Distal radius fractures are one of the most frequent fractures of the upper extremities. The decision for conservative or surgical treatment is made after appropriate diagnostics using conventional radiographic and usually computed tomography imaging examinations. If the indications for surgical treatment are present, various options for reduction and fixation are available. The spectrum ranges from closed to open procedures up to accompanying arthroscopic support. Appropriate preoperative patient education about the procedure and the planned postinterventional treatment is essential. The goal…

Systematic review: dorsal bridge plating in distal radius fractures.

PURPOSE: Distal radius fractures are the most common upper limb fractures in adults (up to 18% of all fractures in the Emergency Department). Conservative management is possible for the majority, the preferred surgical technique being volar plate fixation. Dorsal bridge plating (DBP) is an alternative method of treatment for complex fractures. DBP acts as an internal fixator and can be used in patients needing early rehabilitation. This systematic review assesses the demographics, functional and radiological outcomes and complications of using DBP in patients with distal radius fractures compa…

[Carpal Malalignment in Malunited Fractures of the Distal Radius].

PURPOSE: To evaluate and classify carpal alignment in malunited fractures of the distal radius. MATERIALS AND METHODS: On standardized lateral radiographs of the involved wrist of 72 patients with a symptomatic extra-articular malunion of the distal radius, 43 with a dorsal and 29 with a palmar angulation, radius tilt (RT), radiolunate (RL) and lunocapitate angle were measured. Malposition of the radius was defined as RT plus 11° in dorsal malunion and RT minus 11° in palmar malunion. A palmar tilt of the radius was marked with a minus sign. At the time of corrective osteotomy 9 dorsal maluni

Overlapping Surgery for Distal Radius Fractures: Is It Safe?

Patients who underwent distal radius fracture (open reduction and internal fixation [ORIF]) at a Level 1 trauma center deemed "overlapping" (greater than 30 minutes overlap) were compared against consecutive cases. Unplanned return to surgery within 1 year was the primary outcome. Sixty-two patients were included in the overlapping group and 37 in the consecutive group. There was no difference in unplanned return to surgery 1 year following procedure with three cases (5%) in the overlapping group and one case (3%) in the consecutive group. There was a significant difference (p = 0.02) in proce

Updates on Distal Radius Fractures Past, Present, and Complications.

Before the 1900s, distal radius fractures were misdiagnosed as radiocarpal dislocations and most were treated nonoperatively. Between the 1900s and 1920s there were several anesthesia and antiseptic advancements that led to the advancement of surgical interventions. Then after the continued use and implementation of radiographs in orthopedics, radiographic parameters allowed for a critical analysis of treatment and patient outcomes that led to the further advancement of distal radius fracture treatment. This review will detail the historical content that led us to current practices. Additional…

Dorsal Cortical Breaks in Volar Barton Distal Radius Fractures.

Background: Volar Barton fractures involve the volar articular margin of the distal radius with proximal and volar subluxation of the carpus … ted with either age or gender (P > 0.05). Conclusions: Most (75%) patients with volar Barton distal radius fractures had a dorsal cortical break. Dorsal cortical breaks were not statistically associated with age or gender, suggesting these fracture patterns may not be associated with osteoporosis as previously hypothesized.

Distal radius fractures: strategic alternatives to volar plate fixation.

Volar locking plates have provided surgeons with enhanced capability to reliably repair both simple and complex fractures and avoid the hardware-related complications associated with dorsal plating. However, there have been an increasing number of published reports on the frequency and types of complications and failures associated with volar locked plating of distal radius fractures. An informed, critical assessment of distal radius fracture characteristics will allow surgeons to select an individualized treatment strategy that maximizes the likelihood of a successful outcome. Knowledge of th…

Standardized palmar plating of dorsally displaced distal radius fractures.

With the inauguration of fixed-angle plates, palmar plating has become a widely accepted way to treat dorsally displaced distal radius fractures. The technique by which the plate is applied to the radius varies. Such plates are primarily fixed either distally or at the proximal limbs. In this article, the standardized technique by which osteosynthesis is conducted in our institution is described step by step. The plate is first fixed to the shaft. Reduction is temporarily maintained by K-wires that run through the plate's distal margin. These K-wires are usually reliable in maintaining adequat…
