Propriospinal myoclonus: a neurophysiologic analysis.

We are reporting a neurophysiologic analysis of two patients presenting with thoracoabdominal spontaneous muscle jerks. Polymyographic recordings showed myoclonic bursts with onset in the upper rectus abdominis or lower intercostal muscles followed by rostral propagation to the upper intercostal and caudal propagation to the abdominal muscles by slowly conducting pathways. Jerk-locked back-averaging did not show time-locked cortical or premovement potentials. Peroneal somatosensory evoked response, C-reflex, and intercostal nerve conduction were normal. These findings suggest a generator for t

Influence of the vagus nerve upon the reflex control of the lower oesophageal sphincter.

In 24 control patients the lower oesophageal sphincter responded to graded increments in intra-abdominal pressure by a significant and sustained rise in tone. This response was abolished by atropine and was also absent in nine of 11 patients who had previously undergone truncal vagotomy for duodenal ulcer but not in the remaining two who had recurrent ulceration. In six patients studied after proximal gastric vagotomy a variable response was seen. Gastric efferent vagal function was assessed by a combined insulin/pentagastrin gastric secretory test and did not correlate closely with the lower

Reflex conditioning in a spinal man.

A man with a completely transected spinal cord and a spastic neurogenic bladder was conditioned to void upon the presentation of an external stimulus. A classical conditioning paradigm was employed in which strong abdominal shock (unconditioned stimulus) was paired with an initially neutral mild electrical stimulation of the thigh (conditioned stimulus; CS). After the pairing trials, a reliable conditioned response of urination was elicited by the CS alone. The conditioned response did not extinguish over time, and the procedure left the bladder with clinically safe residual amounts of urine.

Sensation of bloating and visible abdominal distension in patients with irritable bowel syndrome.

OBJECTIVES: Abdominal bloating and distension are common symptoms of irritable bowel syndrome (IBS). The postulated pathophysiological mechanisms underlying these symptoms include increased production, retention, or perception of gas or luminal contents. The aims of this study were to prospectively compare the prevalence of, and clinical factors related to, bloating and distension in an IBS patient population. METHODS: A total of 714 consecutive patients who met Rome I criteria for IBS were prospectively surveyed, and were classified as having bloating alone (B) or bloating and distension (B+

[Sclerosis of the anal sphincter from the point of view of mechanical ileus (author's transl)].

Anal sphincter sclerosis, which leads to recurrent subileus and finally to the complete picture of ileus, is presented in relation to 6 of our own observations. Ileus conditions are triggered off by: 1. years of abuse of laxatives, 2. intraperitoneal adhesions supporting the tendency to ileus (postoperative hernia, soft abdominal wall, intestinal atony), 3. viscerovisceral reflexes initiated by other diseases (heart, lung, gallbladder, liver, kidney etc.), 4. chronic recurrent changes with irritation of the terminal organ itself. Women seem predestined for this disease. The existing subileus o

Utility of the digital rectal examination in the emergency department: a review.

BACKGROUND: The digital rectal examination (DRE) has been reflexively performed to evaluate common chief complaints in the Emergency Department without knowing its true utility in diagnosis. OBJECTIVE: Medical literature databases were searched for the most relevant articles pertaining to: the utility of the DRE in evaluating abdominal pain and acute appendicitis, the false-positive rate of fecal occult blood tests (FOBT) from stool obtained by DRE or spontaneous passage, and the correlation between DRE and anal manometry in determining anal tone. DISCUSSION: Sixteen articles met our inclusi

The association between acute brucellosis with a Guillain-Barré syndrome-like presentation: a case report.

BACKGROUND: Brucellosis is a zoonotic disease caused by Gram-negative bacteria. It has variable manifestations (gastrointestinal, hepatobiliary, skeletal). Neurobrucellosis may develop at any stage of the disease (acute, subacute, and chronic phases) and affects the central and peripheral nervous systems. Acute peripheral neuropathy mimicking Guillain-Barré syndrome caused by brucellosis is rarely reported: only four cases in children were found in the literature review. CASE PRESENTATION: We report a case of a 4-year-old Syrian boy who presented with fever, weakness of lower limbs, backache,

Naloxone blocks suppression of cough by codeine in anesthetized rabbits.

Opioid receptors which are involved in cough generation are abundantly expressed in the brainstem. Codeine is a potent μ-opioid receptor agonist. In the present study we examined the effects of naloxone, a μ-opioid receptor antagonist, on mechanically-induced tracheobronchial cough and on the cough suppressing effect of codeine in six pentobarbitone anesthetized spontaneously breathing rabbits. A single dose of naloxone (0.4 mg/kg) followed by a single dose of codeine (7 mg/kg) were administered intravenously. The number and amplitude of cough and sneeze reflexes were examined sequentially; be

A case of reversible paraparesis following celiac plexus block.

BACKGROUND AND OBJECTIVES: Permanent and acute reversible paraplegia following celiac plexus block (CPB) have been reported. We report a case of prolonged reversible paraparesis after alcohol celiac plexus block. CASE REPORT: A 72-year-old man with primary multicentric pancreatic tumor and multiple hepatic metastases underwent alcohol celiac plexus neurolysis for severe abdominal pain radiating to the back. The patient had complete pain relief after the block but developed paresthesia of the left leg, which then spread to the right leg. Subsequently, loss of flexion and extension of the muscl

Anorectal physiologic testing for bowel dysfunction in patients with spinal cord lesions.

PURPOSE: Bowel dysfunction is common in patients with spinal cord lesions. This study aims to determine whether there are any discriminatory findings at anorectal physiologic testing in patients with spinal cord lesions. METHODS: Twelve consecutive patients (6 females) with significant spinal cord lesions who had mixed symptoms of constipation, fecal impaction, and fecal incontinence were evaluated by perfusion manometry and pudendal nerve terminal motor latency. None of the patients had a sphincter defect as evaluated by endoanal ultrasonography. RESULTS: The median age was 54 (range, 40-87

Bedside decompression of abdominal compartment syndrome caused by spontaneous pneumoperitoneum: A case report.

Pneumoperitoneum is the presence of free air within the peritoneal cavity and indicates perforation of a hollow viscus. However, it may also occur in the absence of perforation and in this case, it is called spontaneous pneumoperitoneum (SP). A 57-year-old female patient who was intubated and mechanically ventilated due to respiratory failure developed abdominal compartment syndrome (ACS) secondary to massive SP. Peritoneal lavage was performed for the patient both to achieve decompression and to support the diagnosis. Many surgeons proceed with laparotomy as a reflex response for SP due to la

Significance of urethral vascular pulsations in genuine stress urinary incontinence.

Urodynamic tracings of 100 women with genuine stress incontinence revealed urethra vascular pulsations in 15 patients, while 85 lacked vascular pulsations. The presence of vascular pulsations correlated with the maximal urethral closure pressure and identified a subgroup of genuine stress incontinence with normal sphincter strength and reflex activity. The only identifiable cause of stress incontinence for each patient in this group was a deficiency in the anatomic supports of the urethrovesical junction responsible for poor transmission of intra-abdominal pressure to the urethra during stress

[Disturbances of micturition after general surgical operations (author's transl)].

Impaired micturition after general surgical operations can be due to three causes, which occur individually or in combination. The first comprises side effects of anesthetics upon the autonomic nervous system. Inhibition of the abdominopelvic reflex, initiating micturition, due to the operative trauma represents the scond cause. Both the vegetative and the mechanical traumas, lead to decompensation of the urinary bladder, whose function is impaired by neuropathic factors and outflow obstruction. Pulmonary, cerebral, and abdominal insufficiency prolong the effects of both traumas. The third cau

Abdominal Tics in Children and Adolescents: A Phenomenology-First Diagnostic Framework and Differential Diagnosis.

AIM: Abdominal tics are a less commonly described axial or tonic tic phenotype in children and adolescents with Tourette syndrome and chronic tic disorders. Their abdominal location and symptom language may create diagnostic uncertainty with gastrointestinal, epileptic, myoclonic, respiratory or functional presentations. METHODS: This clinically focused narrative synthesis identified relevant literature through targeted searches of PubMed/MEDLINE, Scopus and Web of Science from database inception through March 2026, supplemented by a targeted PubMed/MEDLINE and citation-chasing update during

Acute Calculous Cholecystitis With Sinus Bradycardia: Cope's Sign Encountered.

Acute cholecystitis and cardiac ischemia can have a similar clinical presentation in some patients. Nonspecific electrocardiographic changes and arrhythmias can occur in acute cholecystitis and gallbladder disease that can confuse the treating physician leading to extensive cardiac workup. Emergency physicians and cardiologists should be aware of these changes so they can expedite the management of acute cholecystitis, which can lead to the resolution of these changes. We report a case of a 53-year-old male who presented with diffuse abdominal pain, nausea, and vomiting. His ECG showed sinus b

[Intestinal gases and digestive pathology: clinical syndromes and their treatment].

A number of annoying and frequent disturbances caused by an increase in intestinal meteorism have aroused the interest of students since the last century. Aerophagy, abdominal distension, flatulence represent the commonest aspects of this frequent gastroenterological pathology. The new diagnostic approaches have made it possible to classify the principal mechanisms involved in the production and elimination of intestinal gases. It has thus been possible to study with greater precision the composition of gaseous content and the origin and removal of intestinal gases. It is possible to demonstra

Nurse-led respiratory care bundle and pneumonia prevention in critically ill patients with abdominal sepsis: a multicenter before-and-after controlled study.

BACKGROUND: Patients with abdominal sepsis face an increased risk of pneumonia due to systemic inflammation, prolonged bed rest, impaired cough reflex, and respiratory muscle dysfunction. Pneumonia increases mortality, delays recovery, and prolongs hospital stays, underscoring the value of pulmonary care. This study evaluates the association between a nurse-led respiratory care bundle and pneumonia incidence and early prognosis in patients with abdominal sepsis. METHODS: A comparative study of patients with abdominal sepsis in two stages was conducted from January 1, 2021, to June 30, 2023 (c
