[Is there a future for the Excimer laser in refractive surgery?].

The refractive surgery concerns all the surgical procedures implicated in the refractive power change of the cornea. Its clinical results are just known, and a new physical procedure is becoming capable to replace surgery: the Excimer laser. Without any instrumental contact with the corneal surface, the laser beam is able to remodel the corneal tissue, and to treat astigmatism, myopia, hypermetropia. Millions of people could be treated by such a laser, and could leave their glasses. Biological effects on rabbit and monkeys are presented. An argon fluoride excimer laser (193 nm) with a moving s

Corneal and Whole-Eye Higher Order Aberrations Do Not Correlate With Ocular Residual Astigmatism in Prepresbyopic Refractive Surgery Candidates.

PURPOSE: Ocular residual astigmatism (ORA) is the proportion of manifest astigmatism that is not explained by anterior corneal astigmatism. The role of higher order aberrations (HOAs) in the level of ORA has not been profoundly studied. The purpose of this study was to evaluate the effect of different corneal and whole-eye HOAs on levels of ORA using a multivariate modeling approach. METHODS: This is a retrospective analytical study including a sample of healthy refractive surgery candidates. One eye of every patient was randomly selected. A total of 294 eyes from an equal number of patients

Cataract surgery in setting of ocular surface disease.

PURPOSE OF REVIEW: The purpose of this article is to review the available strategies to successfully identify and manage ocular surface disease (OSD) pre, intra and post-cataract surgery. RECENT FINDINGS: Cataract surgery and OSD have an intricate relationship: the surgical procedure can induce or exaggerate OSD symptoms, while OSD can negatively impact surgical refractive outcomes and increase the rate of postoperative complications. SUMMARY: Improving the health and stability ocular surface is the key to enhance post cataract surgery refractive outcomes and avoid complications. This is piv

Use of mitomycin-C for phototherapeutic keratectomy and photorefractive keratectomy surgery.

PURPOSE OF REVIEW: Experience with mitomycin C (MMC) application during corneal surface ablation procedures such as phototherapeutic keratectomy and photorefractive keratectomy has grown over the last decade. This review will highlight our understanding of the utility, mechanism of action, and safety profile of MMC. RECENT FINDINGS: MMC, although still off-label treatment for ophthalmic surgical uses, is now widely employed for photorefractive keratectomy corrections of greater than four to six diopters. The last few years have seen the refractive surgery community embrace its use for virtual

BCLA CLEAR Presbyopia: Management with corneal techniques.

Corneal techniques for enhancing near and intermediate vision to correct presbyopia include surgical and contact lens treatment modalities. Broad approaches used independently or in combination include correcting one eye for distant and the other for near or intermediate vision, (termed monovision or mini-monovision depending on the degree of anisometropia) and/or extending the eye's depth of focus [1]. This report reviews the evidence for the treatment profile, safety, and efficacy of the current range of corneal techniques for managing presbyopia. The visual needs and expectations of the patie

[Astigmatism].

Astigmatism is the most frequent refractive error worldwide followed by hyperopia and myopia. Internal astigmatism has to be differentiated from external astigmatism. Furthermore, external astigmatism can be divided into "with the rule", "against the rule" and "oblique". The summation of internal and external astigmatism results in the refractive cylinder. Astigmatism has for a long time been regarded as a two-dimensional phenomenon; however, only a three-dimensional consideration expanded the view on existing analytical methods (topography and tomography). Alpins' vector analysis is a commonl

Intracapsular infection after small-incision lenticule extraction.

A 40-year-old man had uneventful small-incision lenticule extraction. Three days later, the uncorrected distance visual acuity was 20/20 and the cornea was clear without intraocular inflammatory in both eyes. At 6 days, the patient presented with a foreign-body sensation in the right eye but with no decrease in vision. Dense infiltration was visible at the inferior nasal corneal rim on slitlamp microscopy. Anterior segment optical coherence tomography showed the lesion in the junction of the capsular bag and the lenticular edge. Confocal microscopy showed a large number of inflammatory cells a

The current and future status of refractive surgery.

Kerato-refractive surgery has developed rapidly over the past decade. Many surgical procedures have been described for the correction of myopia, hyperopia, and aphakia. For the treatment of myopia, radial keratotomy has been utilized extensively. Most investigators agree that radial keratotomy results seem to be better in patients with 3 diopters or less of myopia. Keratomileusis is a complicated procedure which can correct high degrees of myopia, however, this technically difficult procedure is performed by very few surgeons in North America. Epikeratophakia for the treatment of aphakia is pr

Oculoplastic considerations for refractive procedures.

PURPOSE OF REVIEW: Refractive surgery is one of the most popular elective procedures performed in the world. Given that dry eye is a common complaint following keratorefractive surgery, evaluation, and treatment of periocular conditions that further predispose the patient to dry eye symptoms is an important part of the presurgical assessment. Periocular conditions and surgeries can also affect the ocular surface and keratometry, and should be addressed. For example, ptosis, orbital fat herniation, ectropion, and eyelid masses have been shown to induce corneal topography changes and astigmatism

A comparison of induced astigmatism in 20- vs 25-gauge vitrectomy procedures.

INTRODUCTION: Surgically induced astigmatism is an unwanted variable that can lead to poorer visual and refractive outcomes in patients undergoing vitrectomy even when a technically precise procedure has been performed. This study assesses the difference in surgically induced astigmatism (SIA) between the traditional 20-gauge vitrectomy and the newer 25-gauge sutureless technique by comparing pre- and post-procedure keratometry readings. METHOD: The study is a retrospective consecutive case series of vitrectomies performed by a single surgeon. There were a total of 47 patients, eight with bil

Aqueous Misdirection After Trabeculectomy in a Down Syndrome Patient With Angle-closure Glaucoma.

Down syndrome is a genetic disease caused by trisomy of chromosome 21 that is characterized by numerous systemic abnormalities including intellectual disability, stereotypical facies, and congenital heart malformations. Ocular abnormalities are commonly seen with Down syndrome including corneal disease (keratoconus), refractive error, and atypical irides (Brushfield spots). We report the first case of aqueous misdirection in a patient with Down syndrome after trabeculectomy. Patients with Down syndrome often have small, hyperopic eyes with narrow iridocorneal angles and may be at increased ris

Complications and challenges of refractive surgery.

PURPOSE OF REVIEW: This review provides an overview of possible complications of the most commonly performed refractive surgeries: laser-assisted in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), small incision lenticule extraction (SMILE), and lens-based refractive surgery. While the vast majority of refractive surgeries are safe with successful outcomes, complications can still occur which requires surgeons to have an in-depth understanding of complication prevention and management. RECENT FINDINGS: Although overall rare, corneal laser refractive procedures and lens-based r
