Unsuccessful Clinical Trials in Retina: Lessons Learned.

PURPOSE: To analyze patterns of nonsuccess in phase 2 and 3 interventional retinal clinical trials conducted in the United States between 2015 and 2025. DESIGN: Perspective based on retrospective analysis of unsuccessful clinical trials. METHODS: Unsuccessful trials that were terminated, withdrawn, or completed without meeting primary endpoints were evaluated across indications, trial phases, intervention classes, and mechanisms of action to identify recurring design, feasibility, and operational challenges. RESULTS: High attrition rates were observed across diabetic macular edema, diabetic…

Retina microrheology via oscillatory atomic force microscopy.

Viscoelastic properties of tissues, including elasticity and viscosity, are crucial for understanding development and disease progression. However, traditional atomic force microscopy (AFM) indentation methods provide limited insight into these complex tissue properties. This study establishes microrheology via oscillatory AFM to assess both the elastic and viscous components of tissue mechanics. We first compared indentation AFM to oscillatory AFM on mouse retinal tissue and found that the Young's modulus of indentation AFM (956.8 Pa) was statistically similar to the elastic component (storag…

Retina-Inspired Models Enhance Visual Saliency Prediction.

Biologically inspired retinal preprocessing improves visual perception by efficiently encoding and reducing entropy in images. In this study, we introduce a new saliency prediction framework that combines a retinal model with deep neural networks (DNNs) using information theory ideas. By mimicking the human retina, our method creates clearer saliency maps with lower entropy and supports efficient computation with DNNs by optimizing information flow and reducing redundancy. We treat saliency prediction as an information maximization problem, where important regions have high information and low…

Novel artificial intelligence applications for pediatric retina.

PURPOSE OF REVIEW: This review examines the emerging role of artificial intelligence (AI) in the detection and management of pediatric retinal diseases, with a focus on systems that achieve expert-level performance in classifying fundus abnormalities. We highlight models developed for clinical application as assistive or autonomous tools with the potential to enhance early identification and referral, streamline care delivery, and improve access to care. RECENT FINDINGS: AI systems, have shown high diagnostic accuracy in identifying retinal pathology associated with retinopathy of prematurity…

Emerging computational motifs: Lessons from the retina.

The retinal neuronal circuit is the first stage of visual processing in the central nervous system. The efforts of scientists over the last few decades indicate that the retina is not merely an array of photosensitive cells, but also a processor that performs various computations. Within a thickness of only ∼200 µm, the retina consists of diverse forms of neuronal circuits, each of which encodes different visual features. Since the discovery of direction-selective cells by Horace Barlow and Richard Hill, the mechanisms that generate direction selectivity in the retina have remained a fascinati…

Heidelberg retina tomograph in ocular Behçet's disease.

PURPOSE: To compare the optic disc topography of patients with ocular Behçet's disease (BD) with age-matched controls, using Heidelberg retina tomograph (HRT). METHODS: This study included 32 patients (51 eyes) with ocular BD (active and/or inactive), 38 patients (74 eyes) with nonocular BD, and 62 normal subjects (62 eyes). All patients and control group underwent complete ophthalmologic evaluation. Intraocular pressure was less than 22 mmHg in patients and in the control group. The optic nerve heads of all subjects were imaged with the HRT (software 2.01a-M). The following stereometric para…

Myopia-aphakia. II. Vitreous and peripheral retina.

Biomicroscopical examination of the vitreous and peripheral retina was performed on 103 aphakic eyes with myopia of at least -6-0 dioptres. Retinal breaks were found in 19 eyes (18-4 per cent). Posterior vitreous detachment was present in all but one of the eyes. The high prevalence of retinal breaks in aphakic eyes with high myopia is compatible with the high incidence of retinal detachment in such eyes.

[Angioid streaks of the retina: diagnosis and treatment].

Angioid streaks (AS) are irregular breaks in a calcified Bruch's membrane that appear as pigmented linear lesions radiating from the optic disc. In approximately 50% of cases, AS are associated with systemic diseases such as pseudoxanthoma elasticum, Paget's disease, various hemoglobinopathies, and they may also be idiopathic. Visual impairment in patients with AS is primarily related to the development of choroidal neovascularization along the streaks, followed by retinal pigment epithelium atrophy. Diagnosis of AS is based on clinical examination and modern imaging methods. This literature r…

The electroretinogram as a means to study the physiology of the retina.

The electroretinogram (ERG) is an important assay for retinal physiology. This noninvasive recording can be used for basic and disease-related research. This chapter addresses ERGs, recorded in humans and nonhuman primates, for assessing retinal physiology and for clinical application. We will also describe ERG recordings in mice that are often used for studying basic retinal processes and for preclinical research. The basics of ERG recording techniques and experimental procedures will be discussed. A brief description of the standard full-field stimuli and their corresponding ERG responses is…

Aging reprograms the rhythmic transcriptome in the retina.

PURPOSE: Aging is a significant risk factor for various retinal degenerative diseases. However, its impact on diurnal transcriptional profile of the retina remains largely unexplored. Addressing this knowledge gap is crucial given the rising prevalence of age-related retinal diseases. METHODS: We conducted high-throughput RNA sequencing on retinal samples collected at 4-h intervals (n = 24 per group) from young (6-week-old) and aged (20-month-old) C57BL/6 mice maintained under a 12h:12h light-dark cycle. Differential expression analysis was performed using edgeR (FDR <0.05, |FC| > 1.5). Rhyth…
