Fistula risk score-adjusted comparison of postoperative pancreatic fistula following laparoscopic vs open pancreatoduodenectomy.

BACKGROUND: To evaluate a risk-adjusted comparison of clinically relevant postoperative pancreatic fistula POPF (CR-POPF) following laparoscopic pancreatoduodenectomy (LPD) vs open pancreatoduodenectomy (OPD) using the fistula risk score (FRS). METHODS: We retrospectively analyzed 579 patients who underwent LPD (n = 274) or OPD (n = 305) between 2012 and 2019 at two tertiary hospitals. Using the FRS, the risk was stratified into four categories; negligible, low, intermediate and high risk. RESULTS: The median FRS was significantly higher in the LPD than in the OPD group (5.4 ± 1.2 vs 3.9 ± 1

A new tool for identifying risk of repeated intimate partner violence adjusted for the population of Montenegro: a cohort study.

Current estimates suggest that approximately 20% of women and 11% of men have been victims of intimate partner violence. Risk identification tools for repeated domestic violence currently exist and have typically been designed by police and judicial authorities, based on the criminal history of the perpetrator. A follow-up cohort study of 238 cases of intimate partner violence (judicially processed and officially judged as DV according to Montenegrin Criminal Act) was conducted by reviewing randomly selected court files. The results showed that certain perpetrator characteristics (unemployment

Improved fracture prediction using different fracture risk assessment tool adjustments in HIV-infected women.

OBJECTIVES: A fracture risk assessment tool (FRAX) using clinical risk factors (CRFs) alone underestimates fracture risk in HIV-infected men. Our objective was to determine whether accuracy of FRAX would be improved by considering HIV as a cause of secondary osteoporosis, and further improved with addition of dual-energy X-ray absorptiometry parameters in HIV-infected women. DESIGN: Subgroup analysis of Women's Interagency HIV Study. METHODS: We included 1148 women (900 HIV-infected and 248 uninfected) over age 40 with data to approximate FRAX CRFs and 10-year observational data for incident

Estimated Cardiac Risk Associated With Macrolides and Fluoroquinolones Decreases Substantially When Adjusting for Patient Characteristics and Comorbidities.

BACKGROUND: Some studies have found that antimicrobials, especially macrolides, increase the risk of cardiovascular death. We investigated potential cardiac-related events associated with antimicrobial use in a population of patients with acute myocardial infarction. METHODS AND RESULTS: For 185 010 Medicare beneficiaries, we recorded prescriptions for azithromycin, clarithromycin, levofloxacin, moxifloxacin, doxycycline, and amoxicillin-clavulanate. In the following week, we recorded death, acute myocardial infarction, atrial fibrillation or atrial flutter, a non-atrial fibrillation/atrial f

Decreased risk adjusted 30-day mortality for hospital admitted injuries: a multi-centre longitudinal study.

BACKGROUND: The interpretation of changes in injury-related mortality over time requires an understanding of changes in the incidence of the various types of injury, and adjustment for their severity. Our aim was to investigate changes over time in incidence of hospital admission for injuries caused by falls, traffic incidents, or assaults, and to assess the risk-adjusted short-term mortality for these patients. METHODS: All patients admitted to hospital with injuries caused by falls, traffic incidents, or assaults during the years 2001-11 in Sweden were identified from the nationwide populat

A case study in generic drug use: should there be risk adjustment in incentive payments for the use of generic medications?

BACKGROUND: Encouraging generic drug use has reduced health care costs for payers and consumers, but the availability of therapeutically interchangeable medications or generic medications of choice is not equal across disease states. The extent to which systems of care are able to substitute with generics is not well understood. OBJECTIVES: To (a) define and measure the maximum generic rate (MGR) of currently prescribed drugs within an academic medical group in and (b) illustrate differences across drugs associated with selected underlying diseases. METHODS: Prescription claims data were exa

Adjustable vs set-pressure valves decrease the risk of proximal shunt obstruction in the treatment of pediatric hydrocephalus.

INTRODUCTION: The use of programmable shunt valves has increased dramatically in the practice of pediatric hydrocephalus. Despite theoretical advantages, it remains unclear if the use of programmable vs set-pressure valves affects shunt outcome. MATERIALS AND METHODS: The clinical and radiological records of all pediatric patients undergoing ventriculoperitoneal (VP), ventriculopleural (VPl), and ventriculoatrial (VA) shunt surgery from 2001 to 2004 at an academic institution were reviewed. The association of programmable vs set-pressure valves with subsequent shunt revision was assessed by K

Stock health analysis and forecasting using risk-adjusted metrics and LSTM-guided Monte Carlo simulation.

The complex and nonlinear behaviors of financial markets make it exceedingly challenging for average investors to evaluate the performance of a specific asset or make meaningful predictions about its future behavior. Investing in stocks involves various evaluation techniques. However, many of these techniques are restricted to a small set of variables or measurements that are static and historical, limiting their ability to assess the evolving risk-return tradeoff. The objective of this research is to develop a more sophisticated technique that integrates stock evaluation and forecasting by co

Educational approaches to the relationship between cervical adjusting and risk of cervical artery dissection; a survey of chiropractic faculty.

BACKGROUND: Cervical artery dissection (CeAD) remains a rare but clinically important condition that has generated longstanding debate regarding its potential association with cervical spinal manipulative therapy (cSMT). Although biomechanical and epidemiologic studies have examined this relationship, little is known about how chiropractic educational programs address CeAD or how individual faculty member's perceptions shape instructional practices. This study surveyed chiropractic faculty members who actively teach cervical manipulation to understand how educators conceptualize CeAD risk and

Adjustable Versus Fixed Loop Devices and Revision Risk After Primary ACL Reconstruction With Hamstring Autograft.

BACKGROUND: Adjustable loop devices (ALDs) have become popular for graft fixation in anterior cruciate ligament reconstruction (ACLR) because of their combination of convenience and biomechanical strength. Their design introduces a new mechanism of failure (loop lengthening), which is not present in fixed loop devices (FLDs). PURPOSE: To identify the association between use of ALD and risk of revision and reoperation after hamstring tendon autograft ACLR compared with other femoral fixation devices. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Data from Kaiser Permanente's ACL

Weight-adjusted waist index as a practical predictor for diabetes, cardiovascular disease, and non-accidental mortality risk.

BACKGROUND AND AIM: Identifying a more suitable marker among various measures of adiposity, demonstrating strong associations and predictive ability for clinical use, remains a topic of debate. Weight-adjusted waist index (WWI) has been proposed as a novel index of adiposity, yet its exploration is limited, especially in Chinese populations. This study seeks to examine the associations between body mass index (BMI), waist circumference (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHTR), weight-adjusted waist index (WWI), waist circumference divided by body mass to the power of 0.333
