A 13-year-old boy presented with persisting abdominal pain after a skateboard accident. Primary clinical and laboratory findings disclosed no signs of intra abdominal bleeding. Ultrasound scanning indicated rupture of the spleen which was confirmed by acute exploratory laparotomy.

Spontaneous or traumatic ruptures almost always occur in a cuff that has degenerated due to overuse. Despite the lack of absolute anatomico-clinical correlation, two clinical types can be described: a conflictual, often hyperalgesic shoulder with its painful arch corresponding to a small anterosuperior rupture, or a pseudo-paralytic shoulder betraying a major posterosuperior rupture. Standard radiography with Leclercq's manoeuvre is performed primarily to find out whether the cuff is continent or incontinent. Modern medical imaging (ultrasonography or MRI) adds precision to the lesions when su…

In comparison with injuries sustained in adult life, ruptures of the fibular ligaments before growth is complete are more frequently characterized by chondral, osseous and periosteal loosening, which was seen up to the 14th year of age in about 50% of our own population of 129 patients up to the 16th year of life. The average age of patients with osseous and chondral lesions was clearly lower than that of those with periosteal injuries, which was lower than that of patients with inter-ligament ruptures. Accidents at school and during school sports classes were the most frequent cause, being in…

Eardrum (tympanic membrane) rupture in humans and animals in relation to various blast pressure-time patterns was reviewed. There were few systematic studies on eardrum rupture as a consequence of blast overpressure. Most reports did not describe the area of the eardrum destroyed. The peak overpressures required to produce a 50% incidence of eardrum rupture (P50) were summarized. Most of the animal data pertained to dogs. The highest P50 for dogs, 296 kPa, was associated with smooth-rising overpressure. For complex wave patterns occurring inside open shelters subjected to nuclear blasts, the P…

Infectious mononucleosis is an acute viral illness associated with a high incidence of splenomegaly, although the incidence of splenic rupture is low. When rupture occurs, the mortality has been significant, presumably, because a history of trauma is not present. The spleen may be vulnerable to injury due to the histopathologic changes that occur as a result of this illness. Essentially all patients with spontaneous rupture related to infectious mononucleosis have epigastric or upper abdominal pain. The diagnosis of splenic rupture may be confirmed in a variety of ways. In those patients who a…

Although theoretically important, the bulk of obstetric literature indicates that scar separation following a lower transverse uterine incision is not a significant problem in clinical obstetrics. The need for emergency intervention for such scar separation is not increased over that in any laboring patient for a number of other causes. Ideally, the capability of emergency intervention should be available for any laboring patient. In reality, however, such a situation will not commonly be present in all hospitals in the United States. The absence of in-house anesthesia coverage does not appear

The authors report a case of rupture of the right cornu of the uterus in the ninth month of pregnancy in a woman who had had her right tube implanted into her uterus. This complication is considered, in the discussion, to be rare. It is very difficult to prevent it happening during pregnancy. It would seem that a prophylactic caesarean operation should be carried out if the insertion of the placenta is in the area of the implantation.

We report a case of previously healthy male with hypotension and cyanosis with no evidence of underlying congenital heart disease, pulmonary hypertension, pulmonary artery aneurysm, or central venous or pulmonary artery catheter insertion. Autopsy revealed a spontaneous rupture of the pulmonary artery with hemopericardium.

Eleven cases of tracheobronchial rupture are described. Nine were the result of external non-penetrating trauma and all but three had other serious injuries. The remaining two were caused by endobronchial intubation. Of the cases caused by external injury, respiratory tract injury was confined to the cervical trachea in three. Two required tracheostomy and repair and the third was managed conservatively; all made satisfactory recoveries. Intrathoracic rupture was recognised on or soon after admission in three cases. One patient died of uncontrollable pulmonary haemorrhage before he could be op…

Two cases of spontaneous rupture of the kidney manifesting as acute abdomen are described. In both cases urgent transabdominal nephrectomy was carried out. The whole distal half of the kidney was tumorous and rupture, with extensive perirenal and retroperitoneal haemorrhage. Histological findings showed in the first instance renal liposarcoma, in the second instance an atypical lipoma of the kidney. The first patient survives 4 years, the second one year, after the operation both are in a good condition.

A patient with Brucella arthritis presented as 'pseudothrombophlebitis syndrome', with rupture of knee synovial membrane into the calf. Bacteriological identification of the same organism from the patient's blood, knee effusion and calf-swelling aspirate suggested the diagnosis, and ultrasonography of the calf swelling gave supportive evidence. Synovial rupture into the calf in infective arthritis is a rare occurrence and this has not been reported previously in Brucella arthritis.

Complete avulsion of the female urethra secondary to blunt trauma is uncommon. It is associated with pelvic fractures, and because of the close association of the urethra and vagina a vaginal laceration also occurs. The paucity of lesions associated with pelvic fracture may be explained by the relative mobility and shortness of the urethra in the female. The treatment of urethral trauma in females has not been established. Vaginal, transpubic, or retropubic approaches have been used successfully. We report on 3 cases of urethral trauma with anterior vaginal lacerations treated by retropubic ap

Spontaneous splenic rupture in hemophilia A is an uncommon occurrence which is difficult to diagnose. We have reported a case of hemophilic rupture of the spleen unaccompanied by thrombocytosis, which has been emphasized as a diagnostic clue to hemophilic splenic rupture. Hence, splenic rupture should still be considered in any hemophilic patient with pain in the left upper quadrant and a normal platelet count. Splenic rupture in hemophilia must be recognized because its treatment requires not only clotting factor infusion but also surgical intervention.

This paper reviews 58 patients with traumatic rupture of the diaphragm treated at the Royal Adelaide Hospital during an 11-year period. The series appears to be the largest reported from one centre. The majority of cases resulted from blunt trauma to the trunk caused by motor vehicle accidents. More than half these patients sustained multiple injuries. A smaller group of patients had penetrating injuries to the diaphragm from gunshot or stab wounds. Clinical diagnosis of the ruptured diaphragm was seldom possible; an erect X-ray film of the chest was the best clue to diagnosis. In the acute ph…
