INTRODUCTION: Untreated ununited scaphoid fractures will almost inevitably progress to radiographic and symptomatic osteoarthritis of the wrist. This may lead to subsequent morbidity and lifelong disability especially in young males in which the fracture scaphoid is more common.

BACKGROUND: The scaphoid fractures account for 50%-80% of all carpal bone fractures in young individuals. Non-union of the fracture occurs in approximately 5%-10% of undisplaced scaphoid fractures. Current management varies significantly among different places and surgeons.
OBJECTIVES: The purpose of this review is to investigate the evidence of the effectiveness and safety of various treatments of acute scaphoid fractures.
METHODOLOGY: Systematic review and metanalysis of all the randomised and quasi-randomised trials comparing different treatments of acute scaphoid fractures.

Currently described sources of bone graft, such as iliac crest and distal radius, for supplemental fixation of scaphoid fractures are suboptimal. In our experience, olecranon bone has the advantage of providing a convenient source of corticocancellous block graft that can be harvested within the same sterile operative field used for fixation of the scaphoid fracture, and it also causes less postoperative pain compared to that obtained from iliac crest. Here, we describe our surgical technique for harvest and use of olecranon bone graft for fixation of scaphoid fractures.

The authors present their experience with treatment of scaphoid non-unions by the operation according to Matti-Russe. They compare their results with those of other authors and present a relatively high number of successfully treated patients with stable non-unions localized in the midline of the scaphoid bone as well as a high patient satisfaction rate with this method of treatment. Authors describe the results of the partial selective denervation from radio-volar exposure of the wrist, simultaneously with the osteosynthesis of the pseudoarthrosis of the scaphoid with Ender plates. After a 1-8 years follow up, 3/4 of the patients whose scaphoid pseudoarthrosis was 6.5 years old in average, and had at the operation middle or severe arthrosis of the wrist became symptomless or had only mild symptoms. It is stated that the denervation may be a good supplement of the scaphoid pseudoarthrosis operations and of sanative character performed in the arthrotic stage already.

A combination of forces makes the scaphoid bone susceptible to fracture. The common mechanism of trauma is a force applied to the palmar aspect of the wrist while it is in extreme dorsiflexion. One hundred fractured carpal scaphoids were immobilised alternatively in above-elbow and below-elbow casts. Preventing the pronation and supination of the forearm did not reduce the immobilisation time, which, with either type of cast, averaged 7 weeks, after exclusion of the fractures with delayed union. These eight fractures, which did not unite in 3 months, were operated on using a lag screw fixation, whereafter consolidation was achieved.

Scaphoid nonunions are a complex challenge that are frequently encountered by hand surgeons. Because of the tenuous blood supply of the scaphoid and secondary deformities that occur as a result of delayed or unsuccessful treatment, the treatment of scaphoid nonunions frequently requires the use of internal fixation and supplemental bone graft.
