Hemorrhagic neuropathy is an extremely rare condition. This condition refers to bleed into or around a peripheral nerve, causing either an extra neural or an intraneural hematoma. When they do occur, it is usually due to iatrogenic/inherited coagulopathies or as a consequence of injections targeting nerves. We present a case of sciatic nerve palsy developed secondary to anticoagulant therapy (Warfarin). MRI imaging showed features of sciatic nerve hematoma following which warfarin was withdrawn. The patient showed symptomatic improvement and a follow up ultrasound of left thigh showed resoluti

OBJECTIVE: To investigate the effects of different doses of topical dexamethasone (Dex) on sciatic nerves with simulated compressive neuropathy.
METHODS: Thirty-two Wistar rats were divided into four groups of 8: Sham group: no compression of the sciatic nerve + no treatment; Saline: chronic compression of the left sciatic nerve for 4 weeks + saline; 0.8% Dex: chronic compression + 0.8 mg of Dex; 3.2% Dex: chronic compression + 3.2 mg of Dex. Two sponge strips soaked with saline or Dex were placed under and over the nerve for 30 min in both Dex groups. Mixed-nerve-elicited somatosensory evoke

A 73-year-old female patient who had severe neuropathic pain due to sciatic nerve hypertrophy with the Klippel-Trenaunay Syndrome has been presented. Localized hypertrophic neuropathy is in one region and characterized by concentric proliferation of Schwann cells around the axon. It is very rare in the absence of generalized hypertrophic neuropathy. Very little is known about the etiology and the course of this neuropathy. Klippel-Trenaunay-Syndrome (KTS) is a rare syndrome characterized by hemangioma, abnormalities of the venous and lymphatic systems, and limb enlargement due to soft tissue a

Sciatic nerve injuries associated with acetabular fractures may be a result of the initial trauma or injury at the time of surgical reconstruction. Patients may present with a broad range of symptoms ranging from radiculopathy to foot drop. There are several posttraumatic, perioperative, and postoperative causes for sciatic nerve palsy including fracture-dislocation of the hip joint, excessive tension or inappropriate placement of retractors, instrument- or implant-related complications, heterotopic ossification, hematoma, and scarring. Natural history studies suggest that nerve recovery depen

Ischemic neuropathy is common in subjects with critical limb ischemia, frequently causing chronic neuropathic pain. However, neuropathic pain caused by ischemia is hard to control despite the restoration of an adequate blood flow. Here, we used a rat model of ischemic-reperfusion nerve injury (IRI) to investigate possible effects of hepatocyte growth factor (HGF) against ischemic neuropathy. Hemagglutinating virus of Japan (HVJ) liposomes containing plasmids encoded with HGF was delivered into the peripheral nervous system by retrograde axonal transport following its repeated injections into t

Mirror-image allodynia is a mysterious phenomenon that occurs in association with many clinical pain syndromes. Allodynia refers to pain in response to light touch/pressure stimuli, which normally are perceived as innocuous. Mirror-image allodynia arises from the healthy body region contralateral to the actual site of trauma/inflammation. Virtually nothing is known about the mechanisms underlying such pain. A recently developed animal model of inflammatory neuropathy reliably produces mirror-image allodynia, thus allowing this pain phenomenon to be analyzed. In this sciatic inflammatory neurop

Two patients with prior prostate surgery sustained peripheral nerve injuries after transurethral collagen injection for the treatment of urinary incontinence. In the first patient, brief lithotomy positioning caused a gluteal compartment syndrome and sciatic neuropathy. In the second patient, obturator neuropathy was due to leakage of collagen along the course of the obturator nerve. This is the first report of peripheral nerve injury in patients undergoing transurethral collagen injection.

We investigated the anti-hyperalgesic effect of neutralizing antibodies (AB) to tumor necrosis factor-alpha (TNF) in two murine models of neuropathy, the chronic constrictive sciatic nerve injury (CCI) which has a strong epineurial inflammatory component, and the partial sciatic nerve transection (PST), a 'pure' nerve injury model. In both models a single AB injection intra-operatively as well as on day 4 reduced thermal hyperalgesia significantly, whereas mechanical allodynia was only reduced with intraoperative but not with delayed treatment.

INTRODUCTION: Anatomical variations of the sciatic nerve are of significant clinical importance due to their implications in hip surgery, regional anesthesia, and the evaluation of sciatic neuropathies. The most widely accepted description of sciatic nerve-piriformis muscle relationships is the Beaton and Anson classification, which encompasses six recognized patterns. However, rare anatomical variants exist that do not conform to this classification and may predispose patients to nerve injury or diagnostic challenges.
CASE REPORT: We report a rare, unclassified anatomical variation of the sc

Useful diagnostic techniques for the acute phase of sciatic nerve palsy, an entrapment neuropathy, are not well established. The aim of this paper is to demonstrate the diagnostic utility of magnetic sacral motor root stimulation for sciatic nerve palsy. We analyzed the peripheral nerves innervating the abductor hallucis muscle using both electrical stimulations at the ankle and knee and magnetic stimulations at the neuro-foramina and conus medullaris levels in a patient with sciatic nerve palsy at the level of the piriformis muscle due to gluteal compression related to alcohol consumption. Peripheral neuropathies are a recognized complication of labor in the post-partum period. Herein, we describe an uncommon presentation of sciatic mononeuropathy due to ischiofemoral impingement during labor. A 29-year-old, gravida 4 para 2, female presented post-partum with acute left lower limb paresthesia and left foot drop, following spontaneous vaginal delivery of twins. Neurological examination demonstrated no activation of the left sciatic-innervated muscles and sensory loss in the same distribution. Electromyography (EMG) demonstrated an acute complete left sciatic mononeuropathy. MRI o

This is a rare case of sciatic neuropathy in a 46-year-old man secondary to an intraneural ganglion cyst from the hip joint extending upward to the lumbar plexus and the L5 nerve root. Magnetic resonance neurography showed that the joint fluid passed through a labral tear and extended rostrally along the articular branch and the peroneal portion of the sciatic nerve to near the L5 nerve root. The condition was treated by arthroscopic disruption of the joint connection to the nerve with resolution of the intraneural cyst and partial neurological recovery.

The aim of this research is to study the effects of nimodipine, gabapentin, ketamine and imipramine in the partial sciatic nerve transection (PST) model of neuropathic pain in rats. PST was produced in young Wistar rats of either sex by partial destruction of the sciatic nerve. A decrease in the latency to paw withdrawal reaction on the hot plate was considered as development of neuropathy. The drugs were given daily from the third day of the procedure, and evaluation was done on days 7, 14, 21 and 28. There was a significant decrease (p < 0.05) in the paw withdrawal response in the nimodipine

Neuropathic pain remains a major complication of various forms of injury to peripheral nerves in humans. Recently, 2 models of peripheral mononeuropathy in the rat have been described which closely resemble the human condition. Bennett and Xie3 described one model induced by the placement of 4 loose ligatures around the entire sciatic nerve; Seltzer et al. have described a second model produced by the placement of a tight ligature around one-third to one-half of the sciatic nerve. It is the purpose of this work to compare the effect of these injuries on the time course and magnitude of hyperal

We report a case of ischemic neuropathy of the sciatic nerve in a patient with an anterior column fracture of the acetabulum operated by ilioinguinal approach. It resulted from occlusion of the blood supply to the sciatic nerve. There were no signs of a vascular insult until ischemic changes ensued on the 6(th) postoperative day on the lateral part of great toe. The patient underwent crossover femoro-femoral bypass grafting and there was a complete reversal of the ischemic changes at 6 months. The sciatic nerve palsy continued to recover until the end of 1 year; by which time the only deficit
